The post holder will work as an autonomous practitioner and will have relevant post registration experience for the role and will undertake specialist clinical duties as required within a defined specialist clinical area. To work alongside members of the multidisciplinary team to deliver Occupational Therapy services to citizens within a defined specialist clinical area. This will include citizens with multi-pathology and complex needs, using evidence-based citizen centred principles to assess, plan, implement and evaluate interventions and maintain associated records. They will have responsibility for managing their own specialist clinical caseload. In addition, they will provide clinical supervision and support to Occupational Therapists, student and support staff. Responsibilities:
As a Social Worker in the Community Mental Health Team for Older People, your responsibilities will include:

• Assessment & Planning: Conducting assessments in accordance with statutory duties, and working collaboratively to develop care plans that help individuals achieve their personal outcomes.
• Case Management: Managing a caseload of individuals, coordinating care and support services in partnership with internal and external partners, and monitoring the progress of care plans.
• Co-production: Working with individuals and their carers to create care plans that focus on overcoming barriers and achieving the best possible quality of life.
• Supervision & Mentorship: Providing supervision to student social workers, mentoring junior staff, and participating in personal development reviews.
• Partnership Working: Collaborating with health and social care professionals, statutory, and voluntary services to ensure coordinated and effective care for service users.
• Safeguarding: Ensuring all safeguarding concerns are reported promptly, and maintaining compliance with Health and Safety legislation and Council policies.
• Professional Development: Engaging in training to maintain professional registration with Social Care Wales, with opportunities to pursue further qualifications, including Approved Mental Health Practitioner (AMHP) training.

Requirements:

• Recognised qualification in social work (Dip.SW, CSS, CQSW or equivalent).
• Social Care Wales registration.

On a daily basis have due regard for personal and colleague safety in relation to physical aspects of the job; this would include lone working and carrying of equipment. Ensure all team members work within the policy. Have regard to the workload of, and the pressure of, professional colleagues and junior team members, taking appropriate action as necessary.
