What you will be doing

You will join an inclusive and supportive team that works well together to provide care to people with an emerging or personality disorder, using your knowledge and skills to help people along their recover pathway towards leading a valued and fulfilled life.

Your day-to-day will include ward rounds, analysing behaviours and adapting care accordingly with colleagues, updating patient records, and providing overall care and support, which may include assisting with personal hygiene.

It can be quite a challenging environment at times, so a positive attitude, compassion, resilience and vigilance are important. But it is hugely rewarding to see people improve because of your involvement, knowing that you’ve made someone’s life better.

If you have a driving license you may be asked to drive for the service.

The shift pattern is 12.5 hours a day, 3-4 days a week including nights and weekends.

You will be working at The Chimneys Clinic, a 12 bedded service providing specialist care that is specifically designed to support women that have a diagnosis of an emerging or personality disorder.

Working alongside the multidisciplinary team at The Chimney’s, you will support with mindful practice, high levels of engagement and think on your feet to create activities to aid the recovery for each young woman within their journey.

The focus of the service is to enable the young women to work on ways to decrease harmful behaviours and replace them with effective strategies to enhance their lives and help them reach their personal life goals.

THE ROLE
As a Support Worker, you will work within a dynamic team, acting as a keyworker to young people, delivering tailored support packages. Key responsibilities include:

• Conducting initial assessments, safety planning, and action plans with young people.
• Supporting young people with essential living skills, including budgeting, cooking, and attending appointments.
• Managing housing and property tasks, including maintaining property standards and ensuring voids are minimized.
• Promoting engagement with education, training, or employment opportunities.
• Addressing challenging behaviors calmly and professionally, adhering to safeguarding protocols.
• Liaising with external agencies to provide holistic support to service users.
• Ensuring compliance with tenancy agreements and taking necessary action for non-engagement.

THE CANDIDATE
The ideal candidate will have previous experience in a similar role, supporting young people with diverse needs. Essential skills and experience include:

About me and the role

I live in my own home and need support with personal care, activities, preparing meals and housekeeping.

I am looking for a trusted companion to support me to improve my confidence, independence, communication, and general wellbeing while keeping me safe and helping me to enjoy life.

I like visiting country parks and cafes, puzzles, and movies, and see my family regularly.

We are looking for a positive, supportive, and proactive individual with experience of caring for people with additional needs.

The successful candidate will be employed by my sister as my budget holder and care manager and will become part of a small support team. Full training will be given through shadowing existing team members.

Tasks and duties

Food and drink

Preparation of meals / snacks
Working with the client to prepare his own drinks and meals where possible
Managing personal hygiene

Maintain personal care routines including support with washing, changing clothes, shaving and brushing teeth
Support with personal healthcare appointments as needed
Interests and community access

Support to explore different interests both inside and outside the home.
Support to access the local community to enjoy preferred activities
Maintaining the home

Support to ensure the clients home and garden is kept clean and tidy.
Encourage the client to engage in simple household activities to build confidence in this area
General

Develop and maintain routines that the client is comfortable with including day, and night routines.
Follow the client care plan and direction from the team leader.
Gently encourage communication through relationship building

The client needs 24-hour support and 9.5 hours of 2:1 care and is supported by a small team to promote relationship building.

Long Day shift: 07:00hrs – 20:15hrs

Short Day shift: 08:00hrs – 17:00hrs

Night Shift: 20:00hrs – 07:15hrs

(Some flexibility around hours possible if needed)

Shifts are agreed with the team leader and generally will rotate depending on team needs / preferences. There is likely to be opportunity to occasionally work more hours if desired. We are looking for a team member who is willing to cover some night shifts.

Terms and conditions

The Annual holiday entitlement will be equivalent to the work which would normally be done during 5.6 consecutive weeks.

Satisfactory references from two referees are preferrable but not essential.
Completion of a satisfactory probationary period of 6 months.

Main Duties of the Job: Formulate, implement, and monitor progress of the SUS and CC projects to ensure that programmes and project work streams are planned, documented, and managed effectively to assist in their successful delivery. This will also include responsibility for preparing documentation and tracking project plans and benefits. Provide administrative and project support to the Speaking Up Safely and Culture Change team. Organise and coordinate meetings, workshops, and events. Assist in the development and implementation of Speaking Up Safely and Culture Change strategies and initiatives. Maintain accurate records and documentation related to Speaking Up Safely and Culture Change projects. Liaise with internal and external stakeholders to ensure effective communication and collaboration.
