What does a live-in carer do?
As a professional live-in carer in the South West of England, you’ll provide personalised care that enables clients to stay in their own homes. We offer flexible rota patterns to help you balance work and personal life, and we welcome carers with at least six months of professional experience. You’ll stay with your client during your chosen rota pattern, caring for one person (or sometimes a couple) and following a tailored care plan. We are looking for a Community Support Coordinator to join our amazing team in the Tayside area. This role is focused on making sure that people affected by chest, heart and stroke conditions get the help they need and get back to doing the things that matter to them.

Through our Community Support Services, you will support people to have the confidence and tools to self-manage their health condition and live as independently as possible. You’ll also help people get back to doing the things that are important to them – from simple things like walking to their local shop following a stroke, to returning to a sky-high hobby of gliding!

The following are the core responsibilities of the Care Navigator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: a. Deal with all general enquiries b. Provide an effective and efficient reception service to patients and all other visitors to the practice. c. Maintaining and monitoring the practice appointment system d. Process personal, telephone and e-requests for appointments e. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately f. Signpost patients to the correct service g. Initiating contact with and responding to, requests from patients, team members and external agencies h. Data entry of patient registrations and relevant patient information as required i. Input data into the patients healthcare records as necessary j. Ensuring all letters, reports, results etc are scanned and filed promptly, ensuring that all recent correspondence is available when patients are seen. k. Creating clinical referrals to the District Nurse team, Community Matron and Secondary Care via the e-referral system l. Manage all queries as necessary in an efficient manner m. Carry out system searches as requested n. Summarise new patient medical records o. Process patient deductions via the PCSE portal p. Maintain a clean, tidy, effective working area at all times q. Monitor and maintain the reception area and notice boards r. Support all clinical staff with general tasks as requested s. Requesting patient investigations via the ICE system.
