You will be managing the day-to-day running of the service(s), developing and implementing person centred support plans, conducting care needs assessments, recruiting and inducting new colleagues, completing supervisions/appraisals, completing medication competencies and medication audits, attending mutli-disciplinary meetings, completing quarterly audits and reports, effectively manage the services P&L, ensure regulatory, legislative, internal and contractual requirements are met including relevant notifications and adherence to policies and procedures. An exciting opportunity has arose to join our Pharmacy, Optometry and Dental team (POD) as a Business Support Officer. This role is a key part of the business support functions to primarily support with business needs, therefore we are seeking an organised and efficient administrator to join the team.

The core responsibilities include:
• Provide a high level, professional and confidential PA service, covering all aspects of secretarial, administrative and organisational support

We are looking for a Support Worker to join the Hammersmith & Fulham Outreach and Housing First team.

The Thames Reach Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.

Thames Reach welcomes applications from people with transferable skills.

You will:

.Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough
.Work alongside more senior colleagues to identify the fastest and most appropriate route away from the streets for the people that we are working with
.Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation
.Manage a small caseload of housing first clients.
