The LPN/LVN is assigned to a Patient Aligned Care Team (PACT). The LPN/LVN is technically proficient in initiating, performing and completing assigned duties in providing care to a variable patient population. He/she observes and interviews patients and significant others to gather data and reports and/or documents as per policy. The LPN/LVN recognizes and reports to RN immediately signs and symptoms of emergencies such as evisceration, hemorrhage, shock, coma, seizure, and airway obstruction. Job duties also include TeleEye, TeleDerm, TeleAudiology, and all other duties as a TCT.

Provides patient care and administer medications. The LPN/LVN is responsible for the storage, proper cleaning techniques, and use of reusable medical equipment (RME). He/she must be able to locate the Standard Operating Procedures (SOPs) for all RME in the clinical area. She/he is responsible for carrying out proper procedures for caring for patients in an isolation environment by following proper infection control procedures.

Duties include but are not limited to:

• Uses judgment in selecting the appropriate order and sequence of procedures.
• Observes, identifies, and responds to the patient's needs for care, including medication, equipment- assisted care and patient /family education. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors.
• Observes for physical and /or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or MD/DO/provider.
• Recognizes and reports to RN immediately signs and symptoms of emergencies such as evisceration, hemorrhage, shock, come, seizure, and airway obstruction.
• Demonstrates knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems.
• Is technically proficient in initiating, performing, and completing assigned duties in providing care to variable patient populations.
• Prepares and administers prescribed medications (oral, topical, rectal, subcutaneous, intramuscular and/or intravenous, administers (except research/investigation) including insertion of medication into the eye, ear, nose, bladder, rectum, and vagina; prepares and performs TB or cocci skin testing; gives allergy and flu injections and documents, performs treatments according to established procedures; accurately assesses the need for PRN medications and prepares, administers, and evaluates responses.
• The LPN/LVN will assist the Physician, PA, NP, and/or RN with clinical procedures which include: pre/intra and post procedural care.
• Demonstrates knowledge and skills necessary for concentrated effort and self-direction in carrying out the most complex practical nursing care assignments, including assignments for selected acute care, long term or ambulatory care, or psychiatric patients requiring skilled nursing care. Initiates emergency procedures for which the employee is trained and reports actual and potential safety hazards within the workplace.
• Follows all safety procedures and wears protective clothing as appropriate.
• Demonstrates effective interpersonal skills with both internal and external customers.
• Possesses knowledge and understanding of human behavior, patient motivations, and reactions to situations and ability to appropriately utilize this knowledge in working effectively with patients, family members, and other staff.
