We are looking for an experienced Nurse Associate, who acts within their professional boundaries, supporting and working alongside a team of healthcare professionals, GPs, and a non-clinical team in general practice. In this role they will be supported by a GP and Manager who will develop, manage, and mentor them. The post holder will work as part of a multi-disciplinary team in a patient facing role. The post holder will take responsibility for areas of chronic disease management within the PCN. The post holder will provide primary support to general practice staff and carry out Chronic Disease Management annual reviews delegated by a supervising clinician or senior colleague. The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. The post holder will ensure that the practice integrates within the community to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. The post holder will be required to monitor and support the lead nurse with the management of patients with long-term conditions/chronic diseases, using a holistic patient centred approach. You will also be required to take vital signs and observations and identify signs of improvement, report any deteriorations or concerns, e.g. pulse, blood pressure, respirations, oxygen saturations, peak flow, blood glucose levels, ECG, assessing nutrition and hydration and undertake pathology tests, for example urine culture, swabs and undertake wound care. The post holder will be required to administer medications as prescribed by the GP/Pharmacy and in accordance with NMC standards, e.g. via nebuliser or injections using sub-cutaneous or intramuscular route under Patient Specific Directions. Pharmacy Technician - Job Description Responsible to: This role is directly employed by the North West Leicestershire GP Federation and the post holder will work for the North West Leicestershire PCN. Job Summary The post holder is a Pharmacy Technician, who acts within their professional boundaries, supporting and working alongside a team of pharmacists and technicians in General Practice. In this role they will be supported by a Senior Clinical Pharmacist who will develop, manage and mentor them. The post holder will work as part of a multi-disciplinary team in a patient facing role. The purpose of the role is to lead improvements to maximise safe, cost-effective best practice in prescribing to improve the quality of patient care. You will help patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in general practices, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines. The post holder will provide support to general practice staff with regards to prescription and medication queries. Carry out medicines switches in GP practices in line with practice and CCG agreed specific protocols reviewing patients records; amending records and informing patients. They will help support the repeat prescriptions system promoting electronic repeat dispensing and online ordering. Carry out housekeeping whilst in patient records e.g., dose optimisation and medicines synchronisation. The post holder will be the first port of call to answer and deal with any medicines related queries from administration and reception staff plus any medicines related queries from dispensary staff. To develop therapeutic alliances with people experiencing homelessness using the principles of trauma informed care, as a way forward to enabling relationship building, assessment and treatment for people experiencing homelessness To undertake specialist homeless health care at an advanced level. The post holder will be able to make high-level clinical decisions, and initiate and carry out complex interventions based on their own interpretation of the clinical situation. To act as a health case manager for individuals identified with high levels of unmet health need, and work towards individual health outcomes as defined by the project goals To undertake holistic physical and psychosocial health assessments on the street for people experiencing homelessness, in order to identify unmet physical, mental health, addictions, safeguarding, housing and social care needs. This includes the need for screening, health promotion and harm reduction work. To carry relevant equipment on street shifts which enables the assessment of clinical risk, and can be used to provide immediate and necessary care e.g. thermometer, sphygmomanometer, oxygen saturation monitor, basic 3 resuscitation equipment e.g. a pocket mask resuscitator, basic dressings and first aid equipment etc To proactively identify immediate clinical risks to life and limb when seeing individuals on the street (e.g. related to possible infection, chest pain, abdominal pain, neurological concerns etc), and take appropriate action to enable the individual to access appropriate and timely care e.g. calling an ambulance, or taking senior advice, nurse prescribing etc. To independently undertake mental capacity assessments if an individual refuses necessary care, and to make consequent, best judgment decisions on whether someone has mental capacity to refuse. To understand what action to take in these circumstances (in line with organisational policy), and to deliver this. To provide immediately necessary clinical / therapeutic primary care treatments on the street if needed (based on best possible evidence) that will improve health outcomes for people experiencing homelessness where relevant e.g. wound dressings, nurse prescriptions / Patient Group Directions etc, but to work to signpost mainstream health services, and work to support patients into safe and effective treatment pathways where possible To offer immediate wound care assessment and basic wound dressing as needed. What you’ll be doing?

Ensuring the safe preparation and supply of veterinary medicines, ensuring that all dispensing errors and near misses are logged in accordance with the company’s procedures and to undertake any activity necessary to rectify/prevent errors and maintaining the controlled drugs register

Implementing and adopting standard operating procedures (SOPs) to ensure procedures are in place for the avoidance of risk, and working with key teams across the business (customer service, purchasing, operations & clinical department)

About the role: Assure that integrated pathways are in place for service users who require treatment for their health needs relating to their addiction and that the clinical model is effective and in line with CGL policy and national guidance Implement/maintain an immunisation and vaccination scheme to help combat blood borne virus (BBV) and other infectious diseases and provide advice and information to other professionals on BBV and the care of individuals with drug and alcohol issues Undertake regular CPD and create an environment that encourages your team to learn and develop and disseminate information to your team from the senior operational and clinical management teams Ensure that all clinical services are delivered to a high standard in line with national guidance and CGL policies, procedures and best practice adhering to CQC Essential Standards Manage the clinical aspects of implementations of new clinical developments within the service as required Effectively manage and develop all nursing staff and participate in the core training programme as and when required and be an active support for your peers within the region/organisation Attend, organise and chair (as required) relevant clinical advisory groups and committees and partnership meetings, to ensure appropriate representation of services Work alongside the Lead Doctor (and or Non-Medical Prescriber (NMP)) to formulate, plan and implement individual treatment programmes to ensure safety and optimise outcomes for service users. This will include supporting nurses assisting detoxifications from substances across operational sites. About you: Valid registration with the NMC and a current pin Significant experience of working in the health/social care field ideally within substance misuse Experience of working and managing people/teams in the clinical services and substance misuse, social care fields An in-depth knowledge of clinical interventions for use with a range of service users Understanding of UK Drug Strategy and Social Policy reforms Knowledge of different treatment models and harm minimisation interventions In depth knowledge of the regulatory framework within which the clinical services and the organisation operate Awareness of, and ability to work within, clinical legislation in relation to service users Be an excellent communicator, both verbal and written with good IT skills Able to manage in a facilitative and coaching style Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders Ability to manage change successfully in a way that prioritises the needs of service users Understand the importance of information governance processes and commit to follow and apply all necessary safeguards Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others

Our client is Drug & Alcohol Recovery Service based in Newham. The post of Cluster Lead Nurse is to provide clinical leadership and clear direction for the Newham integrated nursing team and to promote development and delivery of clinical health and wellbeing interventions.
Hours per week: 37.5 Monday to FridayContract Length - TBCPay rate: £38 - £42/hr (negotiable)
Primary duties:You will play a key role in ensuring the nursing team delivers evidence-based clinical interventions, including vaccinations, blood-borne virus screening, health, and wellbeing assessments to support drug and alcohol prescribing and detoxification programs and will provide a clinical supervision role for the nursing team.To ensure that the service's clinical model is functioning effectively, and all clinical services are delivered in line with the companies' policy and national guidance.You will be involved in a clinical audit of the service under the direction of the Deputy Director of Nursing.Management and development of nursing staff.
