To carry out health assessments and be competent in reporting back health assessments to the wider MDT. To identify barriers to effective communication that will regularly be evident including: cognitive impairment, expressive and receptive communication difficulties, embarrassment, anxiety, pain, fear and visual/hearing impairments; to develop and implement strategies to overcome these barriers. To use communication skills to address language and culture diversity issues. To maintain accurate and up to date documentation in line with legal requirements and communicate assessment and treatment results to the appropriate disciplines. Provide specialist palliative care advice and support to patients and their families who have life limiting illness and reside in North Hertfordshire, Stevenage and Royston areas, 7 days a week (including band holidays) 9-5pm. This could be in a clinic, virtually or in the patients home. Provide specialist palliative care advice and support to generalist nurses, care agencies, health and social care professionals including doctors and consultants, to ensure high quality individualised patient care. This will include support with education and training in conjunction with the band 7's in the team. Participate in triaging new referrals to the combined specialist palliative care services across East and North Hertfordshire, based at the integrated Care Co-ordination Centre at Abel Smith House in Stevenage. Work towards enhancing your palliative care skills including symptom control with the support of the band 7 team members. Work independently as an autonomous practitioner and as a member of a multidisciplinary team whilst being aware of your personal well being and professional boundaries. Maintain confidentiality at all times and manage information sensitively. Demonstrate discretion, dignity and respect when communicating with patients, colleagues and other agencies.
