The PM&Rs Service staff physiatrist reports to the Chief, Physical Medicine & Rehabilitation Service (PM&RS). A staff physiatrist routinely performs inpatient and outpatient musculoskeletal and neuromuscular examinations, determines diagnoses, and initiates treatments that emphasize restoration of function. The staff Physiatrist works with a team of health care providers to establish a comprehensive rehabilitation program. Different patient populations requiring comprehensive rehabilitation programs include those with electromyography, amputations, spinal cord injury, stroke, traumatic brain injury, musculoskeletal and neuromuscular impairments, and those with chronic severe disabling pain syndromes.

Service Responsibilities:

• Manages out-patient and in-patient patients including CLC patients.
• Provides polytrauma rehabilitative care including TBI/SCI
• Leads or participates in clinics focused on an amputee care, seating/power mobility care, non-interventional pain management

The Gastroenterologist must possess an active license in any state or territory in the United States and must always maintain privileges, at the Amarillo VA Health Care System, to be able to work as a physician. The Gastroenterologist must be board certified or board eligible in Gastroenterology and maintain professional competency as indicated in the delineation of privileges. All physicians must maintain current BLS certification as indicated per medical center policies.

- The Gastroenterologist will provide comprehensive care for patients who have illnesses within the scope of Gastroenterology, with an additional focus on patients with hepatitis. In addition to patient care, the physician may have responsibilities in other areas such as, education. The Gastroenterology Staff Physician will be located in the Specialty Clinic at the Amarillo VA Medical Center in Amarillo, Texas.
- The Gastroenterology staff physician should be skilled in providing patient care and in coordinating the activities of other health care providers. He/she should be an effective teacher for fellow staff members, house staff medical students and in allied health care disciplines. He/she will be currently licensed in a State, Territory, or the District of Columbia and should preferable be a certified or qualified for examination by the American Board of Internal Medicine, or other appropriate specialty certifying boards.
- The Gastroenterology staff physician is responsible for providing primary, secondary, and tertiary care in both hospitalized and outpatient settings, identifying patients' health problems and designing diagnostic and therapeutic plans to meet these needs. He/she is responsible for coordinating activities of other health service personnel, resolving problems affecting patient care and providing appropriate education for the patient/family members regarding the patient's care, patient's illness and its management.
- A major percentage of the physician's time will be devoted to patient care. Depending on other activities, sufficient time will be allotted to administrative duties.

The AMSA works collaboratively in an interdisciplinary coordinated care delivery model (i.e., PACT, Specialty Clinics, Compensation & Pension (C&P), Community Care) and performs clerical duties, customer service and other duties as assigned for the proper and timely treatment of patients. You will function as a case manager for 25-30 assigned patients and responsible for assessing and continually reassessing the needs of the veterans and caregivers, delivering nursing care in the home, and keeping physicians and interdisciplinary team members aware of changes in the patients' conditions.

The functions of the RN Care Coordinator/Maternity Coordinator include, but are not limited to

• Contacts pregnant veterans on a regular reoccurring basis throughout the duration of
  pregnancy and the postpartum period to assess clinical and/or social care needs, facilitating
  care and services between VA and non-VA care providers.
• Assists veterans who have presented to VA or non-VA emergency department for gender specific
  maternity related emergencies, to secure appropriate follow-up care such as lab,
  ultrasound, emotional support, and/or other care as needed.
• Ensures seamless and timely return to Primary Care following delivery by assisting with a
  follow-up appointment within designated timeframe.
• Documents patient care interactions accurately, succinctly, and timely.
• Assists with managing Teratogenic medication communication, prescriber education and
  electronic medical record alerts and counseling documentation.
• Completes initial and re-assessments of patient condition and response to treatment and other
  clinical findings to include mental health screenings, and other screenings. Initiates follow-up as
  needed.
• Provides a full range of nursing care to patients with complex care needs.
• Applies problem-solving skills and communication skills; works as a member of the Women
  Veteran Program, Comprehensive Primary Care-Women's Health Team, educating staff,
  learning from staff and improving processes resulting in improved patient care.
• Participates in committees to ensure an environment of dignity, respect, privacy and safety for
  women Veterans.

The Social Worker is assigned to the West Texas VA Health Care System Patient Aligned Care Team. The Social Worker must use a high level of skill in assessing and treating the complicated psychosocial problems of veterans. Social Work responsibilities include the assessment of psychosocial problems, often at a time when the Veteran is experiencing crisis and include the provision of short-term, solution- focused counseling with veterans and/or their family members/significant others. Referrals typically originate from members of the interdisciplinary team, from Veterans and/or their family members/significant others coming as walk-ins to seek assistance, and from community professionals.

The Social Worker is responsible for the monitoring of, and assisting with discharge planning when necessary for, Veterans assigned to the PACT Team while Veterans are hospitalized in any number of local medical facilities. The Social Worker must complete thorough assessments to determine the presenting problem to determine in- home factors impacting the problem and to determine obstacles to treatment and/or follow through with recommended treatments. Consultation with members of the outpatient interdisciplinary team is provided to coordinate a collaborative effort to meet the agreed upon goals for the Veteran's treatment needs and include liaison with community professionals regarding needed services for veterans and/or their families.

Works with various patient populations that include, but are not limited to, homeless Veterans, Hospice/Palliative Care, Homebound, and Spinal Cord Injury and Disease Veterans. Works with Veterans needing assistance with, but are not limited to, Home Health services, financial resources, VA benefits, Long term care decisions, and transportation services.

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).
