Social Worker - West Integrated Cluster Team

This is an exciting time to join our Integrated Cluster Network Team. The cluster teamwork and support to develop a team around the person and their family or those important to them.

The people we support can have long term complex challenges around their physical health, mental health and emotional well-being. It works with both older and younger adults. We aim to provide an holistic support to allow to live their lives as they wish to and ensure voice, choice, independence and control in their lives We, are seeking to appoint creative, enthusiastic, ambitious full time permanent social workers within the Integrated Cluster Networks. The Teams consist of Social Workers, District Nurses, Occupational Therapists; Physiotherapists; Dietetics; Speech and Language Therapy; Pharmacy and Community Psychiatric Nursing. We work closely with G.P. surgeries and the third sector partner agencies to assess and support the needs of adults affected by long term complex and adverse conditions as well as their carers and families.

Using a strength-based outcome focused model of Social Work, the teams support adults, and their carers to achieve positive outcomes within their lives and encourage wider inclusion in local communities. Maximising an individual’s emotional wellbeing is seen as key in effective social work practice.

We are keen to continue to develop innovative approaches to social work practice. We work closely with GPs and other cluster partners and utilise the Contingency Care Model to prevent unnecessary admissions to hospital and care homes, as part of our role to maximise the independence and choice. The continued development of this dynamic service model will require work on widening partnership opportunities with statutory partners; the third sector and with individuals from the community.

We are particularly looking for those who are interested in community work and therapeutic social work practice. We are keen to develop resilient community connections, groups and services in Bridgend. The positions will either be based in our office in Pyle or in Glanrhyd. We will offer focussed and committed development opportunities for the successful candidates.

We are looking for an experienced and committed Senior Social Work Practitioner to join our regional forensic social work team at Caswell Clinic. This role will involve deputising for the Social Work Manager and supporting the wider Social Work Team. You will be required to work to social work standards specific to secure services and manage your performance within the legal framework of this area of mental health practice.

You will work with an established multi agency clinical team across the health boards and local authorities in South Wales. Work will include supporting vulnerable individuals in an inpatient setting and in the community. You will be part of the social work team in Caswell Clinic which allows areas of specific interest to be developed and you will have access to specialist training as well as opportunities to deliver training to a wider cohort.

You will hold a complex caseload of individuals who may present with complex trauma, attachment disorder, severe distress, emotional dysregulation and who may challenge care givers. You will be a resilient and boundaries individual, able to work in a challenging environment. You will need to have effective communication skills, both verbally and in writing representing the needs of the patients with integrity and sensitivity to the needs of the service.

The successful candidate will have a relevant qualification in Social Work and significant post qualification in mental health practice.

We are recruiting support staff to work in Glyn Cynffig. We are looking for enthusiastic, creative, and flexible people to work with individuals who are experiencing mental health problems, mild learning difficulties, addressing substance misuse and others who are vulnerable, enabling them to gain skills necessary for independent living. You will be required to monitor progress on the implementation of individual plans, recording such monitoring as required.

A positive and supportive approach is a requirement of the role as the successful candidate will need to support people in a person-centred way with all areas of identified needs. This will include household and domestic tasks, shopping and food preparation, hygiene, personal health, budgeting, social activities, offering guidance and advice to enable people to make informed decisions and exercise their rights. You will be expected to assist people to register with a doctor and dentist of their choice, supporting them during visits if this is a requirement.

The role will involve recording all financial transactions relevant to Petty Cash, in line with Financial Regulations.

To support you in your role you will receive appropriate training, one to one supervision with your line manager, as well as regular team meetings.

For this post, you will need to be flexible and will participate in shift work. As you are required to work weekends, nights when required and bank holidays you are entitled to the enhanced rate of time and a third for these hours. You will have sleeping in duties, (for which an additional allowance is paid). The hours will be arranged by the manager of the service and subject to the needs of people supported.
