This post provides an exciting opportunity for a senior clinician to work in our Mental Health Services for Older People (MHSOP) wards, based across 2 sites (Highbury in Bulwell & Blossomwood in Sutton-in-Ashfield).

The post holder will work, across 5 wards taking referrals to support the nutritional needs of our dementia & mental health patients. The working hours are typically 8-4pm across the working week.

This post will be supported as part of the Inpatient Therapy Team, which consists of a Team Leader, Occupational Therapists, Speech & Language Therapist, Physiotherapists & Support Workers. It is also supported by a Professional Lead Dietitian across the care group and an AHP Lead.

The Clinical Lead Dietitian will be responsible for the integration and provision of a comprehensive and quality dietetic service for all MHSOP inpatients. The post holder will use expert knowledge and experience to educate and support patients and their carers regarding their dietary management. The post holder will provide specialist nutritional and dietetic advice, support, clinical assessment and education to patients/carers, nurses and other members of the multi-disciplinary team. The post holder will also be responsible for providing advice around good nutritional care into MHSOP community services.

The Clinical Lead Dietitian will contribute to further service development, policy writing and lead on delegated service development projects.

This is an exciting opportunity within the Digestive Diseases directorate Gastroenterology team to work as an Advanced Clinical Practitioner (ACP).

The Acute GI ACP will be responsible for integrating ED, SDEC and virtual ward work streams into the broader Acute GI service. The ACP will work closely with Acute GI nurse practitioners and the Acute GI clinical lead to develop the service and ensure a robust pathway of care for our patients.

The role includes:

Interface between Acute medicine, ED and Gastroenterology

Clerking patients in ED and SDEC, facilitating access to diagnostic imaging/procedures, ensuring ward-based procedures are completed as needed (e.g. paracentesis) and completing discharge summaries for SDEC patients.

Preparing entries for patients in the Gastroenterology Virtual Ward, attending the consultant-led Virtual Ward round three times per week. Co-ordinating care for patients on the virtual ward by linking closely with the community and hospital VW team.

Capturing data pertaining to SDEC and Virtual ward in conjunction with coding team/analysts to demonstrate the value of these services to the wider department and to the Trust.

Development of specific outpatient work streams for trainee and qualified ACPs, e.g. Hepatology ward discharge clinic +/- Acute GI “GIFU” clinic, with consultant oversight.

Innovation and expansion of SDEC/Virtual Ward pathways in close conjunction with the Acute GI clinical lead.
