Develop and deliver evidenced based psychosocial interventions where appropriate. This is to include the Change Model for individuals diagnosed with Complex Psychological needs (Personality Disorders) and undertake training where appropriate. Undertake holistic social work assessments using strengths based model collaboratively with services users, relatives and carers in order to promote the appropriate support towards recovery. Undertake a range of procedures and practices underpinned by theoretical knowledge or relevant practice to provide a service that is tailored to meet the needs of the individual. Ensure effective communication with particular regard for individuals who may have difficulties in relating to the complexity of emotional and psychological conditions. This will include skills in managing moods, motivation, counselling, training,empathising and reassurance. Manage an appropriate caseload requiring social work interventions. Ensure appropriate risk assessments and risk management plans are completed and reviewed. Ensure all child protection, adult safeguarding, Mental Capacity Act issues are dealt with according to policy and procedure. Undertake the function of Care-Coordinator in line with Trust policy for the Care Programme Approach where appropriate.
