To work as an integrated member of the Urgent Community Response Service delivering person centred care, assisting Occupational Therapists, Physiotherapists, Nurses, Paramedics and the wider multidisciplinary team with patient interventions. Understand the entire programme and the associated timelines and develop detailed project plans to deliver the required outcomes for agreed area/s of scope.
