Job summary: The Health Care Assistant will work as part of the clinical team carrying out a range of duties ensuring that the highest standard of care is afforded to patients at all times. Duties and responsibilities: Undertake routine tasks related to patient care ensuring clinical protocols are followed, referring to GPs when necessary. Routine tasks will include: dressings, removal of sutures, blood pressure monitoring, phlebotomy and ECGs. Ensure that anomalies are reported to a senior member of staff. Notify the GP/Nurse of any significant patient issues. Liaise with and assist members of the administration team. Job summary: Receptionist Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way. Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone. Job responsibilities: Ensure an effective and efficient reception service is provided to patients and any other visitors to the practice Deal with all general enquiries, explain procedures and make new and follow-up appointments. Using your own judgment and communication skills, ensure that patients with no prior appointment but who need urgent consultation are seen in a logical and non-disruptive manner. Explain practice arrangements and formal requirements to new patients and those seeking temporary cover, and ensure procedures are completed. Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery. Action repeat prescription requests and ensure that they are ready for collection by the patient within 48 hours, either on the computer or manually. Enter patient information on to the computer as required. Ensure complaints are dealt with as per the complaints procedure in a courteous and efficient manner. This is a new role for a Healthcare Assistant to support our care homes in our local area.

You will work closely with the community nursing team, delivering care to residents which will include diabetes care / insulin administration, wound care, venepuncture and catheter care.

Our service works 7 days a week. You will be able to demonstrate flexibility as well a sound clinical basis and understanding of community working.

1. To contribute to the delivery of a variety of interventions for patients with long term conditions, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.

We have part time and full-time hours available.

The nursing teams are based in Loddon, Long Stratton and Diss, and consist of Nurses, Assistant Practitioners and Healthcare assistants. The Therapy team are based with the Nursing team and work very closely. They consist of Physiotherapists, Occupational Therapists and Assistant Practitioners.

Clinical

The post holder will:
1. Apply competency based treatment techniques/care to patients with specific conditions under the supervision of a qualified practitioner, following a prescribed treatment/care plan.
2. Plan and prioritise own visits to delegated patients, and ensure these are documented in personal work diary.
3. Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis
4. Make basic judgements on patients’ response to prescribed treatment/care plans, and report findings to a qualified practitioner.
5. Liaise with other health care professionals as required.
6. Overcome barriers to understanding, e.g. patients’ with hearing or cognitive impairment.
7. Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy.
8. Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.
9. Demonstrate dexterity and coordination when undertaking treatment of patients where accuracy is important, e.g. taking blood sugars, venepuncture, supervising transfers, and passive exercises.
10. Be responsible for ensuring equipment is used safely, following relevant training, and that it is maintained appropriately.
11. Contribute to the requisitioning of supplies and equipment through electronic ordering systems.
12. Work predominantly independently on specified tasks, with regular clinical support and supervision by a qualified practitioner.
