The Deputy Program Coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area. Their health care and psychosocial problems and needs are complex and require a high degree of clinical oversight and creative problem solving. Services are provided in the BHIP Mental Health Teams. The incumbent is required to interact not only with the patient but also family, significant others, community agencies, and other staff. The BHIP Social Work Coordinator should have advanced clinical knowledge of both substance use and complex mental health diagnosis. The BHIP Social Work Coordinator provides guidance and oversight regarding clinical practice and professional standards for Mental Health Clinicians assigned to the BHIP Teams. Some of the duties include, but are not limited to;

• Provides both administrative and clinical supervision to the staff as assigned.
• Expected to set priorities and make assignments according to capabilities of employees and oversee personnel management functions of staff assigned.
• Evaluates subordinate's performance and offers instruction, advice, or counseling on clinical and administrative matters.
• Responsible for monitoring compliance with social work and mental health practice standards and guidelines on documentation, workload, data entry, ethical practice, and service delivery.
• Effectively handles minor disciplinary measures such as warning and reprimands and recommends action in more serious cases.
• Provides general and position specific orientation to BHIP staff, including position-specific competencies.
• Participates in the application of FPPE/OPPE, Social Work Scope of Practice, monitors continuing education requirements, and other requirements for the staff assigned to supervise.
• Responsible for and provides guidance to BHIP clinicians regarding clinical Social Work practice including promotion of Social Work and Mental Health services as a key component of treatment.
• Interviews Veterans referred for substance use and/or mental health treatment to establish facts about the Veteran's situation, presenting problems and their causes, and the impact of such problems on the Veteran's functioning and health as part of a comprehensive psychosocial assessment.
• Uses clinical training, insight, and experience to interpret data and to identify viable treatment options.
• Independently evaluates the Veteran's situation, including the Veteran's reactions, coping abilities, as well as their ability to arrive at a reasoned conclusions.
• Performs insightful assessment of serious and complicated cases involving psychiatric illness, substance use, catastrophic medical conditions, dementia, and other high-risk diagnoses.
• Provides clinical services to Veterans and family members/significant others in support of the veteran's treatment.
• Other duties as assigned.

We have an exciting opportunity for an individual to join our LFC Foundation team as a Children’s Health Coordinator. In this role you will oversee the day-to-day coordination of the children’s health programmes, and a large element will involve exercise/activity delivery, weight management educational workshops and mentoring.

The LFC Foundation currently delivers a number of children’s health programmes including MOVE, an exercise referral programme that supports children with respiratory conditions and obesity issues, to access physical activity in the community but also in a supported environment.

What will you be doing?

• Work closely with the Health Manager to establish strong and lasting relationships with Primary Care staff and other health professionals
• Provide specialist coaching support for children with CF, asthma and obesity issues
• Deliver weight management sessions to families and support them to make positive lifestyle changes
• Deliver mentoring sessions to children/young people poor mental health
• Deliver activity sessions in the community
• Deliver the projects in line with the project plans ensuring all KPI’s are met
• Ensure quality and safeguarding standards are achieved and maintained
• Work closely with partner organisations
• Support the development of all children’s health programmes
• To encourage and mentor participants to access our community activities and workshops
• Support beneficiaries to identify and access physical activity opportunities in their local area that reflect their needs, interests and abilities

• Provide timely reports and analysis of performance

Who are we looking for?

To be successful in this role you will have recent proven experience working with children and adults in an exercise environment achieving lasting behaviour change, including working with children and young people with poor mental health and experience delivering weight management sessions or similar. It is desirable you will have experience delivering virtual exercise classes/remote consultations and a proven track record of delivering preventative health interventions and signposting individuals to specialist services.

Essential qualifications for the role:

• Level 3 Exercise Referral qualification
• Level 2 weight management qualification
• Mental health first aid qualification
• First Aid at work qualification

You will be able to demonstrate an understanding of the issues around physical activity and behaviour change for children and young people living with long-term health conditions. You’ll be able to showcase excellent personal organisational skills and interpersonal skills, empathy and resilience.

• Ensures that Veterans with positive screening results for depression, Intimate Partner Violence (IPV)/Domestic Violence (DV), Military Sexual Trauma (MST), and Post Traumatic Stress Disorder (PTSD) during their pregnancies or postpartum are referred to the appropriate specialists.
• Evaluates care and outcomes to ensure timely and appropriate provision of services. Advocates for holistic care through Veteran/family education and provider interaction and implements an educational plan to meet the Veteran care needs.

Major duties of this position include but are not limited to:
Functioning as the coordinator of care for Veterans at suicide risk receiving care through the Battle Creek VAMC healthcare delivery system including its Outpatient Clinics. The SPC also serves as an advisor to the Associate Chief of Staff for Mental Health, Mental Health Service Chiefs, facility leadership, program managers and other medical center staff concerning suicide prevention strategies for individual patients, for the facility and for the Veteran community as a whole in conjunction with the SPCs at the Wyoming Health Care Center and Battle Creek VAMC, and other members of the Suicide Prevention Team.

Specific functions of the SPC include, among others, identifying Veterans at risk for suicide, tracking appointments and coordinating enhanced care as needed, educating providers, Veterans, families, and members of the community about risk factors and warning signs for suicide, and identifying treatment options. This is accomplished through:

• Tracking all suicide attempts and patients with serious suicide ideation at each facility through the use of the "Suicide Behavior Report" progress note. The coordinator will implement the use of this template at each facility and be placed as a co-signer or additional signer on the template in order to be made aware of all attempts and patients with serious ideation.
• Ensuring that Suicide Behavior Flags are posted on the charts of relevant Veterans and monitoring the need for continued flagging.
• Assuring that all Veterans on the facility tracking list are being followed closely by the appropriate provider on a regular basis as determined by the coordinator and the provider.
• Tracking any no-show appointments by these patients and following up with the patient as soon as possible to reschedule the appointment.
• Following up with Veterans who have contacted the Veterans Crisis Line.
• Maintaining required databases and reports per the National Suicide Prevention Program.
• Preparing Heads Ups and Issue Briefs as needed.
• Monitoring and maintaining the REACH Vet database and the clinical care required.
• Monthly contact with Veterans deemed at high risk for suicide as defined by national guidelines and with those Veterans who remain actively on the high-risk list.
• Monitoring the completion of the treatment plan, safety plan, and means reduction for those patients who have been identified to potentially pose an increased risk for future suicidal behavior.
• Disseminating, organizing, and, where possible, conducting evidence-based and promising, emerging interventions.
• Maintaining awareness of suicide treatment options and making recommendations to the patient's provider when new options become available.
• Obtaining any relevant HIPAA clearances to allow transmittal of facility data on suicide attempts and individuals at risk to the Suicide Prevention Center of Excellence for trending and quality assurance purposes; and transmitting the data following cyber security requirements as applicable for this sensitive data.
• Assuring that all of these patients have a 24-hour resource number including the VCL to call and ways to get assistance if an immediate crisis should occur.
• Establishing an on-going Gatekeeper Training program with non-clinical frontline staff such as clerks and telephone operators to assure that they are aware of options for patients with whom they come in contact during crisis situations.
• Working with community resources such as VSO's and other community groups to assure that they are aware of how to recognize Veterans at risk for suicide and their options for getting these Veterans assistance.
• Developing and implementing an ongoing education plan for providers, Veterans, families, and members of the community on risk factors and warning signs for suicide, and treatment options.
• SPCs will participate in training activities and scheduled conference calls as required by the Office of Mental Health and Suicide Prevention (OMHSP)
• Performs other related duties as assigned
