To provide a comprehensive range of clinical duties at the agreed Surgeries within our Primary Care Network, which may also include working within our Enhanced Access Service. To undertake surgery consultations, home visits where necessary, and telephone queries from patients and health care professionals. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation To undertake on-call duties as part of the Surgery on-call rota, where agreed. This may include requesting that patients present at the surgery, telephone consultations, home visits and triage. Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions Assess, identify, and refer patients presenting with mental health needs in accordance with national and local guidelines. Assessing the health care needs of patients with undifferentiated and undiagnosed problems Screening patients for disease risk factors and early signs of illness In consultation with patients and in line with current practice disease management protocols, developing care plans for health Recording clear and contemporaneous consultation notes to agreed standards. Collecting data for audit purposes. Compiling and issuing computer-generated acute and repeat prescriptions.

Medical services Based in Sheldon/Kenyon ward on Mondays to Fridays 09:00 17:00. There are no on-call duties but will involve day-to-day in-patient care including: - Clerking of admissions - Assessment and formulation of management/discharge plans in conjunction with the MDT - Mental capacity assessment - Management of acutely deteriorating patients - Palliative care - Meetings and case conferences with families. All the doctors working within the Medical Unit will work as a TEAM. Flexibility in cross over arrangements is expected at times of annual leave, sickness and study leave, or any other absences of colleagues. Undertaking supervision of junior doctors with the care and management of patients on the medical wards or outlying wards. Providing cover for general ward duties eg when trainee doctors obligatory training coincides with other medical staff commitments. Assisting with inpatient or day-case patients for intravenous therapies (supervision and prescribing) or specific endocrine tests; liaison with nurse practitioners involved.

The post holder will uphold and display Trust values of Commitment, Care & Quality Responsibilities for Catering Assist patients to order food, where necessary taking into account special needs and medical requirements. Liaise with Kitchen Manager /staff to ensure food provided is of a consistent high standard and meets patients needs Prepare and serve hot/cold snacks (e.g. toast) as requested in line with Trust policy. Take joint responsibility with the nursing/midwifery team to serve meals, ensure patients intake is recorded and reported and that dietary needs are met. Ensure red trays are provided for all patients deemed nutritionally at risk. Provide eating assistance, e.g. cutting up food, placing food within reach, and encouraging patients to eat. Ensure all food and beverages are served at the correct temperature according to food hygiene regulations. Take responsibility for the Ward kitchen, ensuring it is clean at all times, e.g. discarding out-of-date food, stock rotation and labelled correctly Ensure patients whose fluid intake is not medically restricted have easy access to fresh water (e.g. jugs and glasses). Responsibilities for Cleaning Assist in monitoring and maintaining cleaning standards on the Ward, which will include spot cleaning when required. Transport and dispose of clinical, domestic and confidential waste (outside of the domestics duties) as per the policy, e.g. taking waste to the ward disposal area. Clean and prepare beds and making sure there is adequate linen for the ward. Ensure the patients bed space is respected including attending to patients fruit, flowers, etc. Responsibilities for General Duties Receive, welcome and guide visitors on the Ward, liaising with other staff as appropriate. Transport equipment, consumables and written information as required. Maintain and update non-clinical records and information as required. Receive and pass on information to others, maintaining confidentiality. Deal with initial complaints and conflict if required in a polite, calm manner and report to Nurse/Midwife in charge. Assist with training of staff involved with Ward Housekeeping services into Ward routines to achieve required standards. Ensure their own actions reduce risks to health and safety and to promote a health and safety culture within the workplace. Respond to emergencies as appropriate. Accountable to the General Manager, and working in partnership with Service Directors and Matrons, the Deputy General Manager (DGM) will support in the operational delivery of Emergency and Acute services, focussing on delivery of the A&E waiting time standards, financial performance, governance, change and business development across the directorate. The successful candidate will be responsible for ensuring good governance and processes for monitoring delivery against all KPIs are in place. The Deputy General Manager will play an important role overseeing development of the Same Day Emergency Care (SDEC) pathway, ensuring the smooth transition of patients between primary care, ED and Ambulatory Emergency Care (AEC). The DGM will ensure this is a seamless pathway at the CWH site and will line manage the Service Managers based within A&E and Acute Medicine to ensure continuity across the entire service within the Trust. Ensuring the smooth flow of patients through A&E, for any acutely unwell patients requiring admission to an acute assessment unit, the DGM will be responsible for ensuring adequate levels of medical staffing with the support of a team of medical rota coordinators. Furthermore, the DGM will support in the development of strategic discharge pathways in collaboration with the site team. The Deputy General Manager will work alongside the General Manager in the delivery of the Directorate financial, quality, performance, productivity and workforce targets, agreed as part of the Divisions business planning, appraisal and personal development process. There will be an important component of the role which requires the successful candidate to have a transformational mind-set, approaching challenges with an inquisitive and constructively critical perspective where innovative solutions are found to complex problems. The Deputy General Manager will be accountable for establishing and maintaining effective structures and systems of control for the management of staff, including recruitment, appraisal, continuing professional development and support of the Trusts Human Resource policies and procedures predominantly at the Chelsea and Westminster site, but ensuring continuity at the West Middlesex University Hospital site. The Deputy General Manager will be expected to establish collaborative relationships with colleagues, working in partnership within the Trust and with external agencies to ensure delivery of the divisions business plan and strategic vision. The Deputy General Manager will ensure that line management of his or her staff is undertaken professionally and that each member of staff has a development and training plan consistent with Divisional and Trust objectives. The candidate will be required to line manage the Service Managers to deliver an effective and aligned service across the organisation and support all the staff whom the Service Managers are responsible for the line management of. The Deputy General Manager will be expected to work between the two main hospital sites, delivering a consistent level of cover and leadership between the sites.
