Clinical Functions: To provide mental health/psychosocial nursing care throughout the 24 hour day through the operation of an internal rotation shift system. This is to include potentially working every other weekend and two weeks of nights in every eight. To undertake physical intervention techniques, that requires occasional requirement to exert moderate physical effort for several long periods. Utilise driving skills when transporting young people. Regularly inputting information into patient notes and other records, linked to the young person you will be assigned to work alongside for the day. To eat meals with young people at set times, and support the needs of a young person within the context of the Plym Bridge House eating disorder care pathway. Mental Effort The workload (clinical and operational) will be highly unpredictable and interruptions often require immediate action from the post holder. Supporting colleagues and families in managing difficult behaviour, verbal and physical aggression, and levels of suicidal risk in young people. Emotional Effort Frequent exposure to distressing or emotional circumstances examples include dealing with patient issues relating to child protection or risk to life. Participation in team processes and with staff issues, e.g. safety, risk management, service continuity. There is a requirement for long periods of intense concentration whilst delivering clinical practice. Working Conditions Facilitate the reasonable needs of the young person, within a managed risk environment. Regular exposure to unpleasant working conditions when dealing with aggrieved staff and patients. Exposure to incidents of self harm and the ability to provided thoughtful support and care of young people. Be able to work a full 24hr pattern of work, hours will include early shifts 07:30-15:30, late shifts 12:30-20:30, twilight duties 15:00-23:00 and waking night shifts 20:00-08:00. The needs of a 24 hour 7 day a week service will therefore incorporate working regular weekends. All duties will include a mandatory 30 minute break or longer if required dependant on the lengthy of the shift.

PHYSICAL DEMANDS OF THE JOB This section should describe the nature, level, frequency and duration of the physical effort (sustained effort at a similar level or sudden explosive effort) required for the job. It takes account of any circumstances that may affect the degree of effort required, such as working in an awkward position or confined space. The physical skills required in the job should also be described e.g. requirements for speed and accuracy, keyboard, driving skills etc. There is likely to be frequent sitting and standing. Frequent effort to lift and manual handing of equipment. Keyboard work will be used throughout the working day The post holder will be required to maintain competencies relevant to the role. The post holder will use VDU for high proportion of time. However, all staff will be entitled to a workstation assessment and VDU check. The post holder will be required to work across a range of different community and health care settings. The service requires cover between 0800-1800 therefore some flexibility of working hours will be required to ensure the service is always covered. They will support line managers by attending mandatory and statutory training, completing incident/accident forms for every adverse event or near miss that occurs, report all defects and complaints, and communicate any dangerous situation to individuals potentially at risk. 7.2 Health and Safety at Work You must co-operate with those in authority and others in meeting the statutory requirements and in following policies and procedures. For clinical posts at band 7 and above, the post holder will be required to actively participate in complex audits using research methodology, or participate as required in clinical trials or equipment testing, and will demonstrate high level involvement in local ongoing research projects. 7.6 Sustainability and climate change All staff are expected to take responsibility for the reduction of carbon emissions within their area of the organisation. To deliver a high standard of individualised care, which will include the assessment, planning, implementation, review and evaluation of the Personalised Care Framework. This should be in partnership with the service user, carers and multi-agency professionals as appropriate. The post holder will assist registered staff within the Complex Needs team at harbour Drug and alcohol services, working under supervision, in providing support in the community for people experincing mental health and co-existing substance misues issues. The post holder will work alongside the other services and staff working with people that live complex lives in the community, where they will manage a caseload under supervision from a registered member of staff. The aim is to promote recovery from addiction and improve the health and wellbeing outcomes for individuals, linking them in with Community activities and wider supports which include improving access to activities, meaningful employment, and training. The role will require the post holder to work across the complex lives system, developing in consultation with service users flexible and realistic support packages and person-centred plans within agreed guidelines and service models. This post is seen as crucial in working alongside Registered staff to develop and implement person centred care plans in order that the person using the service can optimise their level of functioning despite any barrier their complex problems may present. JOB PURPOSE The postholder will work within the Plymouth Community Dental Service PCDS to develop, implement and evaluate defined Oral Health Improvement activities. Working closely with clinicians to ensure effective and efficient delivery of oral health programmes in line with national oral health strategies such as Delivering Better Oral Health, an evidence-based toolkit. This role will undertake oral health appointments with patients of the service, including adults/children and those with additional needs or who are medically compromised and to undertake fluoride varnish application under the prescription of a clinician. SCOPE AND RANGE This post will provide a comprehensive oral health service for patients with complex needs, as well as evidence-based oral health promotion to key population groups, in line with national and local strategies for reducing inequalities in oral health. The provision of Oral Health Promotion is multifaceted, requiring a range of inter-personal, presentational, and organisational skills, as well as an understanding of the impact of social and environmental determinants of health. The ability to work independently and flexibly is an essential part of this post. Ability to be able to visit different locations within the city to support with oral health promotion for the dental service. COMMUNICATIONS AND RELATIONSHIPS This postholder will work with patients of the service, other dental colleagues to inform of up-to-date oral health promotion toolkits. Oral health promotion will also be undertaken with wider Livewell colleagues (such as in-patient settings), local residential homes and local schools. PHYSICAL DEMANDS OF THE JOB This post will undertake oral health activity in a dental surgery setting as well as talks in local schools and residential homes. Driving between different locations will be necessary. To support and care for patients duringprocedures using conscious sedation. Follows all sedation related local policies and SOPs, including the correct preparation ofequipment and attends local drawing up drugs training with sedation lead. Records clinical observations on patients sedation record Carries out performance checks and daily maintenance on specialist equipment i.e. PulseOximeter, Oxygen cylinders, Scavenger unit and Relative analgesia machine to sure the equipment is ready for use and if faulty to remove from use and report to the relevant technician. Works as part of a multi-disciplinary team in Freedom day case units assisting operator treating patients under general anaesthesia. Providing post operative care plans for these patients Triage of urgent care patients to assess the urgency of treatment and determine thecorrect booking and then book appointments including outlying clinics. Registered, as a Dental Nurse, with the GDC. Thorough knowledge of dental procedures gained through a minimum of 2 years post-qualification training and experience. Thorough knowledge of routine dental procedures and equipment gained through in-house training and experience including specialised areas such as general anaesthetics. Experienced in treating patients from diverse cultural backgrounds i.e. those who require interpretation services. Knowledge of Basic Life Support techniques and certificate of training and willingness to complete ILS training/certification and regular updates, provided by Livewell Southwest Trained in recognising medical emergencies and assists in dealing with them. Trained by PCDS / Livewell in all Mandatory Training topics, including Manual Handling, Breakaway, Health and Safety when required. Clinical work is carried out without direct supervision using initiative and knowledge of procedures gained through experience and training. Administrative duties are carried out without direct supervision using knowledge of procedures and referring to a Manager for non-routine procedures. Analytical and Judgement Skills a) Map reading planning the safest most cost-effective route for community visits. b) The post holder will provide nursing care and other interventions in the setting most appropriate to meet the patients needs. c) Ability to match patient need with available skills and resources. d) Emergency judgments and clinical decision-making. Planning and Organisational Skills a) To take a lead role in the co-ordination of shift ensuring appropriate allocation of work with the available skill mix utilising, prioritising, negotiation and delegation skills. b) Take a clinical lead in absence of HTT managers c) Regularly take a lead in clinical meeting ensuring all patients are reviewed and decisions and plans are put into action and represent the patients views. d) Liaise with other services to arrange appointments that both meet patient and service demands. 1. Physical Skills a) Confidence to travel in an urban environment b) Administer Intra Muscular Injections c) Competent key board skills d) Ability to take accurate physical observations i.e. blood pressure, pulse and temperature. e) Demonstrates safe clinical skills when providing care for patients receiving specialist treatments e.g. commencement of Clozaril in the community or supporting a patient through ECT treatment. Responsibilities a) Take a lead in rapid crisis assessments and the continuous assessment of patients on the caseload, monitoring effectiveness of the treatments provided. b) Formulate care plans, implement and evaluate them along side patients and carers ensuring informed consent is obtained. c) Plan aftercare, making appropriate agreed referrals for longer-term treatment. d) Collaborate with other Teams and Services in the provision of patient care. e) Take a lead role in the identification of risk and the formulation of risk management plans. f) Take a lead role in clinical decision-making, including maintaining the safety of patients, staff and the public in challenging, potentially violent situations. g) Adhere to the Policy for the Safe, Secure and Handling of Medicines and local Home Treatment protocols ensuring patients receive medication in a safe and timely fashion. h) Assess and monitor the effect of medication and report observations to Medical staff. i) Provide education to patients and carers to enhance their knowledge therefore improving longer-term concordance. j) Promote RECOVERY k) Provide education to patients and carers in line with current policy and good practice guidelines. l) The post holder will be required to work over a 24 hour period. To adhere to the Home Treatment Policy and protocols for the implementation of this service. m) Maintain high standards of clinical care to patients. Responsibility for Policy and Service Development a) Expected to take a lead role in the development of the Home Treatment Team including the formulation of protocols. b) Implement all appropriate National, PCH and Local Home Treatment policies and protocols. Responsibility for Human Resources a) Provide clinical supervision and line management supervision for junior members of staff. b) Have the ability to challenge staff where the standard falls below expectations. Have the ability to foster capability and support staff to develop c) Assist in education programmes provided for junior staff. d) Provide mentorship for students both formally and informally. e) Participate regularly in recruitment and selection of staff. f) The post holder will behave in a manner that does not bring the Team or Livewell SW into disrepute. Responsibility for Information Resources a) Responsible for recording CPA and monitoring the quality of data provided by junior staff. b) Responsible for electronic contacts and monitor the standard of work of junior staff. c) Maintain accurate, timely paper and electronic records. Ensure they are kept safely in accordance with Livewell SW policy Responsibility for Research and Development a) Provide a positive role in clinical audit and evaluation of Home Treatment to ensure high levels of clinical effectiveness Freedom to Act a) The post holder will be individually accountable for their practice in accordance with the NMC professional Code of Conduct or Health and Care Professions Council. b) The post holder will actively participate in line management supervision with the Team Manager of the Team. c) Provide line management for junior staff. d) The post holder will actively participate in clinical supervision in accordance with Livewell SW policy. e) The post holder will actively participate in the Livewell SW appraisal system and provide appraisals for junior staff. f) The post holder will carry out other duties commensurate with the position as required. Assess and monitor risk in own and others practice acting on the results thereby ensuring safe delivery of care. Work as an autonomous expert/advanced practitioner. Work within the Nursing + Midwifery Council (NMC) Code, Standards of Practice and Behaviour for Nurses and Midwives Education / Training In conjunction with the multidisciplinary team, ensure that information provided to health professionals and patients is representative of research based recommendations and are within the National Standards Frameworks where appropriate. To develop appropriate information systems and ensure documentation systems are in place to assist in high-level delivery of care. Promote the services provided at the Trust through attendance at conferences, published papers and via patient-facing media.
