An exciting secondment has arisen for a District Nurse or Community Nursing Sister to support in the leadership of the Poole Central locality Community Nursing Team.

The team have a breadth of experience and a shared goal to offer the highest standard of care to our patients. We work closely with other health and social care professionals across primary and secondary care, as well as with patients, families and carers in their own home.

Your role will be to lead the team and case manage the provision of care within an individual's home or residential home, including palliative care and long term conditions management. You will work autonomously using your own initiative to deliver holistic care to adults with physical health needs.

You should possess excellent leadership abilities, interpersonal skills and time management skills. You should have experience in caseload management and be able to prioritise work. We welcome applications from enthusiastic clinical/counselling psychologists or psychotherapists, with a special interest in working with older people. Whilst this post is suitable for a band 8A, we would happily support the career development of a band 7 via a preceptorship programme.

As a clinical psychologist you will provide highly specialist psychological assessments and interventions to service users in our Community Mental Health Teams (CMHTs). Duties will also include working with families and carers as well as allied care agencies.

Your will manage a complex caseload autonomously and make decisions regarding treatment options considering evidence base and various therapeutic models. You will monitor clinical progress through regular reviews of psychological treatment as well as outcome measures.

In line with experience and role development plans, you will provide supervision and mentorship to junior clinicians and trainees, contributing to their professional development and enhancing the treatment they offer.

The Community Therapy Team (for Bournemouth, Poole and Christchurch) at Dorset HealthCare University NHS Foundation Trust is looking to recruit a motivated and enthusiastic Band 3 Rehabilitation Assistant.

The successful candidate will support Occupational Therapists and Physiotherapists providing and delivering rehabilitation interventions to adults in the community.

An exciting opportunity has arisen for a Band 4 Activity Coordinator to work as part of a multi-disciplinary team on Twynham Low Secure Unit; a 12 bedded Low Secure Unit on St Anns Hospital Site in Dorset. The role will involve working closely alongside the Occupational Therapy team, Nursing Staff and Psychologists to support the development and maintenance of structure and routine into inpatients care and treatment. The focus of this role is to support the discharge pathway of each inpatient on Twynham ward through the structured ward activity programme and linking with external agencies to the ward. The successful applicant will join a well-established Multi-Disciplinary Team, which provides assessment, treatment, and rehabilitation to patients with a mental disorder and associated offending behaviour, whilst being committed to clinical excellence and the provision of high quality evidence based intervention.

We are seeking to recruit a motivated, enthusiastic and dedicated Activity Co-Ordinator who has experience in working with people from a range of diverse social and ethnic backgrounds. Applicants must demonstrate a genuine desire to work in forensic mental health and commit to delivering the highest quality of patient centred care using national quality standards and evidence-based practice.

The successful applicant will be expected to support the inpatient Multi-Disciplinary Team on Twynham Ward in organising, co-ordinating and where needed delivering a wide range of client focused groups.

Come and join our friendly and supportive Facilities team as a Specialist Clean Operative, based in Poole. You will require a clean, full UK Driver’s License for this role.

You will be required to carry out specialist cleans in our wards and kitchen areas as well as our theatre’s; working in conjunction with site domestic staff and ensuring the maintaining of National Cleaning Standards. Training will be provided for any machinery used during these cleans.

This role is a full-time role, hours are 8am – 4pm with some flexibility required. There will be times you will be required to cover all Trust sites located across Dorset. Work vehicles are provided for work use only and will need to be returned daily to the relevant base.

This role will suit someone who enjoys variety in their daily workload, enjoys working as part of a team and able to work independently on occasions.

Communication is key to this role as the successful candidates will be interacting with a variety of staff and on occasions patients in the course of their work.

This is a physical job requiring the ability to work at height from ladders, and the use of floor equipment.

Full training can be provided where previous experience is not available, as we are seeking candidates with a can-do attitude who want to contribute to the patient environment and recovery.

This is an exciting new opportunity to lead in the delivery of improved health outcomes for people with a learning disability. The Practitioner will work with system partners, the community sector and people with a learning disability to increase uptake of the national screening programmes. Access to health care is a leading cause of avoidable death in people with a learning disability and this new role offers an opportunity to make enduring change in the lives of people.

The overall aim of the service is to increase breast, bowel and cervical screening, AAA and diabetic eye screening uptake in people with learning disabilities. The Dorset-wide service will be provided by a practitioner with knowledge of and experience in supporting the needs of persons with learning disability to access health and care. The post will be hosted byDorset Healthcare University NHS Foundation Trust with leadership from the Trust Lead for Health Inequalities (and Consultant in Public Health). In partnership with system stakeholders including voluntary and community sector organisations, the post holder will:

· clarify reasons for persons being on hold or ceased from programmes,

· enable case finding of persons with learning disability and eligible for screening,

· undertake a baseline assessment in order to develop and implement an improvement plan in relation to routine delivery of the five screening pathways, to include for example workforce education and training, development of reasonable adjustment and co-production of resources with lived experience partners.

Robust monitoring and evaluation, and reporting will be agreed.

The Home Treatment Team based at St Ann’s hospital is looking for a Band 3 Mental Health Support Worker on a full time basis 37.5 hours per week, to join our fun, friendly and fabulous team, who have patient care at the heart of all that we do!

The Home Treatment Team East (HTTE) offers an alternative to hospital admission for service users aged over 18 with mental illness through intensive home treatment. We pride ourselves in working flexibly with patients and supporting their carers when in a mental health crisis.

This role requires you to provide a supportive role for carers and service users accessing the Home Treatment Service, carry out assigned tasks as identified by the registered nursing staff in relation to patient needs, to provide support, give time to individuals, and thus promote their
recovery and assist them to remain in the least restrictive environment.

This role will require the worker to undertake shift based work. The service hours cover the period 08.30 – 22.00, seven days per week.

To provide a supportive role for service users and carers accessing the Home Treatment Service, in order to promote their recovery.

To provide high quality care and treatment in conjunction with other professionals through the Trust’s agreed multi- disciplinary assessment and treatment/ care programmes.
