Responsible for co-ordinating and communication with project stakeholders including operational managers, clinical leads, front line staff, patients and carers Provide facilitation for project team meetings as well as supporting the delivery and facilitation of larger events Provide leadership in setting timescales, managing project milestones and overall successful project completion for specific projects within own portfolio Regularly collect, analyse and present data for improvement in a variety of formats Support the project teams in the use of data for improvement, monitoring, measuring and making improvements in services and identifying when changes are not delivering anticipated results, escalating concerns and helping resolve to meet project aims Generate project specific reports at agreed time points Ensure that all projects within the improvement programme are provided with the appropriate level of implementation support throughout the project lifecycle and be flexible in accommodating what support is required, with direction if required from the Cancer Improvement Programme Manager and / or Head of Cancer and Palliative Care Ensure that there is detailed programme of work that drives the progress of projects and subsequently the implementation of the project Identify and manage day-to-day project actions, issues, risks, dependencies and changes for projects within own portfolio Take every reasonable opportunity to maintain and improve your professional knowledge and competence Participate in personal objective setting and review, including the creation of a Personal Development Plan in conjunction with the Programme Manager or Head of Cancer and Palliative Care Ensure that effective Clinical and Corporate Governance systems are embedded into any change process introduced across the Trust. Support service leads to undertake risk and equality impact assessments, in relation to current projects and proposed developments. Develop services in line with the principles and models agreed from any pilots undertaken, local innovation, best practice and national standards.
