This exciting new role is suitable for a nurse with a high level of clinical decision making with a sound theoretical and practical knowledge relating to this speciality is required from an expert practitioner who strives to maintain clinical excellence.
The Home Oxygen Assessment and Review Service (HOS-AR) provides a robust assessment and review service for adult patients who require oxygen on prescription in their own homes, supporting GPs and clinicians across St Helens.
The post holder be responsible for monitoring the performance of the service, delivery of high-quality care, clinical governance, patient safety and clinical quality , they will join a dynamic, multi-disciplinary, community respiratory service with an excellent track record in delivering integrated care and support the further development of an established oxygen assessment service for patients requiring home oxygen therapy, in line with national and local guidelines.
The post holder will be expected to integrate with the wider community team and support the management of patients with respiratory disease. They will also be responsible for the case management of patients, using a holistic approach, to ensure that patients receive optimal care.
This new role will include responsibility for ensuring service development, that is in line with national guidelines, and will also provide leadership and training across primary care regarding oxygen assessment and oxygen therapy.

CLINICAL & PROFESSIONAL RESPONSIBILITIES
• Demonstrate advanced clinical knowledge and skills in the speciality of respiratory medicine in relation to the assessment and management of patients with asthma, COPD, bronchiectasis and interstitial lung disease.
• To undertake nurse led clinics within the hospital and community setting, to assess and monitor oxygen requirements and general respiratory status.
• Work collaboratively to order the required oxygen therapy, undertake home visits for follow up assessments and make any changes to the oxygen therapy as required.
• Provide cover for the Respiratory Rapid Response Nursing Team and other elements of the service whilst working autonomously when required, ensuring a seamless service.
• Provide specialist, expert clinical knowledge to clinical colleagues, patients, and carers/relatives.
• Plan, implement and evaluate care delivery according to changing health care needs.
• Perform advanced clinical skills in assessment and/or diagnosis and/or utilising treatment protocols for patient assessments, to ensure their management is based on principles of best practice.
• Assist in the management of the compressor home loans service. This will include the assessment, education, implementation, monitoring and evaluation of the patient and effectiveness of the therapy.
• Responsible for ensuring the equipment is returned and serviced as per Medical Engineering Department protocol and regulations stated by the Medical Devices Agency.
• Update the comprehensive computerised list of patients who have compressors on loan and call in for service as appropriate.
• Responsible for the formulation, implementation and review of clinical guidelines and policies, introducing patient leaflets/ booklets and participating in standard setting for the oxygen service.
• Participate in appropriate research projects as directed by self, MWL or other members of the team.
• Overall line management responsibility for junior staff, to include clear systems of professional appraisal, training needs analysis and organisation of mandatory/CPD training
• Interpret and analyse clinical and non-clinical facts to form an accurate diagnosis and prognosis for patients with complex problems, which may include the appropriate recommendation of intervention from Primary, Secondary or Intermediate Care and/or other agencies.
• Ensure own practice and that of other members of the team is consistent with current research knowledge and evidence based.
• Promote and display evidence of developments in nursing and related clinical research, which enhances the level of professional knowledge or skills.
• Possess specialist skills to work autonomously across primary and secondary care and provide/support daily case management of all patients under the care of the HOSAR.
• Undertake arterial and capillary blood gas sampling in hospital and community settings in accordance with Team and Trust guidelines and protocols. Obtain and interpret results, prescribe and/or titrate oxygen for individual patient needs as per national guidelines.
• Ensure staff risk assess patients who may need or who are prescribed oxygen to prevent serious untoward incidents, obtaining consent for sharing of personal information and identifying patients who may require withdrawal of oxygen
MANAGERIAL/LEADERSHIP
• Evaluate service delivery, identify areas for improvement and initiate change.
• Ensure the effective, efficient use of physical and financial resources, make recommendations regarding supplies and equipment.
• Maintain safe standards of practice, at all times, and report concerns to the Lead COPD Nurse and/or Directorate Manager.
• Attend and/or contribute to meetings in all health care settings to discuss the respiratory service and its development.
• Responsibility for health, safety and security of others and promote best practice in the clinical area.
• Input into data collection systems collating information about patients as required, facilitating review and audit.
• Analyse invoice reports for financial anomalies to reduce waste and ensure safe and appropriate use of oxygen therapies by patients.
• Analyse concordance reports to identify and review those who may present a clinical risk by over and under using their oxygen prescription.
• Analyse data to identify those who may present a fire risk and risk manage as appropriate
• Respond to information provided by the home oxygen supplier/regional home oxygen lead in an appropriate and timely manner, including clinical escalations generated outside of the organisation.
• Responsible for the formulation, implementation and review of clinical guidelines and policies, introducing patient leaflets/booklets and participating in standard setting for the oxygen service.
• Overall line management responsibility for junior staff in the oxygen service, to include clear systems of professional appraisal, training needs analysis and organisation of mandatory/CPD training.
• Promote and display evidence of developments in nursing and related clinical research, which enhances the level of professional knowledge or skills.
• Identify learning needs, provide ongoing education for staff within the team and evaluate programmes of education to meet those identified needs.
• Participate in the multidisciplinary pre-registration education and training programme for students.
• Participate in clinical supervision either informally or formally.
• Address any specific issues concerning respiratory care that require dissemination to the nursing and medical staff or professions allied to nursing across MWL.
• Provide formal and informal teaching sessions to hospital and community staff involved in the care and management of patients with respiratory conditions.
PROFESSIONAL RESPONSIBILITIES
• To assist the Lead Nurse to meet the commissioned targets whilst ensuring optimum care for patients.
• Adhere to lone working policy and carry lone working devices each shift to maintain safety, activating the device each day to ensure satellite recording of staff location in line with trust policy.
• To observe the provisions of and adhere to all Trust policies and procedures and standard operating procedures.

• Attend all statutory/mandatory training sessions, identifying when training is required and discussing with line manager.
LINE MANAGEMENT/SUPERVISORY RESPONSIBILITIES
• Assist in the evaluation of service delivery, identify areas of improvement and initiate changes.
• Ensure effective and efficient use of physical and financial resources within the service.
• Work collaboratively with the professional agencies to ensure patient needs are met and especially in relation to on-going care and informed discharge with self-management skills.
• Support the Service Manager to evaluate and improve service provision to identify areas of improvement, initiate, and embed change.

· Ensure effective delivery of multi-disciplinary team meetings
RESEARCH & AUDIT
• Continually monitor standards of care and participate in the improvement of care through benchmarking/audit/research.

• Support the development and delivery of annual patient satisfaction survey to monitor patient satisfaction across the service.
