The Case Management programme provides case management and counselling services for up to 14 months for both Community-Based Programme (CBP) and Direct Release (DR) prison-referred clients.
Assistant Case Manager is responsible to perform all the administrative duties of the CMS programme such as handling of client data, submission of relevant reports and statistics and providing support for events and activities.
Programme Support
· Liaise with Prisons to update and maintain the referral list and for the submission of required reports
· Update and maintain client databases
· Vet and compile monthly statistics and payment submissions
· Prepare materials necessary for the various activities associated with the programme
· Perform secretarial support during meetings
· Attend to walk-in and call-in (non-CMS) clients on a preliminary basis
· Conduct initial interview to assess needs of (non-CMS) clients
· Accompany and support case managers for home visits with clients/ family members
· Carry out other duties assigned by the Head/SCM of CMS
Event and Activity Management
· Coordinate and facilitate various activities for clients, such as community service events and family programmes.
· Provide administrative support when liaising with various organisations/agencies
· Prepare materials for the various events/activities
