Work with people, their families and carers, to improve their understanding of their condition. Support people to develop and review personalised care and support plans to manage their needs and achieve better healthcare outcomes. Help people to manage their needs by providing a contact to answer queries, make and manage appointments, and ensure that people have good quality written or verbal information to help them make choices about their care. Assist people to access self-management education courses, peer support, health coaching and other interventions that support them in their health and wellbeing, and increase their levels of knowledge, skills and confidence in managing their health. Provide coordination and navigation for people and their carers across health and care services. Helping to ensure patients receive a joined-up service and the appropriate support from the right person at the right time. Work collaboratively with GPs and other primary care professionals within the PCN to proactively identify and manage a caseload, which may include patients with long-term health conditions, and where appropriate, refer back to other health professionals within the PCN. Support the coordination and delivery of multidisciplinary teams with the PCN. Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and people to be more prepared to have shared decision-making conversations. Take referrals or proactively identify people who could benefit from support through care coordination. Have positive, empathetic and responsive conversations with people and their families and carer(s), about their needs. Increasing patients understanding of how to manage and improve health and wellbeing by offering advice and guidance. Develop an in-depth knowledge of the local health and care infrastructure and know how and when to enable people to access support and services that are right for them. Use tools to measure peoples levels of knowledge, skills and confidence in managing their health and tailor support to them accordingly. Support people to develop and implement personalised care and support plans. Review and update personalised care and support plans at regular intervals. Ensure personalised care and support plans are communicated to the GP and any other professionals involved in the persons care and uploaded to the relevant online care records, with activity recorded using the relevant SNOMED codes. Make and manage appointments for patients, related to primary care. Help people transition seamlessly between secondary and community care services, conducting follow-up appointments, and supporting people to navigate through the wider health and care system. Refer onwards to social prescribing link workers and health and wellbeing coaches where required and to clinical colleagues where there is an unaddressed clinical need. Regularly liaise with the range of multidisciplinary professionals and colleagues involved in the persons care, facilitating a coordinated approach and ensuring everyone is kept up to date so that any issues or concerns can be appropriately addressed and supported. Actively participate in multidisciplinary team meetings in the PCN. Identify when action or additional support is needed, alerting a named contact in addition to relevant professionals, and highlighting any safety concerns. Record what interventions are used to support people, and how people are developing on their health and care journey. Work with your supervising GP and/or line manager (if different) to undertake continual personal and professional development, taking an active part in reviewing yearly progress, and developing the roles and responsibilities and developing clear plans to achieve results within priorities set by others. Work with your supervising GP to access regular clinical supervision, to enable you to deal effectively with the difficult issues that people present. Involved in one-to-one meetings with line manager regularly to discuss targets and outcomes achieved. Establish strong working relationships with GPs and practice teams and work collaboratively with other care coordinators, social prescribing link workers and health and wellbeing coaches, supporting each other, respecting each others views and meeting regularly as a team. Act as a champion for personalised care and shared decision making within the PCN. Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner. Identify opportunities and gaps in the service and provide feedback to continually improve the service and contribute to business planning. Contribute to the development of policies and plans relating to equality, diversity and reduction of health inequalities. Adhere to organisational, practices and PCN policies and procedures, including confidentiality, safeguarding, lone working, information governance, equality, diversity and inclusion training and health and safety. About the role:

This is an exciting opportunity for an enthusiastic and dynamic Care Coordinator to join the Croydon Early Intervention Service (COAST). The post is open to nurses, OT's and social workers.

The post-holder will conduct assessments of newly referred adults to the Early Intervention Service who are experiencing First Episode Psychosis (FEP). They will undertake the responsibilities of a care coordinator within the Care Programme Approach for a defined caseload of service users and their carers. They will provide evidence-based psychosocial interventions and will focus on engagement, recovery and social inclusion as well as addressing physical health care needs.

The successful candidates will be experienced registered professionals who are able to demonstrate a specific interest in working with people with developing psychosis.

This is an exciting time to be working in this field which is undergoing a period of rapid growth and development. We are able to offer excellent support and training opportunities.

1.Assessment: To be responsible for the assessment of newly referred adults with First Episode Psychosis and the subsequent presentation of findings to the wider team. To work with diagnostic uncertainty, providing ongoing comprehensive assessments and outcome monitoring for all service users within caseload. To undertake risk assessments regularly, involving all other health professionals/agencies involved. To ensure that risk assessments are regularly updated, are meaningful and are reflected in care plans.

2)Engagement: Assertively outreach difficult to engage clients. To meet service users regularly, as far as possible at a venue of their choice, be this at home, CMHT centre, education, work or recreational establishments to assist with engagement and encourage participation in treatment
delivery as well as facilitating independence in all aspects of daily living.

3)Support:To provide ongoing care coordination of a designated caseload of service users who will be experiencing their first episode of psychosis. To provide support and advice in relation to service users regarding the issues concerning them. To work collaboratively with service users focussing on a recovery approach. Provide information to help clients make informed choices.

COAST is a multi-disciplinary team providing a comprehensive service to people who may be experiencing psychosis for the first time, living in Croydon. The successful applicants will be enthusiastic about this client group, and committed to working within the recovery approach to deliver NICE concordant care.

4)Interventions:To provide interventions relevant to a first episode client group. To ensure all interventions are age appropriate and that care is sensitive to the needs of black and minority ethnic service users. To provide low intensity CBT and Mindfulness interventions under supervision. To provide psycho-education to services users. To have a client centred approach with a focus on resocialisation and social inclusion.

5)Families and Carers:To involve families and carers in treatment plans where appropriate. This may require direct family interventions. Ensuring services are provided within guidance of the Children and Carers Acts. To ensure that the needs of Carers are assessed and the care services are
provided as necessary. To provide families and carers with psycho-education.

6)MDT Working:To work in co-operation with members of the multi-disciplinary team and all other relevant health professionals to assess, develop, implement and evaluate high quality packages of client centred care. To form effective relationships with inpatient services to facilitate early effective discharge. To take part in all clinical meetings.

7)Communication:To maintain accurate clinical documentation on designated caseload ensuring case notes are up to date as per guidance and in line with electronic patient journey system. To ensure effective verbal and written communication with all involved with delivery of care. The post holder will liaise with other disciplines and organisations to ensure that all issues affecting service user care are managed and communicated to a high standard.

8)Personal Development:To remain up to date with current/emerging good practice relating to community mental health services, in general and Early Intervention services in particular. To abide by the policies of SLAM NHS foundation Trust and Local Authority. To keep abreast of all developments pertaining to the provision of Community Mental Health Care. To contribute to effective MDT working. To meet regularly with Team Manager/Supervisor. To remain up to date with all mandatory training requirements. To undertake CPD as agreed with Team Leader. To

As Learning Support Assistant, your responsibilities will include:

• Attending to the child's personal needs and implementing related personal programmes, including social, health, physical hygiene and welfare matters
• Working with the class teacher and Inclusion Manager to support the implementation of person centred planning and targets for SEND children
• Liaising with appropriate external SEND or health professionals to ensure that advice on provision for individual SEND children is effectively implemented

Clinical Deputy Manager – Wanstead, East London

Location: Chestnut Manor Care Home, 63 Cambridge Park, London, E11 2PR

Salary: £50,000 to £55,000 per annum (depending on experience)

Hours: 40 hours per week

We have an exciting opportunity for an experienced Clinical Deputy Manager to join our team on a full-time, permanent basis at our luxury care home in Wanstead, East London.

As the Clinical Deputy Manager, you will work closely with the Home Manager to oversee the clinical operations of the home, ensuring the health, safety and well-being of our residents. You will lead and inspire the care team, oversee clinical operations and ensure adherence to regulatory standards.

About the role:

• Assist the Home Manager with the day-to-day running of the home, ensuring a high standard of care is provided
• Lead and supervise the nursing and care staff, providing clinical expertise and guidance
• Oversee the care planning process, ensuring residents' individual needs are met with dignity and respect
• Monitor clinical care standards, ensuring compliance with relevant legislation, regulations and policies
• Support the recruitment, training and development of staff to maintain a skilled and motivated team
• Conduct regular audits, including medication management and care plans, to ensure best practices
• Act as a role model for excellent clinical care and leadership
• Build positive relationships with residents, families and healthcare professionals
• Step into the Home Manager’s role when they are absent, ensuring continuity of care and management

About you:

• Qualification as a Registered General Nurse with a current and valid NMC pin number
• Proven management experience within a care home setting
• Strong clinical skills with an understanding of CQC requirements, Safeguarding of Vulnerable Adults, HSE (Health & Safety Executive) requirements and other relevant legislation
• Excellent communication with a positive, flexible and innovative attitude
• Well organised with the ability to multi task in a fast-paced environment
• Motivated to make a difference
