The post holders will work under the direction of the MHST Service Lead and Practitioners within the project, across the secondary schools, special schools and our Pupil Referral Unit (PRU) within the Fylde Coast footprint. Joint working with Options4CYP (Children and Young People) and other teams in the wider system will be essential, to support the emotional wellbeing and mental health needs of children and young people. All teams working on the Fylde Coast with children, young people and families have a strong work ethic of support for each other, with all team members seen as equals. The services have been established for many years and have now expanded to encompass Mental Health Support Teams, with a range of professionals and dedicated admin staff. We have an ethos of person-centred service for our clients as well as good self-care being encouraged within the team. NHS Blackpool Talking Therapies Service operates from a number of community bases across the Blackpool area. We offer interventions and psychological therapies across Step 2 and 3, and have developed pathways with physical health services in line with the National NHS Talking Therapies Long Term Conditions programme. We receive self-referrals and referrals from health care professionals for the Long-Term Conditions arm of the service.There is an expectation that when qualified you will undertake the Long Term Conditions training. As you will be on a Trust secondment contract, in the event of needing to withdraw from the programme. On completion of the course you will be placed into a permanent band 4 Nursing Associate role within the Trust, however this may not be in your current work place. During training you will be paid a Band 3 and there will be an expectation to work 37.5 hours. If successful in securing a place on the course you WILL NOT be eligible for student finance.

To provide an efficient and high quality diagnostic and procedural coding service ensuring, through participation in audits of the coded clinical data, that the coded clinical data meets the required national and international classification rules and conventions. To maintain accurate and comprehensive clinical coded data which has a direct impact on the finance and funding for the Trust. To maintain knowledge base and to work unsupervised on routine coding issues, applying knowledge of the latest developments in clinical coding, taking advice from the experienced team members when appropriate. To take responsibility for own professional learning and development over and above that which is supplied by the Trust. To ensure that clinical data is entered into the Patient Administration System (PAS) for every patient within an agreed time period from discharge in line with Secondary User Service submission deadlines. Requirement to consistently deliver against demanding deadlines and accountable for maintaining accuracy and compliance with frequently changing national standards and clinical coding conventions. Following successful completion of initial training, under the direction of the coding manager and senior team work autonomously without direct instruction or supervision when self-allocating work and achievement of daily/weekly/monthly coding targets. To interpret histology and radiology reports to ensure completeness and accuracy of the coding process. To ensure that an efficient cancer registration service is maintained. To comply with both Trust and Departmental policies and procedures, adhering to local coding policies as appropriate. In liaison with the Clerical Officer and other Trust staff, determine the location of missing health records, to ensure targets are met. To investigate incorrect patient coded data and liaise with the appropriate staff to correct the data on PAS. To advise, support and mentor less experienced staff on coding issues. To provide cover for colleagues absence during holidays/sickness/etc. to ensure continuity of data collection, coding and deadlines. To communicate effectively with clinicians, nursing staff, other healthcare professionals and departments, both internal and external, concerning routine coding issues. To undertake to attend relevant training/workshop sessions, both internal and external to the Trust, this will require travel off site to various locations. Undertake any additional duties to support the needs of the service as directed by the coding manager and/or senior team

The advanced practitioner will demonstrate and work at an advanced practice level applying their extended knowledge and skills in the principles and practice of palliative care to support patients with the most complex needs. Working as an independent practitioner but in partnership with the medical team the post holder is accountable and responsible for their own actions or omissions in line with the Nursing and Midwifery Councils Code and latest guidance on advanced nursing practice. Scope and Accountability The Advanced Clinical Practitioner is a member of the wider hospice team and is required to: Operate in line with the Trinity values and principles. Practice within the NMC Code and HPC code of practice and adheres to all relevant professional, legal and regulatory guidelines. Develop role within the Scope of Professional Practice and guidance on advanced practice. Assume responsibility for own professional development and update and maintain own clinical skills as required for the role, working in partnership with Clinical Manager, Medical Director and organisation developments. Provide a visible clinical profile, having both an advisory and clinical input into patient care. Provide specialist advice and support to patients and carers. Acts as a role model professionally and behaviourally. Work with the multi-professional team and clinical/non clinical services team to promote learning. Work in partnership with all allied Health professionals, to support and enhance patient care. Key Tasks and Responsibilities Work in partnership with the medical team to set, implement and monitor standards of patient care in line with the philosophy and principles of palliative care. To contribute to the delivery of the Trinity Hospice Mission Statement. Works as part of a Specialist Palliative Care Team, demonstrating expert clinical leadership and practice. Carry out a high standard of clinical assessment using emerging advanced practice knowledge and skills, increasingly determining and providing clinical management plans treatment and interventions, prescribing where appropriate. Maintains clinical records in line with hospice and NMC guidance. To develop with supervised training, advanced clinical judgment to plan, implement and evaluate care for patients with complex palliative needs in the in-patient unit, hospital and community settings. Learns to anticipate problems and risks and takes action before crises arise. Liaise with the Multi-Disciplinary team and first contact team to facilitate effective discharge planning. As an independent prescriber, be able to prescribe safely in accordance with local policies, national standards and formularies relating to independent prescribing. Acquire knowledge and skills to influence the setting of standards of practice, and to participate in the monitoring of standards to ensure best practice and the delivery of evidence based and cost effective care, monitoring and reviewing care plans. Provides a source of expertise to all health care practitioners, including undertaking joint assessments as required with members of the wider MDT. Identifies clinical issues and incidents within the specialty that may affect the quality of palliative care services. Working with the nursing teams, Clinical Manager, specialty doctors, Consultants and Medical Team to ensure the delivery of a safe effective service.
