The Crisis Response Team and Hospital at Home Senior Discharge Coordinator, Health and Social Care Practitioner is responsible for junior members of staff, undertaking informal and formal support and supervision.

The Crisis Response Team and Hospital at Home Senior Discharge Coordinator, Health and Social Care Practitioner will be required to actively participate in new initiatives and further changes intended to improve service delivery.

We are developing a hospital at home, resourcing the team to enhance our established “Home First” approach supports patient’s personal
choices, strengths, and ensures inclusion in every stage of their care.

We are developing the service to adopt earlier intervention from our community response teams, which aims to reduce acquired hospital deconditioning, enabling a person to maintain as much of their independence as possible.

We are passionate about continually developing our services to provide gold standard care for older people.

This Crisis Response Team and Hospital at Home Senior Discharge Coordinator, Health and Social Care Practitioner role will work in the community based with Crisis Response Team (part of Manchester Community Response), assisting registered professionals (Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Pharmacists and Social Workers).

As part of the multidisciplinary team, the post holder will be responsible for supporting the Crisis Response Team to deliver urgent health and social care to people in crisis and at risk of hospitalisation. They will also support the Hospital at Home service to reduce length of stay in hospital and provide care for patients within their own home.


The successful candidate will be responsible for junior members of staff, undertaking informal and formal support and supervision. The post holder
will be required to actively participate in new initiatives and further changes intended to improve service delivery.

DUTIES AND RESPONSIBILITIES To aseptically prepare high quality Cytotoxic/MABS/EAMS etc. in accordance with departmental procedures, Good Manufacturing Practice and COSHH regulations ensuring that all work complies with all regulatory frameworks and standards. Undertake daily cleaning and disinfection of the aseptic clean rooms including Isolators, according to Standard Operating Procedures. To ensure ingredients and disposables are assembled, disinfected and transferred through the Aseptic Services Unit appropriately. To carry out in-process checks of aseptically prepared products upon completion of appropriate training. Participate in the safe preparation and delivery of Intrathecal chemotherapy in accordance with Trust policy upon completion of departmental training. To accurately prepare worksheets and labels for aseptic products using the units computer system using JAC and iQEMO program if required. To assist with the preparation of documentation within the aseptic unit. To assist with the recording of data, both manually and on the computer Answer the telephone promptly and politely and refer queries to the appropriate person. As a HCPC registered Senior Specialist Occupational Therapist based on our Neurological Rehabilitation Unit at Trafford General Hospital you will be part of a large multi-disciplinary team working on the only level 2a Neuro-Rehab unit in Greater Manchester.

This is an exciting opportunity to work at the Royal Manchester Children’s Hospital. It is the largest children’s hospital in the UK with 371 beds. The hospital provides children’s emergency, secondary and regional tertiary services.

The Department of Paediatric Surgery is the specialist children’s surgery centre in North West England including Neonatal, Oncological, Thoracic, Inflammatory Bowel Disease and specialist day-case surgery, and the surgery of intestinal failure and short bowel; supra-regional service supporting exstrophy, hyperinsulinaemia of the New-born, Metabolic disorders.

We are looking for highly motivated individuals to join our team and be a driving force in further developing our infection service. These posts would be well suited to junior doctors looking to deepen their infection and microbiology knowledge.

The Department is current undertaking an improvement project to create a cohesive infection service across all the hospitals within the Trust, in what will become one of the largest infection departments in the UK. This will incorporate colleagues in infectious diseases, sexual and reproductive health, respiratory medicine, genitourinary medicine and infection prevention.

Candidates must be eligible for their own GMC registration:an active licence, ceased license or pending application with the GMC at the point of application. Candidates who do not fit this criteria will not progress through to the shortlisting stage.
