The Trust (GEH) is procuring a new Electronic Patient Record (EPR) which will enable seamless, integrated care across the Trust and improve the safety, efficiency and quality of care. This will be a single shared instance of EPR across the three trusts of George Eliot Hospital (GEH), South Warwickshire University Foundation Trust (SWFT) and University Hospital Coventry and Warwickshire, with GEH and SWFT following on with a planned Go Live in 2026 after a UHCW Go Live in June 2024. This post of an EPR Clinical Chief Information Officer (EPR CCIO) will provide the clinical leadership for the GEH EPR programme, in close collaboration with the CCIO at SWFT and within the above overarching single instance EPR governance across three trusts. They will lead and promote clinical engagement, support user centred design, support clinical safety activities and raise the profile of the programme to ultimately ensure that the new EPR is safe, it underpins quality, improves the clinician and patient experience and drives efficient delivery of care. CWPT NHS Talking Therapies is a progressive service which encourages professional development, supervision and peer support with an identifiable career pathway for PWP’s to develop into more specialised or senior roles. We work closely with members of the local community, service Commissioners, GP’s, secondary care services and various voluntary sector agencies.

Coventry and Warwickshire Partnership Trust provides a full range of expanding physical, mental health and learning disability services to children, young adults, adults and older adults over many sites across Coventry, Warwickshire and Solihull.

There are new exciting changes being made to access and referrals into Secondary Care mental health services.

Are you an experienced mental health clinician with a desire to provide the highest quality care to our service users? Do you want to challenge yourself in a fast paced supportive environment where you can continue to grow and develop your skills? If so then we are currently looking for motivated and enthusiastic Band 6 Nurses/Social Workers/ to join our team based in Warwick.

Our Crisis resolution and Home Treatment (CRHT) form part of the community provision for secondary mental health care in Coventry and Warwickshire. We welcome applications from reliable individuals, who are passionate and committed to make a positive difference to our service users.

As the front door to secondary care mental health, the successful candidate will identify any needs using a holistic triaging tool and will refer on to the most appropriate service to meet the needs of the service user. This may include conducting and urgent assessment where urgent needs are identified.

The Home Treatment Team provide care and treatment to services users using the least restrictive principles to ensure recovery is achieved at home in order to avoid hospital admissions. This includes intensive home treatment, gatekeeping admissions to hospital and also facilitating early discharge from hospital inpatient units.

You will be responsible for the assessment, risk assessment/s and formulation of crisis care plan/s and discharge preparation for our service users.

The team/s operate a 24 hour Rota system and you will be expected to work various shift patterns.
