The post-holder will provide specialist advice and support on the development of public health/health promotion programmes and support community health development approaches and initiatives to address health inequalities with a particular emphasis on healthy weight and physical activity. The post-holder will have a lead role in developing and commissioning projects and programmes within the public health directorate's strategy focusing on driving forward the delivery of the whole systems approach to healthy weight and increasing physical activity. In addition, the post-holder will play a key role in supporting and facilitating referrals into the Councils Live Life Better Derbyshire Service that supports local residents to improve their health and wellbeing. We are seeking an enthusiastic and motivated Advanced Therapy Support Worker to join us caring for patients in the community.

We are a responsive, dynamic, multi-disciplinary community team workingwith patients within their own homes, care homes, community support beds and in-patient setting, our service aims to prevent hospital admissions and facilitate discharges; as well as rehabilitation for patients.

Our integrated team comprises of Occupational Therapists, Physiotherapists, Advanced Therapy Support Workers and Generic Support Workers; as well as working closely with our Nursing teams and Social Services to provide an excellent service to our patients. This role is expected to provide cross cover support for our rapid response therapy team in the community and our rehabilitation beds locally in Oakland Village.


The successful applicant will have generic therapy assessment and treatment skills, be able to work as part of a team and will have support from a registered therapist.

Excellent communication skills are essential.

There may be a requirement to work within triage, to support the flow of referrals into the team and be able to liaise with patients/carers/MDT members and referrers from a wide range of services.

Access to and ability to use transport as travel between and across care settings will be required.

The community therapy service is provided 8am-6.30 pm 7 days a week.





This will include initial assessment and reviews of patients, creating specific patient led care plans and meet therapy goals in line with competencies and liaison with other specialist services within the Trust.

The post holder will be able to work independently and adhere to Trust Lone Working policies.

To communicate information to patients and carers regarding their health needs and associated treatment plans.

To report and take appropriate action regarding any change in patient condition or circumstance and to communicate verbal and written information to all relevant integrated care team members.

To support senior therapy staff in the on-going development of services.

To maintain and record accurate health records using system one/TPP in line with DCHS policies.

To observe personal duty of care when using or supplying equipment, to ensure compliance with health and safety regulations and to monitor and report to appropriate registered clinician on the effectiveness of equipment and aids to patients’ homes to maximise use of resources.

The post holder will organise and prioritise their own day- to-day work tasks and activities, be able to work independently and adhere to Trust Lone Working policies, as well as delegate tasks to colleagues and support administrative tasks for the day-to-day running of the services.
