Major duties of the Supervisory Health System Specialist (Corporate Compliance Officer) include:

• Responsible for the initiation and completion of administrative investigations and fact-findings as approved by the Health Care System Director.
• Directing, developing, implementing, and overseeing a multi-disciplinary, health-care-system-wide Compliance Section within the scope of the VHA mandated Privacy Program, Records Information Management Program, Conflict of Interest, and Compliance & Business Integrity (CBI) Program (which includes auditing, monitoring and creation of corrective action plan activities in the areas of coding, documentation, purchased care, exclusionary lists, conflict of interest, accuracy of patient registration, etc.), Controlled Substance Program, VHA policy related to Administrative Investigative Boards, VHA policy related to Access, and VISN equivalent of each of these programs.
• Establish goals and work plans for the Compliance Section and ensures the implementation of the goals and objectives of the section.
• Determine issues, areas, and/or subject matter that create risk for the organization and makes recommendations to executive leadership as to how to avoid or minimize the identified risks.
• Creates written policies and procedures that promote commitment to corporate compliance and address specific areas of risk associated with the Health Care System business, practices, operations, privacy, Records Information Management, investigations, and audits of scheduling and consult practices.
• Creates and maintains local policies governing each of the disciplines within Compliances.
• Creates a formal, written training and education plan for all disciplines with the Compliance Section.
• Monitor the implementation of process improvement projects that are the result of findings made by the Compliance Section.

The Accreditation and Quality Specialist (AQS) Consultant Nurse serves as a Specialist within the Accreditation & Quality Management Programs. Serves as the point of contact and primary support (logistics such as arranging workspace and meals and coordinating staff for interviews) for site visitors engaged in the accreditation process. Manages processes related to survey readiness, IPEC monitors, SAIL data, EPRP data, tracers, chart reviews, and environment of care surveillance. Compiles regulatory data and communicates information to the Chief of Quality & Patient Safety in a format that is conducive for utilization in decision making, strategic planning and performance improvement. Chairs monthly meetings in support of deploying a systematic approach to readiness and integrating readiness activities in all areas. Rounds frequently with staff to work with frontline staff at the unit level on accreditation readiness issues.

The incumbent works with team members of the Quality and Systems Improvement Department.
Incumbent tracks, analyzes, and trends regulatory action plans and monitors for sustainment of
improvement. May participate in root cause analysis, peer review, Lean Six Sigma teams, EPRP monitors, and employee surveys. Works in collaboration with the Patient Safety, HRO, Innovation, Systems Redesign, Risk Management, Infection Control and Utilization Management teams for tracers, chart reviews and QI initiatives. Troubleshoots issues related to quality of care and provides real time feedback to leadership. Responds on site in real time as incidents occur to investigate and prepare follow up reports during administrative hours; follows up next administrative day on occurrences during non administrative tours. Performance other duties as assigned to meet the needs of the Quality and Patient Safety Service, Veterans, and the organization.

Work Schedule: Monday - Friday, 8 hour tour between 7:30am - 5:00pm.

The incumbent is responsible for the following: trending and analyzing data, interpreting Joint Commission and other external agency standards, interpreting VHA handbooks and directives regarding quality management/performance improvement/patient safety issues; collaborating with the facility Patient Safety Officer; collaborating and assisting in system redesign processes and monitoring progress of reviews and follow-action items to ensure completion in a timely manner.

The incumbent is responsible for coordinating and maintaining a comprehensive, organized, continuous readiness program to review and identify opportunities for improvement in order to achieve excellence in the delivery of healthcare. Assists in developing Quality Improvement processes to assure that all required information is procured and acted upon to meet the expectations of the facility, Joint Commission, VHA Headquarters, Office of the Medical Inspector, Office of Inspector General or other external organizations. Responsible for all directives and standards relating to the Quality Management program for areas within assigned responsibility.

The incumbent will assist the Service Line and ensure performance improvement processes are in place, follow-up is conducted, and outcome reported. When required, he/she conducts reviews to evaluate compliance with program regulations, policies and procedures and assists with the development of facility policies and procedures as appropriate. Assists in develop Quality Improvement approaches for standardizing system related procedures to minimize variation. Supports facility activities in the coordination and development of data collection and analysis techniques, primarily in the areas of specific Performance Measures and Performance Improvement indicators.

Incumbent is knowledgeable of Performance Improvement principles; statistical processes including trend and comparative analyses and has the expertise to make recommendations based on findings. Reviews all performance improvement data for assigned areas and assists responsible staff in initiating/facilitating appropriate action based on VA guidance documents. Facilitates education of personnel, both in the facility orientation of employees on performance improvement and in specific training for those involved in the performance improvement activities within services, on hospital Quality Improvement committees and/or within process action teams.

The incumbent conducts various patient care studies based on areas assigned, including review of patient incident report reviews, utilization management reviews, medical record reviews for evidence of required documentation as well as the quality and level of care provided. Provides clinical expertise and consultation to assist in data driven decision making and performance improvement matters. Conducts tracers, operational assessments and participates in the design of special studies to define the cause and scope of issues or trends and assists in the identification of appropriate criteria for problem resolution. Analysis includes trend, pattern and other statistically valid techniques to assist in focusing program attention and efforts. Participates in the development of innovative approaches to the establishment, enhancement, modification, and/or improvements in a variety of facility programs related to performance improvement/quality management. Reviews service/section/committee meetings for results of assessment and improvement activities including conclusions, recommendations, actions and evaluations. Closely monitors and advises services and committees of Quality Improvement topics that need to be addressed at meetings in order to meet The Joint Commission standards compliance.

The Marion Illinois, VMAC is seeking a full time Quality Management (QM) Consultant Registered Nurse (RN) to join Quality Management.

The QM Consultant has position responsibilities that demonstrate experience, and creative approaches to improvement in quality-of-care outcomes. The QM Consultant demonstrates performance that improves the care for a group of patients. The QM Consultant is responsible for the documented outcomes at the program level. Program level outcomes must be demonstrated at any organizational level within a department and/or facility.

Program is defined as a substantial coordinated group of activities impacting patients and/or employees. The QM Consultant is assigned responsibility for their assigned quality management program area activities for the Medical Center and all off-site locations. The QM Consultant collects data, monitors staff compliance with relevant policies, directives, and standards, provides reports detailing trends, and provides direct staff support to all services in planning, designing, integrating, implementing, modifying, and administering, a comprehensive quality management program area. The QM Consultant participates in facility councils, committees, and workgroups. The QM Consultant of Primary Care will conduct quality-of-care reviews as assigned and within the required timeframe.

The QM Consultant of Primary Care acts as a liaison to medical center committees or services to assist in the identification of Quality Management activities, Joint Commission requirements, Office of the Inspector General requirements, and other performance measure activities. The QM Consultant of Primary Care communicates and interacts appropriately and courteously with internal and external customers both in individual and group settings. The QM Consultant of Primary Care maintains confidentiality of patient/employee information electronically, verbally, and written. They perform in a safe manner including infection control practices and in operation of equipment. The QM Consultant of Primary Care integrates and applies pertinent aspects of the Medical Center Quality Management Program into their professional practice. They utilize current evidence- based nursing research and literature in daily practice. The QM Consultant of Primary Care may lead or assist with coordination, education, and sustainment of facility wide tracer programs to promote continuous readiness including but not limited to performing patient and system tracers. The QM Consultant of Primary Care may serve as a member of interdisciplinary groups as directed by the Chief of Quality Safety Value and High Reliability (QSVH) or designee.

They may have other duties as assigned by the Chief of QSVH or designee. The QM Consultant for Primary Care demonstrates Patient Centered Care (PCC) using effective communication with patients, families, visitors, and members of the interdisciplinary team, including the patient in mutual goal setting, and providing patient advocacy when appropriate. Disaster/Emergency Response: RN's have an inherent duty to rescue by the nature of their license and training.
