The successful post holder will be accountable to the Virtual Ward Lead Nurse and will have continuing responsibility for the supporting with aspects of nursing care given to patients with sub-acute conditions in identified community settings, without direct supervision. The Nursing Associate will be personally accountable for the nursing standards of such delegated duties. The Nursing Associate will provide evidence-based care, advice and information to patients and carers.
You will be employed by North Middlesex University Hospital (Enfield Community Services) with primary responsibility at NMUH Virtual Ward. At times the post holder may be expected to work across the wider Enfield Rapid Access Service.
You MUST hold a full UK driving licence and a have vehicle for business use.
• Providing care to patients either remotely or within a community setting, who require high quality holistic evidence-based care that is responsive, effective, and efficient in line with the virtual ward model.
• Demonstrate excellent communication and interpersonal skills
• Support our patients with assessment, treatment, and effective discharge from the service.
• Demonstrate high levels of care and compassion promoting independence, prevent hospital admission or admission to care, support early discharge from hospital helping people receive the care at home.
• Provide necessary support to individuals requiring ongoing care needs, monitor individuals remotely and deliver specific treatment/care plans and be directly involved in the wider MDT approach to admissions avoidance plan of care.
• Support remote monitoring of a patient's condition and escalation to a multidisciplinary team.
• Provides holistic person-centred care with the aim of preventing unnecessary hospital admissions in a variety of ways.
• Providing care to patients either remotely or within a community setting, who require high quality holistic evidence-based care that is responsive, effective, and efficient in line with the virtual ward model.
• Support our patients within a community setting providing timely assessment, treatment and effective discharge from the service.
• Prevent hospital admission or admission to care, support early discharge from hospital helping people receive the care wherever they are all home.
North Middlesex University Hospital NHS Trust is a medium sized District General Hospital based in Edmonton in Enfield, North London. We serve a diverse multicultural population largely from the London boroughs of Enfield and Haringey.
Over the last decade we have rebuilt almost all the hospital and now have facilities that we are proud to work in. We employ over 3,500 staff, more than half of whom live locally. We encourage apprenticeships from the local community and work closely with our local Health Watch.
We are primarily an emergency led hospital with more than 90% of our bed days being used for patients admitted via our emergency and ambulatory units. As well as offering everything you would expect from a major acute hospital, we have a number of tertiary services treating patients with HIV and Sickle Cell Disease and a large cancer and radiotherapy service. We also run our local community Sexual Health Clinics.
The Community division comprises of the 0-19 Service, Health Visitor, School Nursing services, that deliver the Healthy Child Programme as well as a BCG clinic for targeted babies and the Canterbury ward, that supports the acute wards. Also, they have the District Nursing Service division that delivers adult care in Enfield Community.
• Diabetes management for stable diabetes care, including insulin administration BM monitoring.
• Bowel management of stable long-term conditions management including enemas, bowel evacuations methods.
• Respiratory care of stable interventions in accordance with the care plan to include nebulizer, tracheostomy care.
• Measurement of the vital signs of blood pressure, pulse, O2 saturation, Urinary incontinence management including stable managed female catheter changes, blabber washouts, continence product reassessments.
• Urinalysis Wound care management for pressure ulcers, leg ulcers and stable predicable wound care management
• Ordering and monitoring of equipment for wound care management
• Complete venepuncture as required
• Complete Doppler readings as required
