We continue to grow as a recognised and respected multi-professional frailty and urgent care response service, consisting of Geriatricians, Advanced Clinical Practitioners, Paramedic, Nurses, OTs, Physios, Associate Practitioner and Therapy Assistants.
Referrals for urgent therapy intervention are received from 999, 111, GPs, and other community services. Patients are responded to within 2 hours of referral to prevent hospital admission or avoid unnecessary use of the local ambulance service.
UCR Therapy focuses on crisis management and admission avoidance to maintain patient safety at home. UCR Therapy also responds to non-injury fallers to assist them off the floor using equipments
The service operates 8am-8pm 7 days a week and you will be rostered shifts within these hours. Flexible working is available and shift patterns could include a variety of earlies (8-4), lates (12-8) or long days (8-8). Part time hours / fixed days will be considered upon request.
A development role from an experienced band 6 to band 7 for the right candidate – please discuss further with the team.
You must have a current, valid driver’s license with a vehicle for work purposes with business insurance. The service will also have the option of driving a small van for work purposes to enable efficiency of service delivery.

PURPOSE OF JOB:
1. To undertake comprehensive specialist holistic assessments for individuals presenting with an acute crisis in their own homes due to deterioration in their health and wellbeing, to reduce or avoid hospital admission.
2. To formulate individual treatment plans for patients with highly complex physical and social needs to ensure immediate patient safety in their own homes.
1. To work autonomously in a crisis situation and manage clinical risk associated with this.
2. To provide a safe and specialist alternative to conveyance to hospital within two hours of referral.
3. To complete onward referrals to specialist teams as appropriate for longer term maintenance and prevention.

KEY TASKS & Responsibilities:
Clinical
1. Complete specialist, complex assessments demonstrating specialist knowledge of the concepts of biological, physical, social, and psychological sciences in relation to function to ensuring patient safety in their own homes.
2. Complete comprehensive holistic assessments to address immediate presenting needs of the patient utilizing extended clinical skills.
3. Complex manual handling including urgent prescription of specialist equipment to manage immediate risks to patients during a crisis situation.
4. Profession-specific assessments, generalist assessments and, where appropriate and within scope of extended practice, cross-profession assessments to maintain patient safety during acute medical / social crisis.
5. Refer to appropriate clinical services to ensure patient safety is maintained; escalation of patient deterioration
6. Use assessment tools/skills that will ensure an appropriate level of recovery intervention so that patients who present with highly complex needs are timely referred to the appropriate clinicians/teams
7. Complete complex risk assessments, both as an independent clinician and drawing on the wider MDT as appropriate
8. Triage and prioritisation of new referrals and allocation of workload to other members of the team
9. To be responsible for the day-to-day management and prioritisation of UCR caseload
10. Respond to urgent referrals and an unpredictable workload: this will require
prolonged concentration in an unpredictable environment
1. Manage potentially stressful, upsetting or emotional situations in an empathetic manner including when imparting bad news: to ensure this is communicated sensitively to all parties concerned
2. To keep yourself well at work both physically and emotionally, to access peer, clinical and management support when needed and to be aware of own limitations and escalate accordingly
3. Be wholly accountable for your clinical practice taking every reasonable opportunity to sustain and improve knowledge and professional competence
4. To manage a joint therapy caseload, seeking support from peers and senior staff where necessary to ensure the provision of a timely, clinically effective service based on evidence based practice
5. To be actively involved in service development projects towards the development and delivery of UCR, therapy and broader community services, and to contribute to new developments and initiatives within the locality
6. Play an active role in the management of clinical governance and patient safety
Management
1. Line Management and supervision of junior staff (registered therapists and/or non-registered staff), including performance management, objective setting and appraisals
2. Participate in the implementation of departmental policies and the annual performance and service development plan
3. Be an active participant in the Trust’s appraisal scheme
4. Undertake investigations of complaints where required, participate in the risk management process, critical incident reporting, evaluation, action planning, dissemination and change in practice.
5. Take responsibility for cost effective management and safe use of expensive and highly complex equipment, provide recommendations for effective use of resources and contribute to the effective delivery of cost improvement planning.
6. Maintain and provide accurate and timely statistics to support service development and provision
7. To delegate appropriately, and to act on delegation

9. To ensure that all equipment defects, accidents and complaints are reported appropriately to ensure appropriate action is taken
Communication
1. To utilise excellent communication skills in the assessment of, and negotiation of treatment for, patients
2. To communicate positively and effectively with other disciplines both internally and externally
3. To give specialist clinical advice to other disciplines within the multidisciplinary team and outside agencies to ensure a comprehensive and person-centred approach to patient care, whilst continuing to develop key working relationships
4. To liaise with partner organisations re: best practice and patient care
5. To provide appropriate and timely clinical follow-ups and onward referrals and signposting to relevant schemes as required ensuring a seamless service is maintained
6. To ensure timely and effective communication regarding all professional related matters
7. To maintain an accurate and timely record keeping system in line with HCPC and RCOT/CSP standards and in accordance with Trust and local policy
8. To attend and be an active participant in and department staff meetings
9. To be able to encourage and educate others through effective communication skills and be able to gain co-operation where appropriate for treatment, where there may be significant barriers to understanding and expression to motivate them towards self-management
10. To use interpreters when required to ensure effective communication occurs with patients who are hard of hearing or do not speak/understand enough English for the purposes of consent and co-operation for treatment
Education
1. To assist in the supervision and development of junior therapists and non-registered staff, including the assessment of competencies for the technicians within Hospital at Home therapy team. To include but not limited to equipment, positioning and mobilising techniques
2. To be responsible for and to actively seek out opportunities, and record your own Continued Professional Development. To be proactive with regard to learning from other staff within the department and Trust
3. To be actively involved in the appropriate clinical interest groups, locally and within the ICS
4. To contribute to the education of patients, families and carers as appropriate
5. To participate in the in-service training programme and be pro-active in learning from senior staff
