The following are the core responsibilities of the Medical Secretary. We are looking for an experienced Nurse Associate, who acts within their professional boundaries, supporting and working alongside a team of healthcare professionals, GPs, and a non-clinical team in general practice. In this role they will be supported by a GP and Manager who will develop, manage, and mentor them. The post holder will work as part of a multi-disciplinary team in a patient facing role. The post holder will take responsibility for areas of chronic disease management within the PCN. The post holder will provide primary support to general practice staff and carry out Chronic Disease Management annual reviews delegated by a supervising clinician or senior colleague. The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. The post holder will ensure that the practice integrates within the community to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. The post holder will be required to monitor and support the lead nurse with the management of patients with long-term conditions/chronic diseases, using a holistic patient centred approach. You will also be required to take vital signs and observations and identify signs of improvement, report any deteriorations or concerns, e.g. pulse, blood pressure, respirations, oxygen saturations, peak flow, blood glucose levels, ECG, assessing nutrition and hydration and undertake pathology tests, for example urine culture, swabs and undertake wound care. You will help patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in general practices, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines. The post holder will provide support to general practice staff with regards to prescription and medication queries. Carry out medicines switches in GP practices in line with practice and CCG agreed specific protocols reviewing patients records; amending records and informing patients. They will help support the repeat prescriptions system promoting electronic repeat dispensing and online ordering. Carry out housekeeping whilst in patient records e.g., dose optimisation and medicines synchronisation. You will report directly to the SENCO for professional, personal and organisational direction.

You will take a lead on ensuring that academy works in accordance with its duty outlined within the SEND Code of Practice.

You will manage the EHCP process for the team ensuring that reviews are chaired, attended and noted with reports provided in line with guidance.

You will ensure that learner outcomes are recorded and performance against targets monitored, reviewed and recorded against EHC Plans for review purposes.

You will work closely with the Learning Support Coordinator(s) to ensure that support for learners’ requirements is implemented consistently across academy and in line with learners’ outcomes outlined in EHC Plans.

You will collaborate effectively across the academy, Local Authorities, learners, parents, carers and other stakeholders as and when appropriate and required.
