This is a busy and interesting post that requires a high level of confidentiality, accuracy, responsiveness support the Clinical Service.

The post holder will administratively support an extensive range of functions within the clinical services, including Service Leads, supporting the two wards and there will be regular Cashier duties. The working hours for this post is Monday to Friday 9am to 5.00pm.

We are excited to be offering an inspiring role to join our team of Community Dietitians, Assistant Practitioners and administration staff working within our community services division.

This post is based within the Community Services Division with the main area of work being Longmoor House Reablement Service.

The post will also include follow up of the patients into the community setting. Enabling the post holder to support the patients with their transition home.

Longmoor House has 69 beds and replaces the Intermediate Care service provided on Ward 35 at LUHFT’s Aintree University Hospital site.

The facility includes Activities of Daily Living (ADL) kitchens, activity spaces as well as a range of single and dual occupancy rooms. It serves patients needing ongoing nursing, therapy, and reablement needs, working towards collaborative goals to support timely discharge.

The service provides step down capacity from acute trusts for medically stable and optimised patients, whose care and assessment can be continued in a non-acute setting until they are ready to go home. It will also be open to patients needing step up care from community settings that require reablement or convalescence where alternative out of hospital home care has been exhausted.

You will manage a diverse clinical caseload incorporating nutritional support, frailty, chronic diseases, oncology, neurology, and gastrointestinal disorders.



To provide specialist dietetic assessment, diagnosis, and treatment programmes to individual patients with complex needs.

Work with key members of multidisciplinary team (MDT) to provide, advice, support, and care plans to patients to support their reablement and return home.

Provide training, education, and specialist advice to qualified and nonqualified health care professionals in all areas of the specialised field.

Have an awareness of and implement clinical practice utilising the direction provided by National Service frameworks; NICE, Clinical Governance and DoH guidelines to ensure high quality evidence based best practice. Principal Responsibilities:
• Work independently and with the Senior Project Manager to provide project management expertise within the Informatics Merseyside, Partner and Non-Partner Organisations

The Paediatric Dentistry department is responsible for delivering secondary and tertiary specialist paediatric dentistry services. Our clinical space consists of 20 dental chairs and 5 side surgeries.

The appointee will be expected to undertake dental treatment as required, with or without sedation and general anaesthesia. Inhalation sedation and intravenous sedation is undertaken within the department. Comprehensive care and exodontia under GA are provided on site.

Specific Duties

Cleaning
To assist in monitoring and maintaining cleaning standards on the ward.
To carry out spot cleaning to ensure spillages are dealt with swiftly and efficiently.

Catering
To prepare and serve hot/cold snacks (e.g. toast) as requested – offering a 24-hour snack service.
To serve meals in conjunction with the nursing team, ensuring patients intake is known and that their dietary needs are met.
To provide eating assistance, e.g. cutting up food, placing food within reach, encouraging patients to eat.
To prepare hot/cold beverages including appropriate trolley/equipment.
To ensure all food and beverages are served at the correct temperature according to food hygiene regulations.
To prepare areas where food and beverages are served/consumed to ensure a pleasant environment for patients.
To wash all equipment/utensils used in preparation, regenerating, serving and consuming food and beverages.
To take responsibility for the ward kitchen, ensuring it is clean at all times, e.g. discarding out-of-date food, stock rotation.
To ensure refrigerated food is labelled and stored correctly as per policy.

As a leader of the inpatient multi-disciplinary team at Irwell ward, Clock View Hospital, you will provide clinical care to older adults who require specialist inpatient care. You will work closely with a wide range of clinical and professional disciplines in order to deliver holistic care by completing assessments of new admissions to Irwell ward and managing their treatment and ensuring safe discharges back to the community

The post holder will be responsible for completing assessments and formulating treatment plans for service users admitted to Irwell ward from the community. Irwell ward is a 17 bedded functional older adult complex care ward at Clock View Hospital. As a substantive Old Age Psychiatry Consultant on Irwell ward you will work closely with other members of the multidisciplinary team including Social Services, Occupational Therapy, Physiotherapy, acute hospital medical specialties and Primary Care. You will be the Responsible Clinician for patients admitted to Irwell ward and this will involve managing patients who have been detained under the Mental Health Act and therefore the post holder will require a sound knowledge of Mental Health Act law. This will include completing relevant mental health act documentation and completing capacity assessments as required.



The post holder will be expected to lead the multidisciplinary team at Irwell ward in a supportive manner in order to foster positive working relationships. The post holder will complete ward rounds throughout the week and provide clear communication and documentation in relation to assessment outcomes and management plans. They will also be expected to provide leadership and training to trainee doctors and medical students placed on Irwell ward. They will take an active part in the clinical work of the team and a lead role in the development of the service.

Exceptions would be needs based. Irwell ward’s catchment area covers Liverpool and South Sefton. There are two dementia inpatient units in the Trust and these are Fern ward at Leigh Moss Hospital in Liverpool and Kingsley ward at Hollins Park Hospital in Winwick, Warrington. At Clock View Hospital there are a total of six inpatient wards – Alt ward, Dee ward, Irwell ward, Morris ward, Newton ward and Prenton ward. Prenton ward is a Psychiatric Intensive Care Unit (PICU) and apart from Irwell ward the others are for adults of working age. In addition to the inpatient units there is a Community Mental Health Team, Psychology and urgent response mental health teams based at Clock View Hospital.Currently there is a long standing locum Consultant, Dr Sumanth Kumar, who is the Consultant Old Age Psychiatrist for Irwell ward.



The post holder will be responsible for the assessment and management of all service users admitted on to Irwell ward as well as supervision of trainee doctors assigned to the ward. There will be a need to work closely with the wider multidisciplinary team caring for the service users on the ward including the ward Manager, Nursing staff, Healthcare Assistants, Occupational Therapists, Physiotherapists, Psychologists, Social Workers and the Community Mental Health Teams. Discharge planning for inpatients is completed during ward rounds and there is close liaison which takes place with the relevant Community Mental Health Teams, with the service user’s allocated practitioner from the CMHT being invited to attend ward rounds in order to agree discharge plans and arrangements for smooth transitioning from the ward to the community in terms of care. The Trust has fully electronic patient record (Rio), prescribing (EPMA) and mental health act administration systems (Thalamos) in place.

Be responsible for identifying and resolving day-to-day management issues within the Service.

It serves a population of approximately 364,000 in an area of mixed residential, industrial and rural character situated on the edge of Merseyside between the open countryside of Cheshire to the south and Lancashire to the north.

Both Whiston and St Helens Hospitals have undergone a program of extensive redevelopment under a PFI scheme, the total cost exceeding £300 million, completed in 2011.

The Trust was awarded Teaching Hospital status in 2008 and takes in approximately 150 undergraduates annually from University of Liverpool Faculty of Medicine, years 2-5. It is an extremely popular venue for both undergraduates and Foundation Year doctors, ensuring a succession of high quality, enthusiastic trainees. As Lead Employer, on behalf of the Mersey Deanery, the Trust is responsible for over 2,000 trainee specialty doctors based in hospitals and GP practice placements throughout Merseyside and Cheshire.

Clinical areas are supported by a comprehensive suite of IT systems including electronic case records, electronic clinical investigation requests & results, digital radiological imaging, electronic discharge summaries and digital dictation.

Whiston Hospital
This is a teaching hospital, which provides a comprehensive range of Acute Medical and Surgical services. The Emergency Department, Maternity Services, Paediatrics, Medicine for Older People, the Regional Burns and Plastic Surgery Units are based on this site. There is a Critical Care Unit with ICU, HDU, CCU, and an onsite Cardiac Catheter Laboratory. The stroke consultants deliver 24/7 thrombolysis to eligible patients presenting with acute stroke, and the 30 bedded stroke ward accommodates both acute stroke patients and those with rehabilitation needs.

St Helens Hospital
The redeveloped hospital was opened in 2008 as an Ambulatory Care Centre with Intermediate Care and Day Surgery. The Rheumatology, Dermatology and Diabetes services are based on this site, with in-reach of their staff into the acute wards at Whiston Hospital.
