A Vacancy at South West London and St George's Mental Health NHS Trust.
We are looking to recruit an experienced band 7 Occupational Therapist, Social Worker or Nurse to work as a member of our established Adolescent Outreach Team. The team are really fortunate that we have an experienced full multi disciplinary representation, and high levels of retention of staff. The MDT are skilled and highly committed to supporting the wider network, parents and carers to support young people to remain at home, and if required to come to hospital facilitate a safe discharge and reduce length of stay.
The post-holder will work within AOT as a Senior Case Worker providing Care Coordination and Assessment of an agreed caseload of AOT clients, and offering expert consultation to the network and referrers.
The team are always looking to develop the service to meet the needs of the young people and their parents/carers, this role would be at the forefront of this. The team also has 2 band 6 roles, which are development posts and this role would be supporting these roles and inspiring our future senior clinicians.
The team has strong working relationships with our local inpatient service (Aquarius), SLP inpatient units (Snowsfields and BAU), Tier 3 community CAMHS, CAMHS Emergency Care Team (CECS), CAMHS Crisis Line and CAMHS Bed Management.
We would also consider this post as a development post opportunity for an experienced band 6 starting at a band 6 working in a supported way to meet milestones to reach criteria for a band 7.

The post-holder must be committed to effective team working and to the coherent delivery of the care this will include individual, family, group work and telephone support.

About our locations:
Springfield University Hospital, Tooting
Our largest, 19-ward site is in Springfield Village, created following our £150m investment into two new world class mental health facilities. A 32-acre public park, shops and cafes are coming soon. Close to shops, cafes and Tooting Bec on the Northern Line plus Earlsfield and Tooting rail stations and bus routes.
• To deliver timely and accessible assessment and treatment to young people aged 12-17, who have serious mental illness and who are in crisis.
• To work with young people and their families with highly complex problems, and to make appropriate useful formulations and provide interventions or recommendations. In addition to serious mental illness in the young person, there may be complicating factors such as high risk to self, parental mental illness, family dysfunction, homelessness, child protection, the effects of sexual abuse, co-morbidity and possible offending behaviour.
• To enable the effective management of these high risk cases within the community, co-working with Tier 3 where appropriate, avoiding in-patient admissions wherever possible and appropriate.
• To deliver a service that is at point of contact between the hours of 8am to 8pm weekdays and 11am to 7pm at weekends and bank holidays (this will be on a rotational basis).
• To provide specialist consultation regarding formulations, intervention and risk management to Tier 3 CAMHS colleagues and other agencies within the young person’s network.
• To play a key role in cases where in-patient admission is necessary, working closely with Aquarius colleagues to ensure that length of any admission is kept to a minimum.
• To liaise with colleagues in adult services and Early Intervention Services to ensure that effective transition arrangements exist for clients who are likely to require ongoing support after the age of 18.
• To participate fully in the referral and allocation process, advising on the appropriateness of referrals for outreach or inpatient nursing intervention.
• To care co-ordinate a small clinical caseload of highly complex and demanding cases, providing a care package which offers the most effective therapeutic response to adolescents and their families.
• To act as co-worker to fellow Care Coordinators, working jointly and collaboratively with other clinicians in the multi-disciplinary team.
• To devise, co-ordinate and deliver specialist assessment and interventions, drawing on a varied range of theoretical frameworks and treatment modalities.
