The roles and duties of this post include: Managing pharmacy technicians in the locality to ensure medicines management processes are to a high standard across all sites. To provide a comprehensive medicines management service to the mental health unit, including ward pharmacy visits, multidisciplinary team meetings and community and crisis team support. Promote seamless and integrated care for patients with mental health conditions, supporting the transition of patients across care settings so they receive the required level of care to support them in taking their medication safely in the most appropriate care setting. Support discharge planning to provide a smooth and safe transition of care. Ensure seamless pharmaceutical care across the KMPT teams where the post is based. Be responsible for relevant teaching and educational programmes for the staff in the inpatient, community services and for health care professionals working in primary care. Duties of this post will be to support the consultant service across a range of tumour types: Attending Consultant led out-patient clinics (both in the Centre and Peripheral Clinics), and undertaking related admin and chemotherapy prescribing. Supervision of SHOs and ward patients. Review of patients attending for chemotherapy and radiotherapy if required by nursing or radiography staff. Teaching of SHOs and medical students. Involvement in clinical governance and audit activities.

Key Responsibilities: As a senior leader working in partnership with schools and the wider community, supporting children and young people experiencing mild to moderate emotional wellbeing and mental health difficulties and their families in the self-management of presenting difficulties To take the lead in any complex relationships or system issues with our partnership with Education and working within the wider mental health sector. To support clinical supervisors in their task of overseeing the assessment, formulation and treatment and management plans for schools, parents, children and young people being supported by the MHSTs using a range of psychological interventions. Work in partnership with children, young people, families and schools to develop goals, plans for intervention, and agreed outcomes. To support and empower children, young people and families to make informed choices about support. To operate at all times from an inclusive values base which recognises and respects diversity. Provide overall clinical leadership for the MHSTs within Kent & Medway. Undertake accurate assessment of risk to self and others where needed, and support the service to ensure that risk assessments are applied using clinical knowledge drawing on formulation and best practice guidance. Ensure the MHSTs adhere to the service referral protocols and through supervision to help signpost unsuitable referrals to the relevant service as necessary. Ensure the keeping of coherent records of all activity in line with service protocols and use these records and outcome data to inform decision-making. Complete all requirements relating to data collection. Assess and integrate issues relating to transitions, education and training/employment into the overall therapeutic process. Work within a collaborative approach involving a range of relevant others when indicated. Work in collaboration with children, young people and communities to enhance and widen access to support health promotion. The Elderly Care team with 14 funded Consultant Geriatrician posts (13 in place) currently provide a wide range of hospital and community based services including Acute Frailty services as well as running specialist clinics for patients with Parkinsons disease, falls and general medical conditions. We have recently started a Rapid Access Clinic for Elderly as part of Frailty- Same day Emergency Care (F-SDEC). Inpatient elderly care wards include 5 acute elderly care wards and an established purpose built Acute Frailty Unit with10 Assessment beds, 22 short stay beds and 2 clinic rooms. In addition to existing services, we are seeking colleagues to take on the challenge of developing new models of care for frail elderly people in the community such as Frailty. Pioneering work has already established Integrated Locality Review meetings (MDT meetings, 4/week). Consultants are working closely with existing community teams to proactively assess and manage frail individuals in the community. We are about to open a 22 bedded Acute Unit based in a Community Hospital in Sheppey. The post holder will join the existing team of full time Consultants. The post holder will need to take responsibility for the management of patients admitted to his/her specialty Elderly Care ward (12-15 patients) routinely and those admitted under their care whilst on call for Acute Frailty. The latter would average 12-15 per on call. These patients are then streamed to Frailty Short Stay Unit if early discharge planned (<48 hours) or one of the 5 Frailty Base wards for longer admissions. Acute Frailty Unit has a dedicated Elderly care Team with 2 Consultant Geriatricians supported by their own team of junior doctors and a Multidisciplinary Team. There are 10 funded posts for Middle Grades in the department with currently 6 in post. Elderly care Consultants participate in an Acute Frailty On call service. The post-holder will be paired with another Consultant so that in a 12 week block they provide 2 weeks of weekday on call and 2 weekends on call. Full registration with the GMC and the ability to make decisions at a senior level, the role will allow you the opportunity to continue your personal development, and participate in the teaching, research and governance programs within the Department. All subspecialist interests are welcome. There is an opportunity, in negotiation with the other consultants, for participation in the running of the established clinics such as Falls and Movement Disorder Clinic. The successful applicant will also have an outpatient workload comprising of new and follow referrals for managing complex co-morbidities. Typical clinic would have 3-4 new patients and a similar number of follow-up patients. By promoting meaningful activity and sharing lived expertise, facilitators enable Recovery College students to connect, make sense of their own experiences and recognise their own strengths and skills. Facilitators provide new opportunities for RC students to identify aspects of life that give meaning, hope, value and purpose. Facilitators recognise that each individuals recovery journey is unique and deeply personal process and therefore RC students will each have knowledge, skills and experience. This shared learning environment will enrich the content of workshops and courses and the experience of those present. KEY RESULT AREAS: Co-facilitating Recovery College courses and workshops for Recovery College Students and supporting them in their own recovery and in relation to their own aspirations as part of the multi-disciplinary team. To bring empathy and understanding into all interactions with Recovery College Students by sharing lived experience, empathy, compassion and hope. To model /mentor a recovery process and demonstrate coping skills, empowering Recovery College Students to actively participate in their own recovery journey. Main duties of the role: The role is part of the Additional Role Reimbursement Scheme to embed Mental Health practitioners in Primary Care Networks. The purpose of the role is to bridge the gap between Primary Care and Secondary Mental Health services by delivering high quality Mental Health care in Primary Care settings. Provide senior medical support and guidance to the team Work directly with young people and their families and liaise as appropriate with other external CAMHS mental health providers and acute hospitals Participate in the on-call rota frequency is currently 1:14, with a 3% of basic salary, Category A on-call supplement.
