You will be an essential part of a larger clinical team. As the front door for CAMHS services in Berkshire weare responsible forthe assessment and signposting for support of allappropriate referralsinto CAMHS services.

KEY TASKS & RESPONSIBILITIES:

Clinical (CN)
1. 1. Take responsibility for managing patient caseload and following the OT process.
2. Diagnosis of specificcomplexconditions in relation to the OT process.
3. Provide functional treatment for both in-patients, out-patients, community-based patients, individuals and groups.
4. Plan treatment programmes covering all aspects of functional rehabilitation.
5. Assess patients’ activities of daily living, interpret the assessment results, and modify the treatment.
6. Work single-handedly in providing training in daily living skills for patients and following them up at home or into the community where appropriate.
7. Makecomplexclinical decisions regarding patient care.
• Provide clinical advice to the OT team on their given speciality.
1. Identify and complete the referral process to other disciplines as required.
2. Identify the need for and carry out standardised tests with the patients, interpret the findings and plan treatment.
3. Take responsibility for planning, undertaking and completing home assessments when required.
4. Assess, recommend and organise provision of adaptive equipment, minor adaptations, wheelchairs and services as appropriate to individual patients.
5. Demonstrate a sound understanding of clinical risk and reporting untoward incidents both verbally and in writing.
6. Demonstrate a physical ability to carry out OT interventions and therapeutic handling. (There is a frequent requirement to exert moderate physical effort for several short periods during a day.)
7. Responsible for the safe use of equipment used in carrying out OT duties, adhering to the department’s Medical Devices Policy – this includes competency and accuracy to use equipment.
8. Frequent exposure to unpleasant working conditions e.g. bodily fluids, including urine and faeces, unpleasant smells and occasional exposure to hostility in the form of verbal and physical aggression. There may also be exposure to vermin and parasites especially on home assessments.
9. Exposure to distressing circumstances e.g. patients with chronic disease and those receiving palliative care.
10. Communication (CO)
1. 1. Communicate relevant information on patients’ conditions and performance to the multi-disciplinary team (MDT).
2. Liaise with other hospital disciplines, as appropriate, and with community MDTs regarding care and progress.
• Instruct and guide staff in the use of therapeutic activities in relation to their given caseload.

• Communicate with palliative care patients and their relatives regarding their changing ability.
1. Use interpreters when required to ensure effective communication occurs with patients who are hard of hearing or do not understand / speak enough English for the purpose of consent and co-operation for treatment.

1. Maintain and organise records of patients’ treatments in line with department’s and College of Occupational Therapists’ (COT) guidelines.

The post holder will: 1. Be able to combine high level remote monitoring with telephone assessment for patients with short and long-term conditions. Intervene when markers for deterioration are evident. Subsequently, initiating medical and/or nursing/therapy treatments, by alerting and referring to primary care multidisciplinary teams and professionals. These actions will ensure that interventions for patients on the caseload are delivered in the community setting and prevent unnecessary hospital admission. 2. Work with partners in nursing and residential care to ensure optimum health outcomes for their residents. 3. Develop shared caseloads with Integrated Care System (ICS) Network by acting as an integral member of the community team, working with a shared vision, and developing caseloads in partnership with Integrated Care System partners. 4. Support the Digital Health Team Leader in their management role when required. PATIENTS CARE The post holder will: 1. Use advanced clinical skills to remotely monitor and assess the physical and psychosocial needs of a defined client group. 2. Use advanced generalist clinical skills to identify subtle changing health care needs of patients. Subsequently, being able to competently discuss treatment options with other generalists and specialists. 3. Demonstrate responsibility and accountability for the Digital Health clinical caseload and escalate care needs across the whole patient pathway when this is required. Review individualised care plans for patients with short- and long-term conditions on admission to Remote Monitoring, so that their condition remains as stable as possible and, where appropriate, hospital admission is prevented. 5. Work with partners in nursing and residential care to improve the health outcomes of the residents and so prevent unnecessary hospital admissions or extended in-patient care episodes. 6. Analyse complex patient situations establishing a therapeutic relationship to assist the individual patient to adjust to their illness and care with the Digital health service. 7. Use assessment tools/skills that will ensure an appropriate level of nursing or therapeutic intervention so that patients who present with complex needs are referred/escalated via the appropriate care pathway. 8. Discuss all treatment options with sensitivity, knowledge, and expertise. and to act as patient advocate and maintain patient confidentiality. 9. Deliver care that is patient centred, respecting diverse cultural backgrounds, and requirements. 10. Work in partnership with the patients to empower them to make informed choices about their healthcare and support choices. 11. Offer a supportive service to patients and their carers from diagnosis through all stages of the disease process, in conjunction with other healthcare professionals using a range of communication skills to manage care and information empathetically.
