Key responsibilities:

Contributing to the identification of the support needs of young people with High Needs

1. To manage relevant service improvement processes like project support and governance, to drive service-wide service improvement in line with executive and audit requirements.

MAIN RESPONSIBILITIES
1. The provision of direct practical assistance and support to the FRA, to enable them to carry out their SYFRA duties.

2. Arrange and maintain ongoing FRA meeting schedules in liaison with FRA Members, senior SYFR leaders and other relevant stakeholders.

3. Make and co-ordinate all the necessary arrangements for FRA meetings including: preparation of correspondence, agendas, minutes and other supporting documents.

Job Type: Permanent Full -Time

Salary: £24,962 Per Annum (Base salary £23,962 + £1,000 Unsociable Hours) - this role is also eligible for a one-off retention payment of £1000 upon successfully completing 9 months service*

The Care Co-ordinator will: utilise population health intelligence to proactively work with the MDT to ensure they are aware and notified of patients that would benefit from access to personalised care. work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN. raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision-making conversations. bring together all of a persons identified care and support needs from digital information, and explore options to meet these into a single personalised care and support plan, using excellent communication and organisational skills to liaise with other stakeholders as needed for the collective benefit of the patients. help people to manage their needs through answering queries, signposting to relevant services and ensuring that people have good quality written or verbal information to help them make choices about their care. Identify patient need and assist to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level. signpost people to gain access for personal health budgets where appropriate. provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches and other primary care professionals. Ensure awareness of and basic safeguarding processes are in place for vulnerable individuals. Care Co-ordinator Responsibilities: Ensure all mandatory training is completed on an annual basis including; safeguarding, confidentiality, equality and diversity, Cardio-pulmonary resuscitation etc. Ensure infection control guidelines are maintained. Work with a supervisor to take responsibility for developing own practical and theoretical competence, developing own reflective practice skills. Adhere to legislation, policies, procedures and guidelines both local and national, regularly attending workplace and staff engagement meetings. To function as the departments point of contact when dealing with day-to-day correspondence. Collect used equipment from service users in a safe and efficient manner in accordance with departmental and Trust procedures. Sort used equipment by decontamination category as specified by departmental procedures. Participate in daily housekeeping and equipment preparation checks in accordance with Trust and national standards. Undertake daily user tests & approvals as required on washer-disinfectors Undertake cleaning of medical devices by the use of washer-disinfectors or manually where required using personal protective equipment provided. Be able to recognise all surgical instruments and identify trays used within the trust once full competence training is completed. Examination of cleaned devices to validate cleaning processes. Assemble instruments and checking for safety and correct function in accordance with departmental procedures, specifications and best practice. Ensure cleanliness / follow clean room dress procedures and dress codes in the rest of the department and hospital. Package instruments as specified in the clean room. Maintain correct work patterns in dirty and clean zones of the sterile supply unit and conform to clean room requirements as per operational directives. Use of autoclaves to sterilize devices ensuring cycle runs meet prescribed parameters and undertaking daily user tests as required following training. Receipt of equipment requisitions from service users, preparation, dispatch and transportation to destination ensuring correct delivery of items. Maintain stocks of process / raw materials and identify any deficiencies to department supervisors. Assist with stock inventory checks in accordance with Trust and departmental procedures. Ascertain priorities during times of peak workload by discussion with departmental supervisors. Comply with departmental systems for signing off items assembled/checked and adhering completely to the use of tracking and traceability systems and documentation.
