The Forensic Outreach and Liaison Service is a service for Kent, providing a safe, effective and timely pathway from medium/low secure care into the community, for people requiring ongoing specialist forensic mental health care following a secure inpatient admission. The service will provide ongoing care and support for those individuals requiring specialist intervention to address risks and psychological needs and works to support social care needs, which if these failed could potentially lead to relapse and reoffending.

The service is within the South East, Adult Secure Services, New Care Model and will work in parity and close partnership with Surrey and Sussex Forensic Services.

The post holder will work within the community forensic and liaison service, based at the Trevor Gibbens Unit with close support from the Mental Health Practitioners and Senior STR worker. We are currently seeking an enthusiastic and motivated Physiotherapist to join the Therapy Assisted Discharge service (TADS) at Maidstone Hospital.

TADS is a team of occupational therapists, physiotherapists and therapy assistants that enable early discharge from hospital by providing access to therapy assessment and rehabilitation at home within 24 hours of discharge from hospital. The team is part of West Kent Home First and works in collaboration with community health, social care and voluntary agencies.

TADS operates from 8am to 6pm seven days a week. The post therefore involves joining a seven day roster with opportunity for flexible working hours and enhanced rates of pay.
