1. Clinical 1.1 To act as a Specialist Nurse for patients undergoing bariatric surgery and to have direct clinical involvement in complex patient care in the outpatient and in-patient setting. 1.2 To provide a nurse led service for those patients requiring follow-up care after surgical intervention. 1.3 To facilitate all investigations for those patients suspected of post-surgical complications. 1.4 To facilitate the bariatric surgery telephone support service. To be available for patients and their carers in order to assess and manage day to day problems and anxieties and to refer on where appropriate. 1.5 To co-ordinate the bariatric surgery journey, acting as the key worker for those patients under your Care on the Bariatric pathway. 1.6 To assess patients and their complex needs and those of their families and plan, implement and evaluate specialist programs of care, communicating with those responsible for the care, and documenting the appropriate details whilst at all times maintaining confidentiality. 1.7 To refer patients for diagnostic investigations with suspected or proven, endocrinal diseases. 1.8 To ensure that specialist bariatric surgery care information is available. Professional 3.1 To be a key member of the bariatric surgery team contributing to patient assessment and facilitating a collaborative approach to care planning at weekly bariatric surgery Multi-Disciplinary Team (MDT) meetings and contribute to all other MDT meetings, i.e. MDT away days, Network Site Specific Group. 3.2 Ensure that nursing practice is in keeping with NMC Code of Professional Conduct and standards and reflects policies and procedures of UHNS. 3.3 To ensure that bariatric surgery care nursing interventions are evidence based, or best practice and that a uniform high standard of patient care is provided. 3.4 Ensure that plans for the service link to the goals and priorities of the Trust/Directorate and to the bariatric surgery Action Plan, following Peer Review. 3.6 Develop care pathways for bariatric surgery patients. 3.7 Keep up to date with professional issues and ensure they are reflected in practice. 4. We are looking for a motivated, skilled, and experienced Community Psychiatric Nurse Practitioner to join our Bexley Community Mental Health and Wellbeing Hub.

The Community Mental Health Transformation Programme has led to the development of the Community Mental Health and Wellbeing Hub in Bexley and this will be delivered by a combination of clinical staff from Oxleas, Bexley Mind, Primary Care, Bexley LA, voluntary, third party and local partners. This will enable people to receive support with managing their mental health alongside help with issues including employment, relationships, housing, finances and engaging in the local community and accessing community asserts in Bexley. The Hub receives referrals from various sources but principally primary care sources including GPs and the London Borough of Bexley. This is an exciting opportunity to be involved with the development of the service and requires a commitment to integrated working.

The Community Mental Health Nurse Practitioner will act as an effective member of the integrated Bexley Community Mental Health and Wellbeing Hub, with responsibility for face-to-face and telephone assessments of incoming referrals from various sources within the borough of Bexley but principally referrals from GP practices and social services teams in the borough. Job Purpose To enable Bexleycare to deliver a consistent and responsive Community Mental Health Wellbeing Hub in conjunction with the ethos of the NHS Long Term Plan Community Mental Health Transformation Programme. To develop, appraise and improve the community mental health hub, working closely with all stakeholders from Bexleycare and other stakeholders to provide a robust service to people who are experiencing psychiatric and or psychosocial mental health problems in Bexley. Overview of the Post The post holder will deliver and or advise on the delivery of high-quality safe nursing care, ensuring consistently high standards of practice and clinical excellence which comply with the Nursing and Midwifery Council (NMC) code of professional conduct. Bexley Community Mental Health and Wellbeing Hub is a primary care level service in the Borough of Bexley. The post sits within a multi-disciplinary framework and provides assessments (telephone, virtual and face to face) and brief interventions. The team liaises and gets referrals from several sources including GPs, Bexley Local Authority and MIND. As a Community Mental Health Hub Nurse you will promote and develop clinical leadership and clinical practice and be a positive role model to junior members of the team through the practice of evidence-based care. You will be expected to support professional development through clinical appraisal and supervision and contribute to the ongoing educational programme for staff, fostering a positive learning environment. As a senior nurse working within the Hub you are required to encourage and foster collaborative and cross organisational working. Key Task and Responsibilities This job description is an outline of the key tasks and responsibilities expected of a Band 6 Nurse. The post holder will be required to undertake additional duties appropriate to the clinical area/speciality in which they work.
