The post holder will act as a Parkinson’s Disease Nurse practitioner through strong clinical leadership andeffective management and will support primary care services within Neurology and Elderly Care Service in Maidstone & Tunbridge Wells Hospital NHS Trust and Kent Community Health Foundation NHS Trust to promote and deliver nurse excellence for adults with Parkinson’s Disease. This role requires a good level of competence, nursing expertise and evidence based clinical care and decision-making skills.

To be able to work autonomously in managing a defined caseload of adults with Parkinson’s under the supervision of Band 7 PD Nurse Specialist. The post holder will be involved in direct patient care for adults with Parkinson’s Disease from initial diagnosis through to the palliative stages of the illness, by providing advice, assessment and medication management to patients, their families/carers and professionals.

The post holder will be responsible for delivering nurse led clinics on a 1:1 basis with patients as well as providing community visits and supporting educational patient groups for patients who are newly diagnosed with Parkinson’s and potentially offer an in-reach service to local hospitals to prevent hospital admissions and coordinate better care packages upon discharge where needed.

To assist the Community Parkinson’s Disease Specialist Nurse in the provision of the specialist nursing service for all adult patients with Parkinson’s Disease across the West Kent locality.
• To manage a caseload of clients requiring specialist Parkinson’s Disease nursing, within the community.
• Undertake assessment and treatment of clients within both clinic and community setting, who have
complex / and or chronic presentation requiring specialist skills; and determine profession specific and multidisciplinary rehabilitation interventions.
• To provide advice, education and support to patients, their carers and healthcare professionals, and assist in coordinating treatment and care.
• To work across professional, departmental and Trust boundaries to support the delivery of a patient-centredseamless service.
• To work in close collaboration with the Community Neuro & Specialist Services, delivering and evaluating care pathways for clients with long term neurological conditions.

This is a full-time role and a car driver and access to own vehicle is essential to the role.

You will be supported by an administrative assistant, to ensure the Multidisciplinary meetings are set up and planned effectively.
With support from the admin assistant, you will manage the incoming referrals for the MDT, ensuring all members required are invited, and have appropriate information prior to the MDT.

You will require strong IT skills to ensure all actions are uploaded to the correct systems in a timely way.

We have an exciting opportunity to recruit an NMC Registered Nurse Associate to join the existing team based at Maidstone. This is a seven-day service, working between 08:00 and 16:00, with a late shift of 10:00 – 18:00 on a rotational basis.
The team support patients from GP practices within the Maidstone ABC Primary Care Network (PCN).
The teams provide nursing care to predominantly housebound patients in the Maidstone town centre area. Each team takes responsibility for a defined geographical patch, which allows good working relationships with local GPs, care homes, Social Services, voluntary organisations and other stakeholders.

The District Nursing Service also provides local clinics for patients requiring their clinical skills. We believe the addition of the new Nursing Associate role will complement the current skill-mix for providing nursing care within a community setting.
This is a highly rewarding role, and nurses are supported to achieve all the clinical skills required for the role.
Clinical areas will include wound management care, end of life care, medicines administrations and bladder and bowel care, amongst other duties.
You will be supported by senior staff and the team; therefore, these posts will suit those wanting to move to a community setting.
Staff will be required to drive their own car and are able to claim mileage costs and access the NHS car lease provision, following their probationary period.

We are looking for a Band 6 nurse to join one of our Community Nursing teams based in ABC PCN (Maidstone), to support the development of this service. The service operates seven days a week from 08:00 - 16:00, with occasional late shifts of 10:00 – 18:00 on a rotational basis. Weekend working is generally on a 1:4 ratio with days off during the week.
The team support patients registered with GP practices working in the Maidstone town and outlying villages within the ABC Primary Care Network.
Each team has two senior nurses to take responsibility for a defined geographical area, and provide clinical leadership to their Band 5, Band 4 and Band 3 staff. The team will work together to manage caseload patients delivering a high-quality service.
