The post holder will need to be able to use their previous knowledge in assessing referrals and liaise, where necessary, with the GP’s and Cardiologists around referrals into the service. The post holder will be expected to work autonomously but seek advice/support where applicable. Along with referrals the post holder will help to manage a GP help line.

The day unit primarily specialises in metabolic and endocrine testing, however it also takes referrals for day cases from other specialties. The clinics house pituitary, growth hormone clinics, thyroid patient care, osteoporosis care and all manner of diabetic care such as foot care, sexual health and infectious diseases, retinal screening and a young persons clinics.

We currently have a Named Nurse Safeguarding Adult whose banding is under review to make it a higher band. The post is line manager for safeguarding adult nurses and the learning disability and autism nurses. The post oversees the patients where DoLS applications have been submitted as well as close liaison with the tissue viability nurses and ensuring reasonable adjustments are in place for those that require it as well as overseeing the domestic abuse provision and response across the Trust. As a Named Nurse Safeguarding Adults you will also attend Safeguarding Adult Board’s sub-groups alongside the other Named Professionals and engage with multi-agency audits and case reviews. You will be required to complete chronologies and internal management reviews with assistance.

De Wardener is the renal High Dependency Unit (HDU) , a hybrid ward that specialises in the care of renal patients who require intensive monitoring for highly complex needs. This includes patients who have had a kidney or a kidney and pancreas transplant and patients who require non-invasive ventilatory support such as CPAP, BiPAP and High flow oxygen and/or tracheostomy care and inotropic support .There is also a ‘lines’ room on de Wardener where access for dialysis is inserted under imaging guidance. The Integrated Cardio-Respiratory Services is a combined service working across hospital and community pathways, providing community services in clinic and home-based settings. The service is a consultant led multidisciplinary team providing a range of service including; diagnostic clinics, COPD and asthma disease optimisation, exacerbation management, hospital discharge, home oxygen, pulmonary rehabilitation, dyspnoea management and primary care education and support.

The aim is to provide a more accessible, efficient, cost-effective service integrating respiratory services across secondary and primary care, to enable continuity of care and give respiratory patients the opportunities to have access to care closer to their homes and end of life care in their homes.
• To facilitate and manage the provision of high quality care to all adult respiratory patients alongside the consultants and allied health professionals in respiratory medicine in a variety of settings to promote respiratory health within ICHNT and the surrounding community.
• The post holder will provide support and advice to doctors, nursing staff, allied health care specialists, patients and their families/carers, known to the community service, and ensure smooth transitions of care into and within the community setting.

We have an opportunity for an enthusiastic, motivated pharmacist with an interest in medical specialities for a new Band 7 role within the Pharmacy Specialist Medicine Team.

You will support the team with clinical ward services, funding of high cost medicines, Homecare prescription screening and support the Lead Rheumatology pharmacist with biologic MDTs.

You will also be involved in supporting & helping develop trainee pharmacists, band 6 pharmacists & medicines management pharmacy technicians as appropriate.

This role is suitable for an individual who is looking to develop their clinical skills within medical specialities. You will also have the opportunity to develop your knowledge of commissioning of high cost medicines and financial monitoring of medicines usage. You will be a team player with good attention to detail and good communication skills, able to work with a variety of different professionals.

Applicants should be a pharmacist registered with the General Pharmaceutical Council. You should have a postgraduate clinical diploma or equivalent experiential learning. Interviews are likely to be 9th /10th December 2024. The post is fixed term until 31/3/26.
