Diploma or equivalent in chronic disease management. ENB 998 or equivalent. Job Summary: The nurse will demonstrate a high level of clinical skills developed through experience and training. They will be responsible for providing nursing services to patients, which will include treatment, screening, advice and management of patient groups with chronic diseases within the professional competence of the post holder and in accordance with agreed protocols/patient group directions. They will work as a self-directed practitioner supported by a higher-grade practice nurse and other health care professionals to meet the needs of the practice population. The practitioner will accept the responsibility for providing a quality nursing service to patients in accordance with the requirements of clinical governance. Requirement to cover nursing colleagues for annual leave and sickness absence. Requirement to participate in the annual flu/covid vaccination clinics which generally takes place on Saturdays. Disease Management: Dependent upon current experience and appropriate training, the nurse should manage and run Nurse led clinics or assist in the management of GP led clinics within agreed practice protocols, for example: - - Asthma - Diabetes. - Coronary Heart Disease Management. - Hypertension. - Warfarin Management. To work towards the attainment of appropriate qualifications, in order to maintain a high level of knowledge in disease management. Maintain accurate patient records. In conjunction with practice staff, undertake clinical audit ensuring that relevant patient health data is collected. Established in 1998, Dermatology for Animals provides the veterinary dermatology service within the majority of specialist multi-disciplinary veterinary hospitals across Queensland.

We are now looking for Part-time and Full-time veterinary nurses to join our expanding team of veterinarians, nurses and administrators in south-east Queensland.

We are seeking a self-motivated, enthusiastic, and experienced nurse, who wants to excel in providing first rate nursing care, and who is capable across the broad range of skills that encompass veterinary nursing in a dermatological environment. This not only involves utilising patient nursing skills, but also learning about new techniques, becoming competent in dermatological diagnostics, and communicating with clients both in person, by phone and by email.

Veterinary dermatology is unique amongst referral disciplines in fostering long-term relationships with clients and patients. It’s less about fixing our patients and sending them on their way, and more about managing their issues so that they and their owners can live their best lives. It’s also unique in that dermatology rarely presents with genuine emergencies, and this allows us all to have an enviable work-life balance.

The team provide an urgent response and a comprehensive Mental Health Assessment to people who are presenting in an acute Mental Health Crisis, and Home Treatment to those people requiring a period of short-term intensive intervention. Under the direction of the senior health and social care staff in the team and/or the shift coordinator you will assist in delivering interventions with an allocated group of service users with mental health needs. The interventions will be focussed on assisting the person to identify and achieve their agreed goals of Recovery. These will include interventions related to; meaningful occupation, accessing employment, assessing and promoting physical health, social inclusion, activities of daily living and mental health promotion. You will also be involved in supporting the service user to complete appropriate outcome measure tools and participate in reviews of the service users care and effectiveness of the interventions provided.
