We are seeking a well-motivated Support Practitioners to join our outstanding service within the Intensive Support Team (Child and Adolescent Mental Health). These posts will support to children and young people with mental health difficulties within East Lancashire.

You will assist in delivering therapeutic care to children/young people under the direct supervision of qualified staff as well as delivering some of your role by call handling telephone calls from young people, families and referrers. Your main aim will be to provide care to families and young people with severe and complex mental health.

You will ideally have experience of working in a children's mental health environment. You will have effective communication/written skills in English, be a team player and be able to develop therapeutic relationships with children, families and the team. I.T skills are essential.

Access to private transport for work purposes is essential.

Provide and maintain the highest level of care and safety of children and young people under direct/indirect supervision.

The duties are a mixture of call handling and taking referral details; inputting straight onto the electronic record system, as well as some support worker shifts, working directly with children, young people and their families/carers.

To assist in the deliverance of therapeutic interventions that meets the needs of children, young people and families, including planning and implementing a package of care directly to the child, young person and family under indirect supervision from Extended Scope Practitioners and IST Clinical Coordinator.

Attend different sites to participate in direct assessments with qualified staff where indicated and on a rota basis.

Assist and contribute to the holistic assessment of children, young people and families including contributing to individual care plans, monitoring individuals and working clinically autonomously giving 1:1 contact on a regular basis.

To assist with the mental health assessment of children and young people who have been admitted to the wards or are in A&E following a suicide attempt or self harming behaviours on the self harm rota, and assist in the follow up as needed with other professionals involved in the initial assessment.

You will be a highly motivated and enthusiastic RN child, RSCN trained nurse ideally with some special needs school nursing experience, wishing to expand your knowledge and skills of working with children with additional and complex nursing needs. You will be responsible for the training and updating of education, staff using evidence based competency training packs, to carry out procedures which include administration of medication, enteral feeds, tracheostomy, suction, oxygen.

You will lead on the Initial Health Assessment and drawing up of care plans and work in partnership school staff, parents and carers in there implementation.

Contribute to the public health agenda through Personal Health and Social Education and health promotion activities.

You will work alongside the Senior Special Needs School Nurse in order to identify children in need and children in need of protection working in partnership with the families and initiate multi agency working where necessary and to take on-going responsibilities e.g. Lead Professional within the Common Assessment Framework as appropriate. Participate in report writing and attendance at Safeguarding Conferences, Reviews and Core Group meetings with support from the senior special needs school nurse.

Information Governance The post holder must keep up to date with the requirements of information governance; undertake mandatory training and follow NCPC policies and procedures to ensure that NCPC information is dealt with legally, securely, efficiently and effectively. It is important that the post holder processes personal identifiable information only in accordance with the NCPC Data Protection Act notification to the Information Commissioner. The post holder must check with the Data Protection Officer before creating new systems to process person identifiable information to ensure that this is carried out within the scope of the Data Protection Act 2018 notification. The post holder must manage the records they create or hold during the course of their employment with NCPC in an appropriate way, making the records available for sharing in a controlled manner subject to statutory requirements and agreed security and confidentiality policies, procedures and guidelines e.g. Data Protection Act 2018, Freedom of Information Act 2000, General Data Protection Regulations, Caldicott Guidelines NHS Confidentiality Code of Conduct 2003, and professional codes of conduct on confidentiality. The post holder must maintain the confidentiality of information about service user staff and organisational business in accordance with the new Data Protection Act 2018 and Caldicott principles. It is likely that the post holder will be in contact at some time with a form of information system, and therefore is responsible for implementing and maintaining data quality. The post holder, when making entries into records, must ensure that these are legible and attributable and that the record keeping is contemporaneous. It is essential that information recorded within records either on paper, in an electronic format or both paper and electronic is accurate, complete and relevant. General Duties will include the following:


· Prepare clinic case notes with specific attention to the data items required for commissioning purposes and data quality in line with the Trust’s Clinical Records Policy, ensuring that all appointment details are entered onto the Patient Administration System (PAS)

· Arrange when necessary the attendance of interpreter/link workers for patients who require communication assistance when attending clinics.

· Booking in patients attending outpatient clinics and issuing return appointments as directed by the consultant/nursing staff prior to the patient leaving the Department.

· Request and ensure delivery of health records for patients either attending a clinic or an admission, from all departments within this Trust and other hospitals.

· Prioritise own workload ensuring agreed timescales are met each day involving the provision of case notes.

· Adhere to departmental policies and procedures and participate in audits in order to maintain data quality and provide an efficient service

· Maintain the confidentiality of any information obtained regarding patients and be aware of Data Protection and Freedom of Information policy

· Work within the departmental shift patterns to cover the opening hours of the service, i.e. 8.00am – 8.00pm as and when required, including Bank Holidays

· Act as first point of contact for all patients and visitors to the Department, providing a comprehensive reception and clerical Service to patients, relatives, carers, consultants and their teams
