ELCH PICU is the lead centre PICU for South Thames and home base for the integrated paediatric emergency transport service-South Thames Retrieval Service (STRS). PICU is a mixed intake unit, admitting cardiac and general intensive care. Over 1200 children are admitted per annum through 30 physical beds. 10 beds are predominantly for level 2 care. Approximately 35% of admissions are cardiac or cardiac surgical admissions. 50% admissions are as emergencies through STRS. STRS takes over 1750 calls per annum and transfers about 950 children. The paediatric critical care airway service and Evelina ECMO service (respiratory and cardiac) are also based within the PICU.
The unit is accredited to undertake both Year 1 and Year 2 PICM training.

Main place of employment:

The post will be based at the Evelina Children’s Hospital. However, it is a requirement of your employment that you be prepared to work at any additional or different location owned or served by the trust, either on an on-going or temporary basis according to the demands of the service.

• To manage PICU patients under the supervision of the Chief PICU Fellow and Consultant on Duty.
• To take part in a full-shift rota which will include being resident when on the night shift.
• To undertake teaching and guideline development within the fixed rota.

What skills and experience we're looking for

The successful post holder will support and assist teachers and other senior staff as part of a professional team to contribute to raising standards of pupils’ achievement in the vocational area of catering. You will undertake a range of teaching and learning activities under the professional general guidance of a qualified teacher or senior staff member, in line with the school’s policies and procedures. An exciting opportunity has arisen for a dynamic senior nurse to join our night time community palliative care service, the Pal@home Team.

Pal@home is an innovative rapid-response community based night service offering nursing support to patients with palliative care needs in their own home within the Boroughs of Lambeth and Southwark.

The team will most commonly support patients overnight with uncontrolled symptoms, personal care, telephone advice, patient/carer distress, verifying expected deaths and performing care after death. Pal@home support reduces the need for unnecessary hospital admissions or emergency department attendances, and can play a vital role in expediting rapid discharges home from hospital to a patient’s preferred place of care.

Pal@home operates between the hours of 20:00 and 08:00 - 7 days per week work - in collaboration with Specialist Palliative care teams, the Community End of Life Team, Lambeth and Southwark District Nursing Services, SELDOC and LAS and the @Home team.

Job Description

1. Job Summary

- To provide assessment, care and implement palliative treatment plans for palliative care at the end of life or in the deteriorating or unstable phase of their illness to support rapid discharge from hospital and prevent
unnecessary admissions where possible.

- To mentor and support assigned nurses and nursing assistants to deliver high quality, safe and effective care on a day-today basis.

- To participate in rotation of coordination of shifts.

- To work collaboratively with multi-disciplinary Specialist Palliative Care and Community End of Life teams and @Home service to meet the needs of patients referred to the service.

3. Duties and Responsibilities
- Support and manage a group of patients in collaboration with the Senior Nurse Practitioner whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care overnight.

- To effectively hand over to the relevant teams working in the daytime to allow seamless provision of care

- Demonstrate appropriate practice and specialist advice in the assessment, planning, implementation and evaluation of patient care

- Be responsible for the correct administration of prescribed medication including, management of syringe pumps, enteral nutrition, intravenous therapy etc.

- Be responsible for assessing and recognising palliative care emergencies in the specialty.

- To interpret information and take appropriate action and to lead others to do the same.

- Act as an advocate for patients, to ensure a patient centred approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines

- Ensure that patients receive high quality clinical care and a good patient experience, having regardfor their, customs, culture, social circumstances religious beliefs and doctrines.

- To undertake holistic assessment and provide interventions such as pain and symptom control, management of subcutaneous syringe pumps, re-hydration, emergency unblocking catheters/catheterisation, supporting management of urgent nursing related issues, intravenous therapy etc

- Recognise and avoid situations that may be detrimental to the health and wellbeing of individuals and escalate where necessary

- Provide support and guidance to staff in the monitoring planning, delivery and evaluation of individualised care.

- Is wholly accountable for his / her practice in line with the NMC code of professional conduct and takes every reasonable opportunity to sustain and improve his / her knowledge and professional
competence.

- To be responsible, for maintaining a safe environment for patients and other staff and identifying and escalating any concerns about safety when working in the patient’s home.

- Support and enable patients and carers to make informed decisions related to their treatment and management.

- Ensure that staff take precautions to minimise the risk of cross infection in relation to the safe disposal of equipment and body fluids (including blood).
