Job Description & Personal Specification Job Title: Enhanced Nurse, Diabetic Care Specialist Date: 17th October 2024 Salary: £40,000 - £42,000 Contract: 37.5 Hours Per Week Monday Friday Location: Working across 5 Practices in Preston Park PCN Reports to: PCN Operations Manager and Clinical Director Job Summary The Diabetic Enhanced Nurse will provide specialist care and support to patients with diabetes as part of Preston Park PCN. The role involves managing complex diabetic cases, providing education, and promoting self-management to improve patient outcomes. Working closely with the wider primary care team, the Diabetic Enhanced Nurse will ensure that high-quality, evidence-based care is delivered, following best practices in diabetes management. Please ensure you meet the essential criteria and are level 7 qualified. Core Hours 37.5 Hours Per Week, part time hours are also available Key Responsibilities 1. Lead in the management of patients with Type 1, Type 2 and complex diabetes, including those requiring insulin therapy or complex regimes. 2. Conduct comprehensive diabetes assessments, including glycaemic control, cardiovascular risk management and complications screening. 3. Develop and implement individualised care plans to optimise blood sugar control, reduce complications and improve quality of life. 4. Educate patients and their families/carers on diabetes management, including diet, exercise, medication adherence, and monitoring. 5. Provide training in blood glucose monitoring, insulin administration, and understanding the role of diet in diabetes control. 6. Support patients in achieving lifestyle changes to manage their condition effectively, focusing on long-term outcomes. 7. Serve as the clinical expert within the practice and the ARRS team, providing training and support to other healthcare professionals. 8. Ensuring the implementation of the latest clinical guidelines and best practices. 9. Provide expert guidance to the wider primary care team on the management of diabetic patients. 10. Participate in clinical meetings and contribute to case discussion on complex diabetic care. 11. Collaborate with GPs, pharmacists, dietitians, and other healthcare professionals to deliver coordinated care for diabetic patients. 12. Liaise with secondary care specialists, such as endocrinologists and diabetes clinics, to ensure seamless transitions of care for patients requiring advanced management. 13. Lead or participate in multidisciplinary case reviews for complex diabetic patients. 14. Contribute to the ongoing development of diabetes services within the practice, identifying areas for improvement and implementing changes to enhance patient outcomes. 15. Participate in audits and data collection to monitor clinical performance and outcomes in diabetes management. 16. Help the practice meet national diabetes targets, such as QOF (Quality and Outcomes Framework) and local commissioning requirements. 17. Maintain accurate, detailed clinical records of patient assessments, treatments, and outcomes in line with practice protocols. 18. Develop comprehensive diabetes care plans that are personalised to each patient, reviewing and adjusting them regularly. 19. Ensure patient care plans align with overall treatment goals, including cardiovascular health, weight management, and lifestyle modification. 20. Provide training and education to other healthcare professionals within the practices on diabetes management, ensuring they are up to date with current best practices. 21. Actively participate in your own professional development, attending relevant training sessions, conferences, and maintaining clinical knowledge through continuous professional development (CPD). 22. Conduct home visits for housebound diabetic patients when necessary to assess their condition and provide care. 23. Utilise remote monitoring tools, including digital health technologies, to support patients in managing their diabetes effectively at home. 24. Contribute to the development and review of diabetes care protocols within the practice to ensure compliance with local and national guidelines. 25. Identify patients at risk of diabetes complications, such as foot disease, cardiovascular disease, or diabetic nephropathy, and take proactive measures to reduce these risks. 26. Participate in clinical governance activities, ensuring that the diabetes service meets regulatory standards and promotes patient safety and care quality. Essential Skills/Knowledge/Experience Registered Nurse with valid NMC registration Postgraduate certification or diploma at Level 7 in Diabetes Care Extensive experience in managing diabetes care, including complex cases such as insulin initiating and adjustment. Strong understanding of diabetes pathophysiology, treatment options, and lifestyle interventions for managing the condition. Excellent communication skills to educate and support patients, families, and other healthcare professionals. Ability to work independently and as part of a multidisciplinary team in both primary and secondary care settings. Experience in primary care or community settings. Desirable Skills/Knowledge/Experience An understanding of general practice clinical systems Participation in wound care research or quality improvement initiatives

Your new role

Working under the remit of the Head of Data Engineering as part of the Digital Directorate, the Data Engineers will be key members of the Data Engineering team working primarily on day to day ETL operations, as well as working with the wider Performance Team and Digital Team to meet the data management needs of the Trust.

As a Data Engineer you will be fundamental in developing and constructing data models for our services data warehousing needs as well as integrating them into systems and business processes to support self-serve reporting, compliance reporting and third-party data integrations.
You will provide specialist advice across a range of IM&T areas e.g. applications, information interpretation, data analysis, information reporting, information governance.

What you'll need to succeed

As an experienced Data professional with proven skills of managing and maintaining a data warehouse within a large organisation, you will have expertise in writing ETL scripts and code and ensuring that the ETL process performs optimally. The role requirement will involve implementing data flows to connect operational systems, data for analytics and business intelligence (BI) systems.

A key responsibility will be to re-engineer manual data flows to enable scaling and repeatable use in an automated way and to support the build of data streaming systems. You should have a thorough understanding of the concepts and principles of data modelling and metadata management, and you will need to be able to design, code and execute test scripts to support ETL processes defined within our platforms.

You should possess strong in-depth knowledge of Database Administration with MSSQL Server and expert knowledge of Microsoft Azure / AWS Platforms (Specifically Cloud Storage, Compute and Databases).
