This role is based at Old Tea Warehouse in High Wycombe.

Old Tea Warehouse is a medium-high risk supported accommodation service for vulnerable customers aged 18-55. This is a fast-paced, dynamic environment where customers have experienced homelessness and may have complex needs such as substance misuse, mental health, history of offending, etc.

You will be key in the day-to-day running. We don’t run care homes, so there’s no need to provide personal care. The priority in your role includes the safety and wellbeing of everyone in our accommodation, including our customers.

Working with people who have a range of needs, you could be doing any number of things. From supporting customers in conversations with calls from their GP, Social Services, or Benefits teams – to dealing with the emergency services including the Police and Ambulance, when the need arises.

You’ll log all incidents to make sure we have an accurate record of events. And when a customer leaves, you will clean and prepare their room, ready for the next occupant.

Overview
As a Social Care Assistant working within the Home Independence Team, you will be working out in the community to support residents that have been recently discharged from hospital or those who are living with a disability and need help to live independently and establish a routine to help them manage their lives day to day. The Home Independence service supports individuals to regain their independence following illness, disability, or recent stay in hospital. We provide a goal-based assessment for each client and alongside them to support the achievement of their goals, using technology-enabled care devices and occupational therapy aids to enable them to achieve independence.

The team works closely with healthcare professionals, carers, clients and their families to develop suitable packages of support, designed to enhance client independence and meet their needs. We supply advice on specialist equipment options, monitor client progress and make necessary adjustments if required. About the role
No two days will be the same as you meet a variety of people and support their individual needs. Some of the work you will carry out includes.

- Providing people with information and advice in relation to care and support and assistance in accessing care and support through various means.
- Working as part a CQC Registered service in the community, maintaining a high degree of autonomy and utilising own initiative in delivering compliant care and support independently.
- Ensuring an agreed plan meets individual’s care and support outcomes needs and to empower the individual to access early intervention and preventative services.
- Informing senior colleagues and adhere to safeguarding procedure of any concerns which arise in relation to the safety of vulnerable adults.
- Working in partnership with individuals, carers, service providers, voluntary sector and other professionals.
- Providing personal care for individuals, this may include but is not limited to dressing, toileting, washing and general housekeeping.

About you
