Major duties of this position include but are not limited to:
Functioning as the coordinator of care for Veterans at suicide risk receiving care through the Battle Creek VAMC healthcare delivery system including its Outpatient Clinics. The SPC also serves as an advisor to the Associate Chief of Staff for Mental Health, Mental Health Service Chiefs, facility leadership, program managers and other medical center staff concerning suicide prevention strategies for individual patients, for the facility and for the Veteran community as a whole in conjunction with the SPCs at the Wyoming Health Care Center and Battle Creek VAMC, and other members of the Suicide Prevention Team.

Specific functions of the SPC include, among others, identifying Veterans at risk for suicide, tracking appointments and coordinating enhanced care as needed, educating providers, Veterans, families, and members of the community about risk factors and warning signs for suicide, and identifying treatment options. This is accomplished through:

• Tracking all suicide attempts and patients with serious suicide ideation at each facility through the use of the "Suicide Behavior Report" progress note. The coordinator will implement the use of this template at each facility and be placed as a co-signer or additional signer on the template in order to be made aware of all attempts and patients with serious ideation.
• Ensuring that Suicide Behavior Flags are posted on the charts of relevant Veterans and monitoring the need for continued flagging.
• Assuring that all Veterans on the facility tracking list are being followed closely by the appropriate provider on a regular basis as determined by the coordinator and the provider.
• Tracking any no-show appointments by these patients and following up with the patient as soon as possible to reschedule the appointment.
• Following up with Veterans who have contacted the Veterans Crisis Line.
• Maintaining required databases and reports per the National Suicide Prevention Program.
• Preparing Heads Ups and Issue Briefs as needed.
• Monitoring and maintaining the REACH Vet database and the clinical care required.
• Monthly contact with Veterans deemed at high risk for suicide as defined by national guidelines and with those Veterans who remain actively on the high-risk list.
• Monitoring the completion of the treatment plan, safety plan, and means reduction for those patients who have been identified to potentially pose an increased risk for future suicidal behavior.
• Disseminating, organizing, and, where possible, conducting evidence-based and promising, emerging interventions.
• Maintaining awareness of suicide treatment options and making recommendations to the patient's provider when new options become available.
• Obtaining any relevant HIPAA clearances to allow transmittal of facility data on suicide attempts and individuals at risk to the Suicide Prevention Center of Excellence for trending and quality assurance purposes; and transmitting the data following cyber security requirements as applicable for this sensitive data.
• Assuring that all of these patients have a 24-hour resource number including the VCL to call and ways to get assistance if an immediate crisis should occur.
• Establishing an on-going Gatekeeper Training program with non-clinical frontline staff such as clerks and telephone operators to assure that they are aware of options for patients with whom they come in contact during crisis situations.
• Working with community resources such as VSO's and other community groups to assure that they are aware of how to recognize Veterans at risk for suicide and their options for getting these Veterans assistance.
• Developing and implementing an ongoing education plan for providers, Veterans, families, and members of the community on risk factors and warning signs for suicide, and treatment options.
• SPCs will participate in training activities and scheduled conference calls as required by the Office of Mental Health and Suicide Prevention (OMHSP)
• Performs other related duties as assigned

Conducts safety and environmental inspections of the institution and prepares a report for the Environmental and Safety Compliance Administrator’s review.

Promotes the adherence of safety policy for the facility.

Assist in the training of staff and inmates in the techniques of firefighting, occupational safety and environmental protection.

Assist the Safety Administrator or Assistant Safety Administrator in conducting investigations and obtains statements from witnesses and securing physical evidence.

Prepares factual written reports for all concerned that will identify the accident, injuries suffered by staff and/or inmates, cost of property damage and offer recommendations to eliminate future accidents in the future.

Responsible for developing and conducting safety training classes for the inmate population and staff.

· You will perform work as a certified Emergency Medical Technician- Paramedic within the Division of Resource and Visitor Protection, operating independently to make patient care decisions.

Program Management
- The Management and Program Analyst is responsible for human resource processes, to include, but not limited to, entering recruitment requests, coordinate credentialing paperwork, and onboarding requirements such as Compensation Panel and computer access requests.
- He/she will assist the Service Administrative Officer in all matters pertaining to the periodic equipment inventory process. This will include working with Logistics and Biomed, and Engineering staff to locate, track, and appropriately disposition all equipment listed in the Service EIL's.
- The incumbent has oversight responsibility for the GIP/Purchasing process, ensuring that VAAAHS, National, and VISN 10 standards are met. He/she will implement corrections to the process if the standards are not met. This coordination will entail working directly with the Logistics GIP/Purchasing Agent, the Supply Technician, and the facility GIP Team.

Physician Coordination and Clinic Grids
- The incumbent is responsible for coordinating all aspects of Physician hiring for Ambulatory Care Services. This includes recruitment, retention, salary determination, credentialing, and privileging. Additionally, this person will be responsible for Physician Productivity measures in relation to other class 1a facilities; they will perform in-depth analysis/audits for areas below the 25% or above the 75% developing action plans for submission to COS and facility director.
- The incumbent will manage all activities related to the residency program. He/she will initiate all necessary organizational procedures involved in maintaining an active residency program. Close coordination with the University of Michigan is necessary to effectively execute these
responsibilities thus requiring the incumbent to develop and maintain a close working relationship with the Residency Program at the University of Michigan. He/she will facilitate the scheduling for diverse Patient Align Care Team (PACT) teaching exposures for learners. The incumbent will assist with modifying and building new clinic grids for Primary Care Providers.

Data Capture and Coordination
- The incumbent will independently manage, analyze, and coordinate all decisions associated with Decision Support Services (DSS) activities within the Service. These duties will include meeting with various Section Chiefs within the Service, DSS (Finance) staff, and staff within the Service to
strategize and apply a problem-solving approach to supply cost distribution and labor mapping.
- The incumbent will identify necessary changes in cost distribution related to RVU's and determine the appropriate corrections as necessary. He/she will also periodically but routinely review with staff labor and supply mapping and product costing in order to ensure accuracy. In addition, incumbent
will coordinate the completion of annua1 Physician Service Agreements as well as Agreements for incoming staff physicians.

Contract Coordination
The incumbent is responsible for the processes involved in the coordination of all contracts with the department. The creation of contracts consists of preparing the documentation required and submission through appropriate leadership channels. Contracts often require close consultation
with local Logistics and national Contracting staff to complete these processes, and the required maintenance of these agreements throughout the life of the contract. The incumbent will assist AO with equipment and procurement requests.

Peer specialists perform a variety of therapeutic and supportive tasks that include assisting Veterans in articulating their goals for recovery and personal wellness, learning, and practicing new skills, as well as helping veterans monitor their progress, assisting them in their treatment, modeling effective coping techniques and self-help strategies based on the peer specialist's own recovery experience, and supporting them in advocating for themselves to obtain effective services. This position will serve as a community-based peer specialist within the local VA facility catchment area. The working environment may include providing peer support services in places such as Veterans' homes, prisons, jails, treatment courts, shelters, tent encampments, street outreach, etc.

The Extended Care Service Community Living Center Staff Nurse is a Professional Nurse with broad-based clinical knowledge enabling him/her to effectively implement health promotion, disease prevention, and management for adult or older adult residents requiring skilled Nursing interventions in a long term care setting. CLC Staff Nurses typically provide care for residents with complex, mixed chronic medical and/or psychiatric conditions, residents with cognitive impairments, and residents who are terminally ill. The CLC Staff Nurse utilizes the Nursing process when assessing, planning, implementing, evaluating, and documenting Nursing care provided. He/she works collaboratively with the interdisciplinary team when planning and providing care to Nursing Home residents and their families. The Staff Nurse functions independently as a charge Nurse on the unit, organizing and delegating care activities to other RNs, LPNs, and NAs and providing feedback to the Nurse Manager regarding the practice of others in order to improve care. • Facilitates action for community placements through collaboration with Veterans & their families as well as interdisciplinary treatment team members to ensure that appropriate community placements are completed in a timely manner.
• Implements treatment modalities to achieve treatment goals using judgement & skills in utilizing supportive, problem solving, or crisis intervention techniques.
• Exercises a high level of skill in assessing & treating the complicated psychosocial problems of Veterans.
• Completes assessments of psychosocial problems often at a time when the Veteran is experiencing crisis & include the provision of short-term, solution focused counseling with Veterans &/or their families.
• Establishes & maintains effective working relationships & communicates with Veterans, staff, & other parties as appropriate.
• Independently works with Veterans & their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, & psychosocial problems.
• Actively participates as a member of the interdisciplinary treatment team & actively participate through collaboration with Veterans & family as well as interdisciplinary treatment team members.
• Utilizes knowledge of community resources, makes referrals to the community & other government agencies as appropriate, & coordinates services.
• Acts as liaison between Veterans &/or their families, WCHCS, & community resources to ensure thorough delivery of services.
• Provides consultation & education to Veterans & their families regarding community resources, VA benefits & specialty programs, & advance directives.
• Independently conducts psychosocial assessments & treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, & other diversified backgrounds.
• Possesses a high level of skill & expertise to establish & maintain effective therapeutic relationships with Veterans in the Primary Care Clinic &/or their families.
• Independently completes thorough psychosocial assessments to determine the psychosocial functioning & needs of Veterans &/or their families.
• Utilizes knowledge of medical & mental health diagnoses, disabilities, & treatment procedures to formulate a treatment plan.
• Possesses a working knowledge & experience in use of medical & mental health diagnoses, disabilities, & treatment procedures.
• Provides case management with members of the outpatient interdisciplinary team & home & community-based services is provided to coordinate a collaborative effort to meet the agreed upon goals for a Veteran's treatment needs.
• Independently implements different treatment modalities in working with Veterans, families, & groups.
• Knowledge & ability to independently implement treatment modalities, provide educational classes, &/or supportive groups for Veterans & families.
• Possesses skills & knowledge to provide case management services to Veterans & their families throughout the continuum of care.
• Provides consultation, education, & advocacy.
• Provides consultation to other treatment team & staff members regarding psychosocial needs of Veterans &/or their families.
• Responsible for furthering one's own professional growth through continuing education & for ensuring continuing education appropriate to the area of assignment.
• Demonstrates ability to communicate effectively, both orally & in writing with people from varied backgrounds.
• Responsible for supporting the mission, policies, & procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), & the facility.
• Serves on committees, work groups, & task forces at the facility.
• Keeps their supervisor apprised of problems & recommended solutions to problems.
• Provides assistance with the orientation & learning of less experienced social workers & to serve as a preceptor for social work students.
• Demonstrates knowledge & skill in the use of software applications for drafting documents, data management, & other computer systems.
• Maintains a level of productivity & quality consistent with the complexity of the assignment & consistent with Joint Commission & facility standards.

Dietitian (Clinical) duties include:

• Conducts comprehensive nutrition assessments using the Nutritional Care Process (NCP), including nutrition focused physical examinations, to establish a nutrition diagnosis and determine risk and/or degree of malnutrition.
• Develops and implements Medical Nutrition Therapy (MNT) to address the nutrition diagnosis, evaluates and monitors the effectiveness and outcomes of MNT interventions. Modifies the plan as needed to achieve positive patient outcomes.
• Calculates nutrient requirements and necessary diet restrictions or food and drug interactions tailoring the diet prescription to each individual patient.
• Documents MNT in the medical record using the Assessment, Diagnosis, Intervention, Monitoring, Evaluation (ADIME) format.
• Functions independently and accurately when record keeping.
• Identifies patients in need of nutritional support and recommends the appropriate mode of support and communicates appropriately with the health care team.
• Actively participates in formal and informal interdisciplinary team meetings.
• Assesses educational needs and provides nutrition education/counseling for individuals and groups that is tailored to the Veteran(s).
• Utilizes virtual care technology to improve access to care.
• Maintains productivity standards.
• Maintains accurate record keeping of daily clinical activities and completes patient encounters using approved systems.
• Ensures continuous improvement of safe and quality care of Veterans.
• Leads activities in the implementation of new initiatives. Uses data in the development of measures, data evaluation, and initiation of corrective actions.
• Complies with The Joint Commission requirements and other accrediting agency standards to ensure safe and quality patient care.
• Collaborates with the Food Operations Section and department leadership to enhance the Veterans' dining experience, support patient satisfaction, and serve as a liaison between clinical units and the Food Operations Section.
• May perform other related duties as assigned which are appropriate for this grade and assignment.
