Working with senior colleagues to drive continuous improvement, you will lead change and challenge conventional approaches, to create a learning organisation, offering expert advice on regulatory requirements, new services, and projects. Supporting the Director of Quality, you will provide professional nursing leadership and advice, supporting with any learning required. Primary Duties and Areas of ResponsibilityClinical Role The post-holder will: Provide a first point of contact within practices for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan. Undertake clinical reviews including diagnosis, treatment and management of both acute and chronic conditions. Provide assessment, treatment and diagnosis at point of first contact, by attending to patients in a variety of clinical or non-clinical settings according to patients needs. Undertake home visits and care home visits, in accordance with the relevant protocols. Make professionally autonomous decisions for which he/she is accountable. Refer patients to an alternative care setting or treat & discharge as appropriate. Instigate necessary invasive/non-invasive diagnostic tests/investigations and interpret findings/reports. Ensure that professional standards are maintained and within the guidance issued by the Department of Health, the Health and Care Professions Council and the College of Paramedics Ensure that there is a robust system in place for maintaining clinical governance. Work with multi-disciplinary teams within the Practice and across the broader healthcare community, to promote integrated and seamless pathways of care. Contribute to the Practice achieving its quality targets to sustain the high standards of patient care and service delivery. Participate in the assessment of community health needs and help to develop patient and family-centred strategies to address them. Help to develop and set up new patient services and participate in initiatives to improve existing patient services. Teaching and mentoring role The post-holder will desirably: Promote a learning environment for patients, colleagues and other health professionals. Contribute to the planning and implementation of the teaching of and support for new and existing staff in the Practice, including medical students, student Paramedics and newly recruited Paramedics. Professional role The post-holder will: Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research. Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review. Maintain any professional registration such as with the HCPC. Participate in continuing professional development (CPD) opportunities to ensure that up to date evidence-based knowledge and competence in all aspects of the role is maintained including maintaining up to date CPR training. Record accurate consultation data within the patients records. Keep up to date with pertinent health-related policy and work with the Practice team to consider the impact and possible strategies for implementation. Work collaboratively with colleagues within and external to the Practice. Pro-actively promote the Paramedic role within the practice and externally to key stakeholders and agencies. Identify own learning needs in order to remain current and improve service delivery. Non-clinical team role The post-holder will: Identify appropriate opportunities to delegate both clinical and administrative tasks to other appropriate staff. Help the practice operate in a cost-effective manner. Identify and manage care risks on a continuing basis. Participate in practice meetings as required. Participate in audits and inspections as appropriate. Work closely with the doctors and managers in the setting up and/or improving of practice systems for monitoring/measuring performance against clinical governance and quality targets. Ensure all practice policies are fully implemented. Health & Safety and Risk Management The post-holder must comply at all times with the Practice Health and Safety policies. The post-holder will comply with the Data Protection Act (2018) and the Access to Health Records Act (1990)Equality and Diversity The post-holder must co-operate with all policies and procedures designed to ensure equality of employment. Co-workers, patients and visitors must be treated equally irrespective of gender, ethnic origin, age, disability, sexual orientation, religion etc. Respect for patient confidentiality The post-holder should respect patient confidentiality at all times and not divulge patient information unless sanctioned by the requirements of the role. Communication and working relationships The post-holder will establish and maintain effective communication pathways with the following: GPs within the practice and externally with the ICB as a whole Whole clinical team including Nurse Practitioners, pharmacists. Reception and Administration Teams Special Working Conditions The post-holder is required to travel independently between sites, patient homes and to attend meetings etc hosted by other agencies. The post-holder will have contact with bodily fluids, i.e., wound exudates, urine, blood etc whilst in clinical practice and must take precautions as required to minimise the risk of infection/contamination using PPE. Rehabilitation of Offenders Act 1994 Due to the nature of this work, this post is exempt from the provisions of Section 4 (2) of the Rehabilitation of Offenders Act 1994 (Exceptions) Order 1995. This role involves providing direct support and advocating for individuals, children and families, on a citywide caseload with the aim of reducing health inequalities. Our clients are often victims of domestic abuse, trafficking and/or seeking asylum. As well as being homeless/transient they often have complex issues around chaotic lifestyles, substance misuse, debt, mobility, mental health issues and child protection requiring a post holder with a high level of understanding and ability to work in a trauma informed way

Assess, diagnose and plan treatment and care for patients. Clinically examine and assess patients needs. Diagnose and manage both acute and chronic conditions. Prescribe and review medication in line with practice and national guidelines. Prioritise health problems and intervene appropriately to assist the patient in complex and urgent situations.

Work with colleagues in the frailty team to identify patients who would benefit from proactive review. Work with colleagues to undertake patient reviews in a holistic way using the what matters to me model. Work with patients, carers, and families to provide specialist, personalised Occupation focussed interventions. Plan and prioritise a caseload of patients with complex needs, using highly specialist occupational therapy skill. Work alongside GPs and nursing home staff to ensure patients safety, wellbeing and independence are maintained. Work alongside a multidisciplinary team to ensure comprehensive and cohesive patient care. Provide Specialist Occupational Therapy advice and guidance to GP practice staff and the wider MDT in supporting patient care. Liaise with community partners such as district nursing, palliative care teams, voluntary sector organisations, the integrated care therapy team to improve patient and carer outcomes.

Deliver high-quality nursing care to frail and elderly patients, focusing on long-term condition reviews, wound care, and frailty management. Work with colleagues in the frailty team to identify patients who would benefit from proactive review, What matters to me model. Provide wound care support, especially when district nursing services are not accessible. Conduct proactive reviews for patients discharged from the hospital, ensuring safe and effective follow-up care. Assist with annual vaccine campaigns for frail patients, flu & covid vaccinations. Build strong working relationships with GPs, paramedics, and other members of the frailty team to ensure integrated care. Work alongside a multidisciplinary team to ensure comprehensive and cohesive patient care. Liaise with community partners, such as district nursing, palliative care teams, the voluntary sector, and the Integrated Care Therapy team, to improve patient and carer outcomes.
