We are looking for a GPHC registered or soon to be registered Pharmacy Technician to join our enthusiastic and motivated team based at Winchester in our brand-new purpose-built pharmacy department.

This exciting, fully supported progressive rotational post would be suitable for Pharmacy Technicians looking to make the move into hospital pharmacy or those soon to qualify and register with the GPhC. It is a great starting opportunity to develop new skills and to get an introduction into Hospital Pharmacy whilst being able to rotate around different areas of the department to discover what makes you passionate.

This post will require some cross site working due to one of the four rotations being on our Basingstoke site.

You will rotate through the pharmacy department gaining experience in each of the key areas of the pharmacy service, this involves Dispensary, Medicines Management, Medicines Information and Aseptics. On successful completion of the transitional rotational pathway the post holder would then progress to a band 5.

Managerial Duties Deputise for the Specialist Technicians in their absence. Supervise the work of Pharmacy Technicians, Students and Pharmacy Assistant: maintaining standards of service and ensuring completion of all specified documentation. Conduct medication reconciliation for patients recently discharged from the hospital to ensure accurate medication management. Work alongside GPs to perform medication reviews for frail patients, focusing on reducing polypharmacy and improving safety. Provide advice and support to the frailty team on best practices for medication management, including safe prescribing. Collaborate with the multidisciplinary team to deliver proactive care reviews for long-term conditions and frailty management. Support care home medication management where required. Ensure compliance with regulatory and governance standards in all areas of medication management. Responding to patients questions both face to face and over the phone; referring problems or queries to the pharmacist/GP Providing advice and information to patients. Informing patients regarding changes to their medicines in a professional manner and have the capacity to support and reassure patients when necessary, dealing with any issues or concerns that may arise within the boundaries of the post. Support improvements in clinical care through regular practice-based audit and implementing change. Promote cost-effective, safe, evidence-based prescribing in accordance with national and local guidance. To record all relevant information in the patients medical notes and interventions. Carry out housekeeping whilst in patient records e.g. dose optimisation and medicines synchronisation.
