Analyse highly complex information, interpret findings to produce recommendations and actions to be shared, which identify direction for reviews, learning opportunities and support systemic change Work closely with Care Group and/or Divisional management teams, in particular the Associate Directors of Nursing and Professional Practice, and Clinical Governance Coordinators, Ward Managers, Matrons and others as appropriate, regarding the implementation of actions required from patient safety events and reviews Communicate business sensitive and complex information to staff at all levels across the organisation Jointly, with other members of the Patient safety and Quality team and the wider clinical governance team, maintain a pleasant working environment and present a professional and helpful manner when dealing with colleagues and visitors. Work with colleagues to continually strive to improve the efficiency and effectiveness of the team Work in collaboration with the Trust Communications Team with regards to the cascade of patient safety information Foster effective working relationships with colleagues from the Medical Examiners and Coroners offices Seek out and develop a peer-to-peer working relationship with technical and data analytics colleagues locally and nationally For the senior managers holding accountability for Patient safety in the Trust, prepare, collate and circulate papers and reports demonstrating the value of data analytics for patient safety Prepare, collate and circulate papers and reports for Patient Safety and Quality related meetings. Attend meetings to provide support when requested Implement and co-ordinate actions from meetings on behalf of the Patient Safety and Quality team Plan and develop, with the Patient Safety Specialist, the design and implementation of tools to measure clinical effectiveness with regards to patient safety interventions. To proactively monitor and report on the process of QI initiatives Organise and participate in patient safety focussed campaigns Lead on, but in collaboration with other members of the Patient Safety and Quality Team, produce reports detailing statistics and trends in respect of: PSIRP-focussed analytics; patient safety actions; Duty of Candour compliance; clinical effectiveness; risk registers Liaise with relevant staff from the Divisions and provide feedback and recommendations for improved quality care ensuring the appropriate action planning process is fully implemented Lead specific projects as agreed Maintain the Trusts LRMS (DCIQ) and use it to enhance the Trusts ability to identify key themes and trend both from issues arising and potential future areas for review Develop and update patient safety policies and processes, where appropriate Keep up-to-date with local and national guidelines Maintain professional/personal development in line with local/national guidelines Liaise with the Patient Safety Specialist, Clinical Governance Coordinators, and senior personnel within the Divisions to co-ordinate and promote an ongoing patient safety quality improvement programme and a just and learning culture Hold responsibility for ensuring that DCIQ is being used to its full potential Work with colleagues to ensure the clinical effectiveness, and clinical audit systems and processes, are being used to their full potential Support budgetary control in relation to team expenditure Act as role-model for Clinical Governance Coordinators and other staff across the Trust, promoting patient safety awareness at all times Arrange events such as training, workshops and seminars, obtaining costings, arranging venues, delegate packs and attend to provide organisational and administrative support as required Manage members of the Patient Safety and Quality team Lead on maximising the use of DCIQ across the Trust Make available, as required, information stored on DCIQ Use knowledge of the systems to develop and build reports when requested for planned and ad hoc purposes Ensure the secure management and coordination of Patient Safety and Quality related, complex and confidential information Key working relationships These include, but are not limited to, the following: Patient Safety Specialist Patient Safety and Quality Facilitators Central Patient Safety and Quality Team Improvement and Innovation Team Divisional Associate Directors of Nursing and Professional Practice Patients, their families and/or carers Clinical Governance Coordinators Chief Nurse Medical Director Deputy Chief Nurse Associate Director of Patient Safety and Quality Operational Managers/Professional Leads Health and Safety Team Feedback and Engagement Team Medicines Governance Team
