Job Summary:

The Bromley Occupational Therapy Team is looking for a Senior Occupational Therapist to join our dedicated team over the winter period. This role involves working within a hybrid model to provide essential Occupational Therapy assessments, interventions, and support to residents of Bromley, with a focus on maximizing client independence and facilitating safe hospital discharges. The successful candidate will conduct assessments in client homes, deliver equipment recommendations, and provide tailored advice to improve mobility, safety, and independence for Bromley residents.

Key Responsibilities:

• Occupational Therapy Assessments: Conduct home assessments to evaluate clients' needs, recommend equipment, and plan for minor adaptations that maximize independence and safety.
• Moving and Handling Assessments: Evaluate clients' handling needs, identify suitable equipment, and work towards reducing care requirements where possible.
• Discharge to Assess (D2A) Support: Collaborate with the D2A OT to streamline hospital discharges, assessing needs, and ensuring smooth transitions from hospital to home.
• Adaptation Planning: Assess and plan for major adaptations, liaising with contractors, suppliers, and relevant stakeholders to meet client requirements.
• Triage and Suitability Assessment: Support the team in triaging referrals and identifying appropriate services, including OT, reablement, and other suitable pathways for client support.

Essential Requirements:

• Professional Registration: Registered with the Health and Care Professions Council (HCPC) as an Occupational Therapist.
• Experience: Substantial post-qualification experience as an Occupational Therapist, ideally with experience in community-based assessments, reablement, and hospital discharge support.
• Technical Skills: Proficiency in conducting comprehensive moving and handling assessments and recommending equipment for independence enhancement.
• Communication Skills: Strong verbal and written communication skills, with the ability to produce detailed assessment reports and coordinate with multidisciplinary teams.
• Flexibility and Hybrid Working: Ability to work in a hybrid environment, completing community visits as required and adhering to the office-based schedule.

Preferred Attributes:

• Experience within a local authority OT team or similar setting.
• Strong knowledge of the needs related to discharge planning and complex home adaptations.
• Commitment to continuous learning and development within the field of Occupational Therapy.

About the Role

This role is within the Referral and Assessment Service. The Referral and Assessment Teams receive all new referrals from the Bromley MASH and undertake Child and Family assessments to determine how best children's needs can be met. There are 6 teams, each consisting of a team manager and 5 social workers. The teams go onto duty for one week every 6 weeks and during their duty week, social workers will receive new allocations. Referral and Assessment Team Managers oversee/manage approximately 50 families at any given time.

Bromley has made a huge investment in terms of staffing in order to ensure that social workers have lower caseloads. Referral and Assessment Social Workers will carry an average of 8 families. Social workers in the service are positive, motivated and feel that their current caseload allows them complete good social work interventions with families.

Record keeping is a vital and necessary part of the job and being able to communicate effectively is central to the role.

The successful candidate will need to be flexible, committed, and hard working with a willingness to work to quality practice standards, keeping the person at the heart of all interventions. They must have experience of working within a multidisciplinary framework and need to have good negotiating skills. The successful candidate will be working with Partner Agencies and will be required to maintain good working relationships. They will be required to work as a duty worker on an occasional basis, undertake statutory Safeguarding work (s42 Care Act 2014), Mental Capacity Act (2007) and best Interest assessments and may also be required to provide supervision/support to less experienced staff. Some experience of supervision is desirable but not essential.

The following are the core responsibilities of the Care Navigator/ Administrator. There may be, on occasion, a requirement to carry out other tasks. Please see attached document for full job description. a. Process and effectively signpost patients to the appropriate healthcare professional depending on the presenting condition b. Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately including by-pass line. c. To be helpful and friendly to patients who make contact with the practice by phone or in person providing the best possible patient service. d. Making appointments for patients, taking requests for home visits and answering queries in a friendly and helpful manner. e. To keep patients informed of any delays or problems when they are in the practice and deal with any concerns that may arise. f. To assist with cancelling and rescheduling appointments as required. g. Supporting the clinical team with requests. h. Ensuring Care Navigator/Administrator tasks are completed. i. Ensure adequate supplies of registration forms are available. If taking in registration forms at reception to ensure patient has completed the form fully. j. Ordering ambulances and interpreters for patients as required. k. Checking patient details are current and correct and then update patient records. l. To fully understand how the telephone system works. m. To work as a team member with your admin and Care Navigators colleagues. n. Understand the patient complaints procedure and adhere to. o. To ensure that the patient waiting areas are clean and tidy. At the end of each session to tidy up rooms and tidy up any magazines and leaflets. p. In conjunction with the Medicines Co-ordinator, ensure that patients are aware of the 72 hour turn-around for prescription requests. q. Be aware of all Care Navigator/Administrator and office protocols. r. Become familiar with Hospital & laboratory courier collections & ensure procedure is adhered to. s. Be aware of non-NHS & private services charges, how to accept payments & issue receipt, and that these are recorded accordingly in the petty cash ledger book. t. To open and close the practice, checking security and setting the alarms as necessary. u. At the end of the day, secure building, checking that all windows are closed & locked, blinds closed, necessary plugs are out of sockets, switch off lights, switch on burglar alarm. Administrative duties To process registering new patients, making up notes and entering their details on the computer system. To process new arrivals and deductions. To support with GP Links. Management of the generic inbox Scanning and coding all clinical documents. Using the electronic filing system to complete filing. Dealing with internal and external post To complete admin tasks and support with Econsults. To undertake photo-copying and other basic office duties as directed by members of the team. To deal with queries for further information from hospitals or other local providers about patients. To assist in preparing and sending out practice correspondence. Ensure that the system is operational at the beginning of each day & switched on to the answer phone with the correct tape at lunchtime & at the end of each day. (Downham Site only)
