You will manage a team of support staff working Monday-Friday 9-5 across three schemes in Swindon.

All three schemes are commissioned to provide low-medium housing-related support. Swindon Young Parents has two sites providing services for young parents aged 16-25 and the Booker House provides services for customers aged 18+ with learning disabilities.

Responsibilities To act as Care Co-ordinator To carry a caseload and work with evidence based interventions to include: - Assessment of need of the service user and carer Formulation of care plan Implementation of a plan of care for the service user in collaboration with the multi-disciplinary team, referring agent, family/carer and service user, where appropriate. Group HSE Manager - Responsibilities

• Manage all health and safety related issues across 4 manufacturing facilities

To provide consultant psychiatric input to a high functioning community EIS as part of a multi-disciplinary service. The role is to provide a comprehensive assessment and NICE concordant treatment package of care to all patients.

To provide high-quality Mental Health Care for adults following the EIS frameworks

To work closely with other teams and Consultants to ensure coherent and high-quality mental health care for adults with severe and enduring mental health problems within the Heywood,Middleton and Rochdale (Rochdale borough) area

To work with the MDT to support the development of the service, and to attend local and divisional service development meetings

This is an exciting time to join the Trust and there are many opportunities to contribute to the organisations’ development and to look at service redesign models. Rochdale has a very strong teaching commitment and candidates will be expected to contribute to the teaching of medical and nursing staff. There are close links to ongoing research projects. The post holder will be based at the Outpatients department Birch Hill Hospital Rochdale. Rochdale operates an Inpatient- Outpatient model (New Ways of Working) and is staffed by 9 full-time Consultants in Adult Psychiatry and old age psychiatry and a nurse consultant on the female adult ward. Each Consultant Psychiatrist is allocated a Junior Doctor (NW Deanery training schemes) to work with them .

Applicants must be on the GMC’s Specialist Register or eligible for inclusion or be within six months’ of the anticipated award of a Certificate of Completion of Training (CCT) in General Adult Psychiatry at the time of interview for the post.

Low frequency on-call (1 in 31) with full second tier on call and 1 day off after your on call.

The management of acute, long term and substance misuse presentations represent a large cohort of these patients and requires understanding of primary care. HMP Buckley Hall is a Category C male prison with an excellent reputation. The service is currently transforming to a Nurse Led unit with a vision to become a dynamic, forward thinking place of work, staff are supported to develop and valued for their contribution. The nursing team recognised for their practice and collaborative partnerships and rightly proud progressing the Nurse Led Model approach. A Primary Care Manager is needed to engage with this award-winning team continuing its healthcare transformation. The successful candidate will provide clinical management and expertise to a small, diverse primary care team forming part of the senior team. We are looking for someone with excellent skills who can clinically prioritise and manage patient journeys. Experience of offender health desirable however would welcome individuals who are multiskilled across complex care pathways. The role requires excellent communication skills and dynamic work practices. We are looking for someone to deliver equivalent care to that of the community. The successful candidate to be responsible for implementing policy and procedure as well as support the nursing team to ensure care of the highest standard.



Support the Head of Healthcare in operationally leading the provision of high-quality clinical health care services ensuring a consistent and co-ordinated approach to the delivery of quality evidence-based practice. Responsible for monitoring daily performance of the nursing team and service delivery against key performance indicators including working towards the HJIPs (Performance indicators) and effectively driving improvements to patient care and experience. Work with subcontractors, partners and team leaders to ensure the provision of highquality evidence based patient care and the efficient use of resources, maintaining visibility and accessibility to patients and staff. Support the Head of Healthcare by taking a strong level of responsibility in maintaining the Care Quality Commission standards and audits against the KLOE. To represent the Head of Healthcare in local meetings with the Prison and other interested parties e.g., Coroner’s court. The post holder will be expected to be a role model to lead others in line with our shared commitment to our values; Working Together, Integrity, Respect and Recognition, Compassion and Innovation.
