Starting Salary: £29, 269 progressing to £32,076 (pro rata for part-time)

Through some of our fantastic development opportunities our staff have been able to complete sponsored degree courses (dependent on need and entry requirement criteria) and have gone on to become social workers or occupational therapists.
You will work as part of a busy social care team at Barnet and Chase Farm Hospital, we are part of a busy integrated hospital discharge team at Barnet Hospital, completing social care assessments to ensure that people are discharged in a safe and timely manner. You will develop close working relationships with the hospital’s Discharge Team and care providers to ensure effective care planning. You will adhere to Hertfordshire’s Connected Lives Practice Principles by ensuring that people are supported to maintain their independence and receive person-centred care and support.
This is a fast-paced environment; discharging people safely but timely from hospital therefore, you’ll need to be able to work under pressure and be extremely well organised, taking a pro-active approach to your workload.
There are many complex and vulnerable patients within the hospital who require our support to get home as quickly as possible. Achieving this for each patient requires talented staff with a blend of skills. Each case must be managed carefully using strong professional judgement and in a proactive and tenacious way to take responsibility for efficiently enabling patient’s discharge.
Social care is vital to the successful and safe discharge of people back to their own home. Supporting people with the right care and information at the right time can help prevent future need, enable independence, and promote positive wellbeing that will empower people to live the lives they want to, you will make a real difference to our residents. Driving Roles Only: As part of this role, you will be required to travel to other HCC Sites/Residences either using your own mode of transport or a company car.
